Objectives Patients with serious injury need special care and treatment to control the infection, as wound sepsis is one of the major causes of death. The objective of this study was to develop natural polymeric (chitosan and gelatin) hydrogel sponges containing SSD and evaluate its efficacy in wound healing using animal models. Methods SSD containing hydrogel sponges were prepared by solvent casting technique. Scanning electron microscopy (SEM) and Differential scanning calorimetry (DSC) were used to evaluate morphological characteristics of the hydrogel sponges. Anti-thrombogenic property, drug release studies, drug release kinetics, antimicrobial property, and wound healing effect were also studied in detail. Results The optimized batch of hydrogel sponges (CG4) consists of 1% SSD wt., 10% wt. Gelatin, 1% wt. Chitosan and honey 7.5% wt. as plasticizer. At the 12th hour, in vitro and ex vivo drug release was found to be 76.994±0.67% and 24.22±0.57% respectively. CG4 batch had enhanced in vitro antimicrobial activity as compared to conventional marketed cream. The developed SSD hydrogel sponges showed a faster rate of wound healing as compared to a marketed cream. Animals treated with CG4 formulation showed complete angiogenesis and re-epithelialization by 8th day, whereas 12 days were required for complete wound healing with marketed cream. Conclusions The prepared hydrogel sponges can serve as a potential alternative for wound healing dressing as compared to the marketed product.Objectives Prediction of the antipsychotic's effectiveness is a relevant topic in the field of personalized medicine. Methods The research design of this study is a prospective observation with posthoc analysis of associations of genetic polymorphisms with safety parameters and effectiveness of antipsychotic therapy. We observed 53 adolescents with an acute psychotic episode which were prescribed antipsychotics for 14 days. We evaluated the effectiveness of antipsychotics with the Positive and Negative Symptoms Scale and the safety with the UKU Side Effects Rating Scale, Simpson-Angus Scale, and Barnes Akathisia rating scale. We genotyped CYP3A4*22 (rs2740574), CYP3A5*3 (6986A>G, rs7767746), CYP2D6*4, *9, *10 (rs3892097, rs1065852), ABCB1 1236C>T (rs1128503), 2677G>T/A (rs2032582), 3435C>T (rs1045642), DRD2 (rs1800497), DRD4 (rs1800955), HTR2A (rs6313) by the real-time polymerase chain reaction method. Results We found significantly more frequent "increased dream activity" between CYP2D6 intermediate metabolizers and normal metabolizers (54 vs. 22%; p=0.043). The «increased duration of sleep» was more often observed in homozygotes TT of ABCB1 2677G>T/A (50 vs. 15.8%, p=0.006) and TT of 3435C>T (41.7 vs. 8.2%, p=0.007). Conclusions We found that CYP2D6 and ABCB1 polymorphisms were associated with the safety of antipsychotics in adolescents with an acute psychotic episode.Internet of Things (IoT) plays a prominent role in health-care of patients, which assist the physicians and patients through the assistance in effective decision-making and additionally, in the medical field, IoT plays a significant role in real-time monitoring of the patients. Even though the data provided by the IoT devices ensure the effective decision-making, the data is susceptible to the network attacks. Thus, the paper proposes an authentication protocol for enabling the secure data transmission in IoT based on three functions, such as encryption function, hashing function, and adaptive XOR function. The proposed authentication protocol is named as, Adaptive XOR, hashing and Encryption Key Exchange (AXHE) protocol, which is the combination of the functions, such as encryption function, hashing function, and adaptive XOR function. The protocol ensures the security in the communication through two successive phases, such as registration and authentication of the user, where the user name, password, public keys, private keys, and security factor are employed. The authentication is progressed as seven levels and whenever the security factor matches, the user is authenticated and the communication continues. The analysis of the proposed AXHE is performed using 50 and 100 nodes in the presence of DOS and black hole attacks, which acquires the detection rate, throughput, and detection delay of 0.3859, 0.32, and 6.535 s, respectively.The mitochondrial permeability transition (MPT) has been one of the longstanding enigmas in biology. Its cause is currently at the center of an extensive scientific debate, and several hypotheses on its molecular nature have been put forward. The present view holds that the transition arises from the opening of a high-conductance channel in the energy-transducing membrane, the permeability transition pore (PTP), also called the mitochondrial megachannel or the multiconductance channel (MMC). Here, the novel hypothesis is proposed that the aqueous access channels at the interface of the c-ring and the a-subunit of FO in the FOF1-ATP synthase are repurposed during induction of apoptosis and constitute the elusive PTP/ MMC. A unifying principle based on regulation by local potentials is advanced to rationalize the action of the myriad structurally and chemically diverse inducers and inhibitors of PTP/MMC. Experimental evidence in favor of the hypothesis and its differences from current models of PTP/MMC are summarized. The hypothesis explains in considerable detail how the binding of Ca2+ to a β-catalytic site (site 3) in the F1 portion of ATP synthase triggers the opening of the