Services became less permeable, such that finding a point of entry to those services that remained open required more work of service users and carers. Healthcare professionals were routinely confronted by complex decisions and ethical dilemmas about provision of care, and their implicit judgements about access may have important implications for equity. Many of the challenges of access exposed by the pandemic related to pre-existing resource deficits and institutional weaknesses in care for people living with mental health difficulties. Overall, these findings affirm the value of the construct of candidacy for explaining access to mental healthcare, but also enable deepened understanding of the specific features of candidacy, offering enduring learning and implications for policy and practice.
Unequal distribution of access to resources would often be highlighted after disasters, and may have impact on child mental health. We aimed to elucidate the association between perception of recovery process (dissatisfaction and perceived inequality) and child mental health.
Data from the Great East Japan Earthquake Follow-up for Children (GEJE-FC) study targeting children (aged 4-6 years at the time of the disaster) and their siblings and parents from three affected prefectures (Miyagi, Fukushima and Iwate) in Japan, from August 2012 to January 2018, were analyzed. Dissatisfaction and perceived inequality of recovery process, parental mental health (Kessler Psychological Distress Scale (K6) and happiness) and social capital, and child mental health (Depression Self-Rating Scale for Children) were assessed via self-rated questionnaires. Vulvodynia (chronic vulvar pain) is a sexually debilitating disorder with a prevalence of ∼10%.
To investigate the effectiveness of therapy with local anesthetics (TLA) in women with severe vulvodynia, we conducted a prospective, non-controlled observational study.
45 patients with severe chronic vulvodynia (primary and secondary vulvodynia, 0-10 numeric analogue scale (NAS) ≥6, median 7.9, duration ≥6 months, median 65.2 months) in an outpatient practice in Germany were treated with TLA in 3-12 sessions using procaine 1% as local anesthetic. Effectiveness was analyzed with Wilcoxon signed rank tests and Wilcoxon rank sum tests.

TLA successfully reduced vulvodynia in 36 of 45 patients (80 %, responders). The NAS reduction was from 7.9 to 2.4 (P < .001). Here, we generated an induced pluripotent stem cell line using episomal plasmid vectors from the peripheral blood mononuclear cells of a 48-year-old male with otosclerosis. The morphology and karyotype of the cells were normal. The expression of pluripotency markers was verified by mRNA and protein levels; the pluripotency state of the cell line was verified by successful differentiation into all three germ layers.Age-related macular degeneration (AMD) is a major cause of vision loss among the elderly in the Western world. AMD is multifactorial eye disease with a strong genetic contribution. Here, we report the generation and characterization of induced pluripotent stem cells (iPSCs) derived from peripheral blood mononuclear cells of three patients with AMD carrying rare variants in the complement factor H (CFH) gene. These cell lines were generated for cellular studies investigating the disease mechanisms and developing therapeutic interventions for AMD.
Multiple sclerosis (MS) is a chronic autoimmune inflammatory disease of the central nervous system, affecting more than 2.3 million people worldwide. Fatigue is among the most common symptoms in MS, resulting in reduced mobility and quality of life. The six-minute walking test (6MWT) is commonly used as a measure of fatigability for the assessment of state fatigue throughout treatment or rehabilitation programs. This 'gold standard' test is time-consuming and can be difficult and exhausting for some patients with high levels of disability or high rates of fatigue.
Can short inertial sensor-based gait tests assess perceived state fatigue in MS patients?
Sixty-five MS patients equipped with one sensor on each foot performed the 6min walk test (6MWT) and the 25-foot walk (25FW, at both preferred and fastest speed). Perceived state fatigue was measured after each minute of the 6MWT, using the Borg rating. The highest of these ratings served as a measure of overall perceived state fatigue. It is possible to use shortened gait tests when evaluating perceived state fatigue in MS patients using inertial sensors. Substituting them for long gait tests may reduce the burden of the testing on both patients and clinicians. Further, the approach taken here may prompt future work to explore the use of short bouts of real-world walking with unobtrusive inertial sensors for state fatigue assessment.
Myelin oligodendrocyte glycoprotein antibodies (MOG-Abs) are detected at a high rate in childhood of acquired demyelinating syndrome (ADS), but the spectrum and characteristics of MOG-Abs-associated disorders (MOGAD) in children are to be determined. This study aimed to identify clinical features in Chinese children with MOGAD.
Of 48 children in whom MOGAD were diagnosed in our hospital, we analyzed the manifestations, laboratory test results, imaging characteristics, autoimmune antibodies in cerebrospinal fluid and serum, and response to treatment. We used a cell transfection immunofluorescence assay to test for MOG-Abs in serum.
Of the 48 children, the most common phenotypes were acute disseminated encephalomyelitis (ADEM) (20/48, 41.7%) and optic neuritis (ON) (13/48, 27.1%). The onset ages of ON were significantly higher than those of ADEM (8.68±2.86 & 4.80±2.77, P<0.01). Cerebral lesions manifested as ADEM-like, leukodystrophy-like and other patterns. All children received first-line immunomsed as the only basis for recurrence and long-term immunoregulatory treatment. Most children had a good prognosis, however, the phenotype of ADEM-ON at onset was tend to relapse, sometimes with cognitive impairment, epilepsy, and other sequelae. Repeated ON could cause visual impairment.

Personality traits affect the cognitive functions, mood, and psychological well-being of people with MS and their families. Objective The aim of this study was to evaluate whether personality traits might be predictors of cognitive recovery in people with MS. Methods. Thirty-one patients with a diagnosis of MS were enrolled in this study. Each participant was evaluated by a neuropsychologist before (T0) and after the end of the rehabilitation treatment (T1). Conclusion Our data suggest that personality traits could influence cognitive and behavioral recovery in patients with MS.
A correlational relationship has been well established between Multiple Sclerosis and Acute Myocardial Infarction incidence. However, the etiology underlying this relationship remains unclear. The purpose of this study is to investigate the mechanisms behind the relationship by identifying candidate genes in the interaction between the two diseases.
Using a computational biology approach and existing gene expression data from the NIH Gene Expression Omnibus, meta-analysis was conducted on six datasets to evaluate upregulated or downregulated genes shared between both diseases. Overlapping genes were then evaluated in STRING to find a KEGG biological pathway connecting these genes.
Meta-analysis found 78 overlap genes for upregulation and 65 for downregulation. These genes displayed significant interaction in the STRING network. The most plausible KEGG pathway resulting from the STRING analysis was the Interleukin-17 Signaling Pathway. Seven of the genes from the meta-analysis (S100A8, S100A9, CXCL8, COX2, AP-1, IKBA, and A20) are involved in this pathway.
