The purpose of this study is to evaluate the demographics of patients with orbital blow-out fractures, as well as the success and complications of surgical repair with porous polyethylene membrane sheet implants through transconjunctival technique and to compare the results to previously published studies.

This retrospective study included 57 patients diagnosed with orbital blow-out fractures referred to our clinic between 2018 and 2022. Seventeen patients (29.8%) underwent orbital fracture repair through a transconjunctival technique employing porous polyethylene membrane sheets. The indications for surgery were enophthalmos >2 mm and persistent ocular motility restriction, diplopia, and strabismus. The success criteria were <2 mm of enophthalmos and complete correction of eye motility, diplopia, and strabismus.

The study group consisted of ten females and 47 males with a mean age of 31.12 years. Post-operative vision, intraocular pressure, and hypotony maculopathy recovered very quickly. After incisional surgeries, patients should be warned against both external trauma and minor traumas such as eye rubbing and Valsalva maneuver.
The aim of this study was to examine the clinical and demographic impact of the COVID-19 pandemic on emergency department admissions to a tertiary eye care hospital.

Records of admissions to the ophthalmology emergency department during the pre-pandemic period (Group 1) and pandemic period (Group 2) were retrospectively reviewed to cover the period between March 15, 2019, and March 15, 2021. Application numbers, demographic characteristics of patients were recorded. The cases were grouped by age, sex and diagnoses, and the findings were compared within and between the groups.

A total of 161, 941 patients (Group 1 103, 178 and Group 2 58, 763) were admitted to the emergency department of our hospital within 2 years. All admission diagnoses were significantly lower in the pandemic period than in the pre-pandemic period (p=0.001). However, the rate of sight-threatening cases or cases requiring urgent treatment (retinal diseases, uveitis, etc.) was significantly higher in the pandemic period than in the pre-pandemic period (p=0.001). The most common admission diagnoses were ocular infectious/inflammatory diseases while the foreign body on the ocular surface/ocular trauma group was second. Admissions for allergic and infectious diseases were significantly less during the pandemic period (p=0.001 and p=0.002, respectively). In both periods, the number of admissions of male patients was significantly higher than that of females. The most frequent applicant age group was the 17-40 years age group.

This was a case study carried at tertiary eye hospital in Kathmandu, Nepal. In this study, 2 subjects presenting with complain of sudden onset of diminution of vision for near and distance along with asthenopic symptoms having history of prolonged near work were recruited. Pre-cycloplegic refraction showed fluctuating myopic refractive error which after cycloplegia showed a significant hyperopic shift. Single vision plus lens (base on post-mydriatic treatment) giving optimal vision for near and distance along with tropicamide 1% twice a day basis and abstinence of triggering factor was started as initial treatment modality. Subjects were kept under close surveillance and followed up fortnightly over 2 months. During this period, the dosages of tropicamide were gradually tapered based on symptomatic relief of patient and no recurrence of the condition was observed in both subjects for next 2 month.Spoken dialogue systems must be able to express empathy to achieve natural interaction with human users. As a first step toward solving this problem, rather than generating laughter from user dialogue, we focus on "shared laughter," where a user laughs using either solo or speech laughs (initial laugh), and the system laughs in turn (response laugh). The proposed system consists of three models 1) initial laugh detection, 2) shared laughter prediction, and 3) laugh type selection. We trained each model using a human-robot speed dating dialogue corpus. For the first model, a recurrent neural network was applied, and the detection performance achieved an F1 score of 82.6%. The second model used the acoustic and prosodic features of the initial laugh and achieved a prediction accuracy above that of the random prediction. The third model selects the type of system's response laugh as social or mirthful laugh based on the same features of the initial laugh. We then implemented the full shared laughter generation system in an attentive listening dialogue system and conducted a dialogue listening experiment. The proposed system improved the impression of the dialogue system such as empathy perception compared to a naive baseline without laughter and a reactive system that always responded with only social laughs. We propose that our system can be used for situated robot interaction and also emphasize the need for integrating proper empathetic laughs into conversational robots and agents.Retest-positive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) viral RNA, as a unique phenomenon among discharged individuals, has been demonstrated to be safe in the community. Still, the underlying mechanism of viral lingering is less investigated. In this study, first, we find that the frequency of viral RNA-positive retesting differs among variants. Higher ratios of viral RNA-positive retest were more frequently observed among Delta (61.41%, 514 of 837 cases) and Omicron (39.53%, 119 of 301 cases) infections than among ancestral viral infection (7.27%, 21 of 289 cases). Second, the tissues where viral RNA reoccurred were altered. Delta RNA reoccurred mainly in the upper respiratory tract (90%), but ancestral virus RNA reoccurred mainly in the gastrointestinal tract (71%). Third, vaccination did not reduce the frequency of viral RNA-positive retests, despite high concentrations of viral-specific antibodies in the blood. Finally, 37 of 55 (67.27%) Delta-infected patients receiving neutralizing antibody therapy become viral RNA retest positive when high concentrations of neutralizing antibodies still patrol in the blood. Altogether, our findings suggest that the presentence of high titers of neutralizing antibodies in the blood is incompetent in clearing residual viral RNA in the upper respiratory tract.
The clustering of metabolic abnormalities may weaken vaccine-induced immunity, but epidemiological data regarding SARS-CoV-2 vaccines are scarce. The present study aimed to examine the cross-sectional association between metabolic syndrome (MetS) and humoral immune response to Pfizer-BioNTech vaccine among the staff of a research center for medical care in Japan.

Participants were the staff (aged 21-75years) of the National Center of Global Health and Medicine who had completed the second dose of Pfizer-BioNTech vaccine 1-3months before the survey. MetS was defined according to the Joint Interim Statement. SARS-CoV-2 spike immunoglobulin G (IgG) antibody was measured using quantitative assays. Multivariable linear regression was used to estimate the geometric mean titers (GMT) and geometric mean ratio (GMR) of IgG titers, relative to MetS status.

Of 946 participants who received the second vaccine dose, 51 (5.4%) had MetS. Those with MetS had a significantly lower IgG titer (GMT 4125; 95% confidence interval [CI], 2885-5896) than those without MetS (GMT 5348; 95% CI, 3914-7309); the GMR was 0.77 (95% CI 0.64-0.93). Taking those having no MetS component as reference, fully adjusted GMR (95% CI) for those having 1, 2, 3 or ≥ 4 components was 1.00 (0.90, 1.11), 0.89 (0.77, 1.04), 0.86 (0.68, 1.10) and 0.61 (0.45, 0.82), respectively (P trend = 0.024).

A high-fat diet (HFD) induced perturbation of gut microbiota is a major contributory factor to promote the pathophysiology of HFD-associated metabolic syndrome. TheHFD could also increase the susceptibility tothe microbial infections warranting the use of antibiotics which are independently capable of impacting both gut microbiota and metabolic syndrome. Further, the usage of antibiotics in individuals consuming HFD can impact mitochondrial function that can be associated with anelevated risk of chronic conditions like inflammatory bowel disease (IBD). Despite this high propensity to infections in individuals on HFD, the link between duration of HFD and antibiotic treatment, and its impact on diversity ofthe gut microbiome and features of metabolic syndrome is not well established. In this study, we have addressed these knowledge gaps by examining howthe gut microbiota profile changes in HFD-fed mice receiving antibiotic intervention in the form of amoxicillin.We also determine whether antibiotic treatment in HFD-fed mice may adversely impact the ability of immune cells to clear microbial infections.