PTP/MMC. It is also shown to connect to longstanding proposals within Nath's torsional mechanism of energy transduction and ATP synthesis as to how the binding of MgADP to site 3 does not induce PTP/MMC, but instead catalyzes physiological ATP synthesis in cell life. In the author's knowledge, this is the first model that explains how Ca2+ transforms the FOF1-ATP synthase from an exquisite energy-conserving enzyme in cell life into an energy-dissipating structure that promotes cell death. This has major implications for basic as well as for clinical research, such as for the development of drugs that target the MPT, given the established role of PTP/MMC dysregulation in cancer, ischemia, cardiac hypertrophy, and various neurodegenerative diseases.The article considers, on the basis of analysis of declassified archival documents, issue of the Soviet government regulating price of penicillin in the second half of the 1940s in conditions of continuing drug shortage. The factors that affected formation of selling and retail prices of antibiotic are considered. Based on the calculation of cost of treatment of a number of nosologies in different age groups and comparing it with the level of income of the population, the conclusion is derived that real availability of antibiotics for population in the USSR was achieved by early 1950s.On the eve of the 75th anniversary of the Victory in the Great Patriotic War, one should recall the great neurosurgeon, scientist, organizer of military medical science, founder of prominent scientific school, academician N. N. Burdenko. His first experience as direct participant in warfare occurred in 1904 when he was medical student. He went through four wars being permanently at the front line. He carried out extremely complicated surgery operations, improved medical care and organized evacuation of the wounded. During the Great Patriotic War 1941 -1945, he, being the chief surgeon of the Red Army, headed all activities of organization of surgical care, came out to the troops, trained military surgeons to emergency interventions, developed teaching of wound, developed methods of surgical treatment of blood vessels, traumatic shock, introduced application of antibiotics (penicillin and gramicidin), developed methods of surgical treatment of oncologic diseases of central and vegetative nervous system, improved technique of surgical interventions on spinal cord roots as well as on cerebellum, bulbectomy operations, etc. The academician N. N. Burdenko as Stalin Prize laureate, became one of founders and the first President of the USSR Academy of Medical Sciences. The contribution of academician N. N. Burdenko in the cause of victory in The Great Patriotic War is priceless.The article presents the results of study concerning improving medical care quality in rural areas. The purpose was to investigate the characteristics of medical personnel training in the framework of the "Zemstvo Doctor" program. The activities of the University were analyzed concerning implementation of professional training of medical personnel and including continuing medical education. To achieve this goal, efficiency of educational programs development considering contribution of the "Zemstvo doctor" program implementation was evaluated. The study was carried out in 2013-2019 on the basis of the Medical Institute of Belgorod State National Research University. The structured interview was applied as methodology of collecting primary data. The fact of decreasing of medical profession prestige that occurs against the background of implementation of the state program "Zemstvo Doctor" was established. The overcoming of this situation is necessary to improve medical care quality in rural areas. The focusing on existing regional features of medical care support and taking into account characteristics of health of population are required. In this respect the key factor is the training of rural physicians to employ high-tech technologies, including digital simulation and telecommunications equipment. The relevance of the state program "Zemstvo Doctor" is obvious, but the medical education system has no official instructions concerning the need of correcting educational programs. The efficiency of changes applied to educational programs was determined by subjective assessment of the need and the clinical experience of physicians-lecturers who are involved in resolving scientific and practical problems of health care in rural areas.The problem of physicians-paramedical personnel interaction has long history. There are such reasons for misunderstanding as actual stereotypes and arrogant attitude on the part of physicians, low assessment of medical nurse professional activities, gender inequality. The system of medical care becomes more complicated depending on advanced technologies, sophisticated specialization and disregard to human personality relations that complicates adaptation of specialists and team relationships. Therefore, it is important that all participants in patient medical care understand their individual roles and responsibilities, and realize their functions and actions affect all team members. The increasing tension within team results in decreasing in medical care quality and undermines confidence of patients to medical and paramedical personnel. These misunderstandings contribute to current shortage of nursing staff in Russia. The ways of resolving the "eternal conflict" based on national and international researchers experience are proposed.
