The atypical dysfunction of all tendons in the leg excepting Achilles tendon and plantaris tendon following irreparable sciatic nerve palsy was presented. Tendon transfer using medial gastrocnemius tendon, lateral gastrocnemius tendon and plantaris tendon seemed to be a good choice for treatment of this injury. It allowed reconstruction of a stable, painless, plantigrade foot.

Kommerell diverticulum is a very rare congenital defect of the aortic arch associated with the aberrant subclavian artery. It can present with signs of dysphagia, chest pain, or distal embolization in the upper limb.
We present a case of Kommerell diverticulum with associated large subclavian artery aneurysm in a male patient with chest pain of unknown origin and hypertension. There was an incidental finding of the wide mediastinum on chest X-ray and the patient had a difference in systolic blood pressure in both arms. A right thoracotomy incision was used to successfully excise the aneurysm and reconstruct the subclavian artery. Patient recovery was uneventful.
Endovascular approaches are also an alternative to conventional open surgeries in the treatment of Kommerell diverticulum.

Traumatic amputation of the penis is a rare surgical emergency. Penile amputation is usually caused by self-mutilation, accidents, circumcision, assault and animal attacks. mTOR inhibitor Accidental injury covers a large portion of external genitalia trauma because of its high prevalence and severity of this disease. Here, we report the case of a 21-year-old man who underwent replantation of his self-inflicted partial amputated penis.
We report a case of traumatic penile partial amputation in a 21-year-old man with a history of mental retardation that presented with a one-day history of pain on the penile shaft due to tied penile shaft with a rubber band ten days prior. Genitalia examination showed a partial amputation at the penis shaft region. Reconstruction microsurgery and debridement on the penile shaft and urethral anastomosis were performed. This case highlights the management of traumatic penile partial amputation. The urethral anastomosis and penile replantation were successfully done.
Penile amputation is a rare urological emergency. Most of the cases reported with self-mutilation are a result of severe substance-induced psychosis or underlying psychiatric disorder. Factors that contribute to the successful penile replantation include the severity of the penile injury or amputation, type and mechanism of injury, team expertise available, duration of ischemia time, and use of a microscope at the time of neurovascular bundle repair.

A melanoma can originate at the subcutis without any visible skin lesion.
A 73-year old patient came to the outpatient clinic with a subcutaneous nodule on the right thigh without any visible lesion of the skin. It turned out to be a primary subcutaneous melanoma that could be classified as a primary dermal melanoma (PDM).
A PDM is a very rare subtype of melanoma that stands out for its excellent prognosis in comparison to cutaneous melanomas. No valid reliable staging system or treatment guideline exists for this entity, Breslow depth might overestimate the clinical aggressiveness possibly leading to overtreatment.
It is of great importance for the clinician to be familiar with a primary dermal melanoma. It deserves an appropriate place in the current AJCC system and a treatment guideline for this unique melanoma subtype with relativity excellent prognosis would be beneficial.

