New therapeutic strategies focusing on Wnt pathway can provide an interesting approach for future treatments.Duodenal varices are ectopic varices that are rare but can involve any site along the digestive tract outside the gastroesophageal region. Ectopic variceal bleeding is generally massive and life threatening; the mortality rate is approximately 40%. Up to 17% of ectopic varices occur in the duodenum. However, duodenal varices pose a significant therapeutic challenge due to the lack of standard treatment guidelines. We report a case of duodenal variceal bleeding secondary to portal vein stenosis in a 77-year-old woman receiving chemotherapy for unresectable perihilar cholangiocarcinoma. The patient presented with melena, nausea, vomiting and unstable vital signs suggestive of hemorrhagic shock. Emergency esophagogastroduodenoscopy revealed large nodular varices with a ruptured erosion on top in the superior duodenal angle, and variceal bleeding had stopped by the time of the procedure. Subsequent computed tomography showed the development of portosystemic collaterals; therefore, we performed percutaneous portal vein stent placement to reduce portal vein pressure. Since persistent bleeding was suspected, we also performed endoscopic injection sclerotherapy and achieved successful hemostasis with an improvement in liver function. This case revealed that a combination of portal vein stent placement and endoscopic injection sclerotherapy might be an effective therapy for duodenal variceal bleeding caused by portal vein stenosis.A 55-year-old man was diagnosed with pancreatic cancer of the uncus and received chemotherapy (modified FOLFIRINOX). Ten months later, he was admitted to our hospital with massive lower gastrointestinal bleeding. Contrast-enhanced CT showed ascending colon varices caused by the occlusion of the superior mesenteric vein (SMV) due to pancreatic cancer invasion. Colonoscopy revealed tortuous varices with red spots in the ascending colon. The patient received blood transfusions and was discharged; however, he was hospitalized for recurrent massive lower gastrointestinal bleeding 3 months later. During this readmission, we performed the transileocolic vein obliteration method due to SMV stenosis and the absence of an obvious shunt. He experienced an uneventful post-operative recovery, and contrast-enhanced CT after 2 months revealed no recurrence of colonic varices. Ectopic varices are portosystemic venous collaterals resulting from portal hypertension occurring in any locations other than the esophagogastric region. Colonic varices have rarely been reported before. Patients with pancreatic cancer may present with gastrointestinal bleeding caused by tumor bleeding or esophagogastric varices; however, ectopic varices such as colon varices, a rare complication of pancreatic cancer, should be considered in patients with obscure gastrointestinal bleeding.
The studies IMvigor 210 cohort 2 and IMvigor211 evaluated the efficacy of atezolizumab in patients with locally advanced or metastatic urothelial cancer (mUC) upon progression to platinum-based chemotherapy worldwide. Yet, the real impact of this drug in specific geographical regions is unknown.
We combined individual-level data from the 131 patients recruited in Spain from IMvigor210 cohort 2 and IMvigor211 in a pooled analysis. Efficacy and safety outcomes were assessed in the overall study population and according to PD-L1 expression on tumour-infiltrating immune cells.
Full data were available for 127 patients; 74 (58%) received atezolizumab and 53 (42%) chemotherapy. Atezolizumab patients had a numerically superior median overall survival although not reaching statistical significance (9.2months vs 7.7months). No statistically significant differences between arms were observed in overall response rates (20.3% vs 37.0%) or progression-free survival (2.1months vs 5.3months). At nanomolar concentrations, FL probe was subjected to fluorescence quenching by Fe(III). Fluorescence quenching mechanism of FL by Fe(III) was examined using various analytical techniques such as UV-vis absorption, fluorescence, and Fourier transform Infrared spectroscopy techniques, as well as with scanning electron microscope. The results indicated that photoinduced electron transfer process occurred between FL and Fe and that FL was quenched both statically and dynamically. Thermodynamic parameters showed that the interactions between them were predominantly hydrophobic interactions. Enox caused FL to recover its lost fluorescence properties and an increase was observed in the intensity of the fluorescence. Enox was detected successfully with the turn on fluorescence sensor. The developed Enox biosensor exhibited linearity in the range of 0-1.1 μg/ml. For Enox detection, the limit of detection was measured as 255 ng/mL. Enox biosensor was presented as a practical, simple, and applicable sensor system with high sensitivity and good selectivity. Enox is a medication usually monitored indirectly over anticoagulation. It was presented as a new method for Enoxaparin to be followed directly.Gold nanoclusters (AuNCs) stabilized by glutathione (GSH) have been synthesized using a simple one-pot method, which were used as a fluorescence-enhanced probe for the detection of cysteine (Cys) and GSH. The detection is based on the finding that the weak yellow fluorescence of the AuNCs, with excitation/emission maxima of 430/600 nm, can be enhanced by Cys and GSH via NCs aggregation. This method is selective for Cys and GSH. According to the fluorescence enhancement, the detection ranges of AuNCs for Cys and GSH are 2.49 µM ~ 0.80 mM and 1.99 µM ~ 0.44 mM, with the detection limit of 0.42 µM and 0.27 µM, respectively. In addition, the probe has good anti-interference performance over other common biomolecules. The study assessed the feasibility of integrating the problematic water hyacinth plants into the current treatment process. Grab samples of influent and effluent for each pond were collected between 28 March and 23 April 2019 and the analysis was done following standard APHA methods. Parameters considered include pH, turbidity, TDS, TSS, TN, TP, BOD, and COD. The raw sewage mean pH, turbidity, TDS, TSS, TN, TP, BOD, and COD were 8.08, 580 NTU, 1639 mg/L, 1294 mg/L, 78 mg/L, 8.16 mg/L, 287 mg/L, and 887 mg/L. The mean pH, turbidity, TDS, TSS, TN, TP, BOD, and COD in the effluent from the existing maturation pond, control pilot pond, and water hyacinth pilot pond were 7.7, 7.7, and 7.3; 75, 67, and 47 NTU; 861, 758, and 668 mg/L; 276, 172, and 82 mg/L; 27, 28, and 17 mg/L; 4, 5.28, and 4 mg/L; 114, 52, and 30 mg/L; and 243, 122, and 81 mg/L. It was concluded that the water hyacinth may be integrated into the WSP system to enhance contaminant removal. The water hyacinth in the ponds should be harvested periodically to avoid secondary organic and nutrient loading from dead plants.Purpose The identification of abnormalities that are relatively rare within otherwise normal anatomy is a major challenge for deep learning in the semantic segmentation of medical images. The small number of samples of the minority classes in the training data makes the learning of optimal classification challenging, while the more frequently occurring samples of the majority class hamper the generalization of the classification boundary between infrequently occurring target objects and classes. In this paper, we developed a novel generative multi-adversarial network, called Ensemble-GAN, for mitigating this class imbalance problem in the semantic segmentation of abdominal images.Method The Ensemble-GAN framework is composed of a single-generator and a multi-discriminator variant for handling the class imbalance problem to provide a better generalization than existing approaches. The ensemble model aggregates the estimates of multiple models by training from different initializations and losses from various subsets of the training data.
