Caregivers of patients with chronic conditions or disability experience fatigue, burden and poor health-related quality of life. There is evidence of the effectiveness of support interventions for decreasing this impact. However, little is known about the benefits of home-based nursing intervention in primary health care.

To evaluate the effectiveness of a home-based, nurse-led-intervention (CuidaCare) on the quality of life of caregivers of individuals with disabilities or chronic conditions living in the community, measured at 12-month follow-up.

A pragmatic, two-arm, cluster-randomized controlled trial with a 1-year follow-up period was performed between June 2013 and December 2015. Consecutive caregivers aged 65 years or older, all of whom assumed the primary responsibility of caring for people with disabling conditions for at least 6 months a year, were recruited from 22 primary health care centers. Subsequently, 11 centers were randomly assigned to usual care group, and 11 were assigned to the intrease of 0.04 point (95% CI 0.01; 0.07) in the utilities. No statistically significant differences were found between the two groups at 12 months with respect to the secondary outcomes.

The findings suggest that incorporating a home-based, nurse-led-intervention for caregivers into primary care can improve the health-related quality of life of caregivers of patients with chronic or disabling conditions.

Increased survival of preterm neonates who require hospitalization at the Neonatal Intensive Care Unit has led to an increase in infections. This study aims to describe the temporal trend, risk factors, and outcome of healthcare-associated infections in a NICU of a high complexity hospital, with emphasis on the differences of incidence between bacterial and fungal infections.

The study was carried out from January 2013 to December 2016, with daily follow-up of the newborns by the National Healthcare Safety Network.

The study included 881 newborns, of whom 214 had a bacterial infection, 19 had fungi infection, and 12 bacterial and fungal infections associated. The hospital infection rate was 12/1000 patient-days. SNAPPE>24, days of hospitalization and PICC days were independent risk factors for the development of fungal and bacterial infection, respectively, with statistical significance for bacterial and fungal infections. The mortality rate was 2.6 times higher in those who had fungal infection than in those who had a bacterial infection. The occurrence of invasive infections was higher in the years 2015 and 2016 and Gram-negative bacteria and yeasts were more frequent, impacting morbidity and mortality.

These results point to the importance of constant epidemiological surveillance and measures of effective control of infections in NICU.

Policy interventions in the UK have focused primarily on pricing the disposal of waste through a landfill tax. This article tests the effectiveness of landfill taxation on household waste collection and disposal. Using a ridge regression with correction for fixed effects on a panel dataset of English local authorities, we estimate the landfill tax elasticity of waste for a range of waste disposal and collection streams. Results indicate that the landfill tax is elastic at disposal and effective in shifting waste from landfills to incineration. The tax is inelastic at collection, and encourages recycling. Our results indicate that landfill taxation is an important tool to change how waste is disposed by households and local authorities.Major depression is an episodic disorder which, for many individuals, has its onset in a distinct change of emotional state which then persists over time. The present article explores the utility of combining a dynamical systems approach to depression, focusing specifically on the change of state associated with episode onset, with a self-regulation perspective, which operationalizes how feedback received in the ongoing process of goal pursuit influences affect, motivation, and behavior, for understanding how a depressive episode begins. The goals of this review are to survey the recent literature modeling the onset of a depressive episode and to illustrate how a self-regulation perspective can provide a conceptual framework and testable hypotheses regarding episode onset within a dynamical systems model of depression.PM2.5 and PM10 fugitive dust samples from multiple sources (construction, demolition, industrial, agricultural fields, and bare ground) were collected in triplicate for each size bin, from 18 distinct locations in and around Bhopal, central India. The dust samples were dried, sieved, and re-suspended in a chamber fitted with a suitable sampling system, to collect PM2.5 and PM10 samples onto Teflon and Quartz filters. The filters were subjected to gravimetric and chemical analyses. Trace elements, water-soluble ions, and thermal-optical carbon fractions were quantified using a variety of analyses. These species were then used to develop PM10 and PM2.5 chemical source profiles of the fugitive dust sources. As expected, crustal species were abundant in all source categories. For industrial dust, Fe contribution to mass in both size fractions was about 11.4% and above the upper continental crustal abundance. Further, the source profiles generated for each source were different from their counterparts in the US EPA SPECIATE database and profiles reported in literature. Thus, it will be useful to utilize profiles generated in this study to enhance receptor model performance for the study region. However, collinearity analysis of the profiles revealed that PM10 agricultural and bare ground dust; and PM2.5 construction and demolition dust profile pairs may not be separated by receptor models. Finally, a human health risk assessment revealed that construction and industrial dust may pose significant risk to the population. The Incremental Lifetime Cancer Risk (ILCR) metric revealed that adults (2 × 10-5) and children (1 × 10-5) were susceptible to cancer risk from exposure to metals in PM2.5 fugitive dust. Further, children were more vulnerable than adults. This study summarized and evaluated effects of TDCs on fish thyroid sensitive endpoints including thyroid hormones, thyroid related gene expression, immunostaining for thyroid follicles, eye size and pigmentation, swim bladder inflation as well as effects of TDCs on secondary sex characteristics, sex ratio, growth and reproduction. Changes in thyroid sensitive endpoints may reflect the balanced outcome of different processes of the thyroid cascade. Thyroid sensitive endpoints may also be altered by non-thyroid molecular or endocrine pathways as well as non-specific factors such as general toxicity, development, stress, nutrient, and the environmental factors like temperature and pH. Defining chemical specific effects on thyroid sensitive endpoints is important for identification of TDCs. Application of the AOP (adverse outcome pathway) concept could be helpful for defining critical events needed for testing and identification of TDCs in fish.

Levetiracetam, an antiepileptic drug commonly prescribed for seizure prophylaxis in the neurosurgical ICU, undergoes predominant elimination via the kidneys. Hence, we hypothesize that current dosing practice of intravenous (IV) levetiracetam 500 mg twice daily is inadequate for critically ill patients due to enhanced drug elimination. The objectives of our study were to describe the population PK of levetiractam using a nonparametric approach to design an optimal dosing regimen for critically ill neurosurgical patients.

This was a prospective, observational, population PK study. Serial blood samples were obtained from neurosurgical ICU patients who received at least one dose of IV levetiracetam. We used uHPLC to analyze these samples and Pmetrics™ softwarequired higher doses to achieve target levels.Comprehensive two-dimensional gas chromatography with parallel mass spectrometry and flame ionization detection (GC × GC-MS/FID) enables effective chromatographic fingerprinting of complex samples by comprehensively mapping untargeted and targeted components. However, no nationwide human biomonitoring survey had been conducted to gather information on levels or reference values (RVs) of phthalates in the Taiwanese population. We aimed to establish the urinary levels and RVs of phthalate metabolites and identify exposure characteristics among Taiwan's population. We enrolled 1857 participants 7 years of age and older from the Taiwan Environmental Survey for Toxicants (TESTs) conducted during 2013-2016. Levels of 11 phthalate metabolites in each participant's urine samples were determined using liquid chromatography-tandem mass spectrometry. For all phthalate metabolites except for mono-methyl phthalate (MMP), mono-ethyl phthalate (MEP), and mono-ethylhexyl phthalate (MEHP), urinary median levels were significantly higher in the 7-17-year old group than in the ≧18-year-old group. For most phthalate metabolites and in the general population, the geometric mean decreased with increasing age.