A dedicated drill bit that can achieve low-trauma and self-centring drilling is in urgent need. In this study, a three-step orthopaedic low-traumatic drill bit design was proposed. In order to evaluate the drilling performance of the proposed drill, comprehensive comparison tests were carried out with various commercial medical drills in terms of skiving force, thrust force, temperature rise, and surface quality. The experimental results show that the proposed three-step drill design with the optimal point angle, a small chisel edge, transition arc and web thinning can obtain lower and more stable thrust force, slighter bending force, smaller temperature rise, and higher hole quality compared with the commercial drill bits. The proposed drill shows satisfactory drilling performance and has great application potential in clinical surgery.Growth-preserving spinal surgery suffer from high complications rate. A recent bipolar instrumentation using two anchoring points (thoracic and pelvic) showed lower rates, but its biomechanical behaviour has not been characterised yet. The aim of this work was to combine in vitro and in vivo data to improve and validate a finite element model (FEM) of the spine, and to apply it to compare bipolar and classical all-screws implants. Spinal segments were tested in vitro to measure range of motion (ROM). Thoracic segments were also tested with bipolar instrumentation to measure ROM and rod strain using a strain gage. A subject-specific FEM of the spine, pelvis and ribcage of an in vivo asymptomatic subject was built. Spinal segments were extracted from it to reproduce the in-vitro mechanical tests. Experimental and simulated ROM and rod strain were compared. Then, the full trunk FEM was used to compare bipolar and all-screws instrumentations. The FEM fell within 1° of the experimental corridors, and both in silico and in vitro instrumentation rods showed 0.01% maximal axial strain. Bipolar and all-screws constructs had similar maximal Von Mises stresses. This work represents a first step towards subject-specific simulation to evaluate spinal constructs for neuromuscular scoliosis in children.Starch retrogradation involves nucleation and crystal growth steps, while their relative contribution to the overall retrogradation kinetics and relations with starch fine molecular structures have not been elucidated. In this study, a consecutive reaction kinetics model (CRK) was developed to fit long-term retrogradation kinetics curves for 10 rice starches with distinct molecular structures. Starch chain-length distributions (CLDs) and melting enthalpy kinetics curves for these starches were obtained from our published data. It was shown that these melting enthalpy kinetics curves can be satisfactorily deconvoluted by the CRK model into a combination of nucleation and crystal growth curves. Correlation analysis between CRK model-fitted parameters with starch CLDs showed that starch retrogradation nucleation and crystal growth steps were controlled by distinct starch fine molecular structures. These results have practical applications, as it enables a separate regulation of nucleation and crystal growth steps during retrogradation process of starch-based foods for desirable nutritional properties.Panax notoginseng leaves (PNL) was considered as a promising functional food ingredient with abundant protopanaxdiol ginsenosides. In this study, the influence of different drying methods on chemical components in PNL was characterized by a newly developed heart-cutting 2D-LC-HRMS. Our data indicates that vigorous ginsenoside transformation occurs in PNL processed by sun-air drying and hot-air drying (HAD) at 50 °C, but not shade-air drying (SAD), HAD at 25 °C and steaming prior to drying (SD). Specifically, the main components of PNL, ginsenosides Rb3, Rc, Rb2, Rb1 and Rd, can be transformed into notoginsenosides Fd and Fe, ginsenoside Rd2, Gypenoside XVII and ginsenoside F2, respectively, by highly selective cleavage of β-1,2-glucosidic linkage at the C-3 position. Only SD can inactivate the proteins that mediate this transformation. Different drying methods also greatly affect the quality of PNL products extracted by the conventional decoction method. These findings offer the scientific basis to design industrial drying methods for ensuring the quality of PNL.Applications of mass spectrometry-based metabolomics in food science have developed fast in the last decade. Sample preparation and data processing are critical in non-target/metabolomic workflows but there is currently no standardized protocol for the development of these methods. The impact of data processing parameters or the inclusion of a different matrix is not often taken into account during the selection of an extraction. Thus, this study aimed to investigate the impact of different extractions, e.g., QuEChERS, and data processing on the determination of malachite green metabolites in two different organisms, brook trout and shrimp. The results obtained confirm the need for a harmonized approach for the validation of non-target workflows, as depending on the comparison criteria, the matrix, the mode of ionization or data processing, a different extraction could be chosen. This study also identified for the first time des-methylated leucomalachite green as another metabolite in the two organisms.
We aimed to explore the diagnostic and therapeutic efficacy of cystoscopy with hydrodistension and random biopsies for clinically suspected interstitial cystitis/bladder pain syndrome (IC/BPS).
We reviewed the data of fifty-five clinically suspected IC/BPS patients underwent cystoscopy with hydrodistension and random biopsies. Global Response Assessment was used to evaluate the efficacy. Disease severity was assessed by thorough history, physical examination, 3-day frequency volume chart, visual analog scale of pain, Interstitial Cystitis Symptom Index (ICSI) and clinical phenotype system (UPOINT).
According to the pathologic outcomes from random biopsies, three out of the 55 clinically suspected IC/BPS were diagnosed as bladder carcinoma. Among the 52 IC/BPS patients, thirty-six patients (69.2%) had initial chief complaints of urinary frequency and urgency. Under cystoscopy, twenty-nine patients and 23 patients were classified as Hunner ulcer type and diffuse global mucosal bleeding (grade III glomerulation).
