A lack of stability is also considered a risk factor for fracture-related infection (FRI), although confirmatory studies and an understanding of the underlying mechanisms are lacking. In the present study, we investigate whether biomechanical (in)stability can lead to altered immune responses in mice under sterile or experimentally inoculated conditions. In non-inoculated C57BL/6 mice, instability resulted in an early increase of inflammatory markers such as granulocyte-colony stimulating factor (G-CSF), keratinocyte chemoattractant (KC) and interleukin (IL)-6 within the bone. When inoculated with Staphylococcus epidermidis, instability resulted in a further significant increase in G-CSF, IL-6 and KC in bone tissue. S. aureus infection led to rapid osteolysis and instability in all animals and was not further studied. Gene expression measurements also showed significant upregulation in CCL2 and G-CSF in these mice. IL-17A was found to be up-regulated in all S. epidermidis infected mice, with higher systemic IL-17A cell responses in mice that cleared the infection, which was found to be produced by CD4+ and γδ+ T cells in the bone marrow. IL-17A knock-out (KO) mice displayed a trend of delayed clearance of infection (p=0.22, Fisher Exact Test) and an increase in interferon (IFN)-γ production. Little is known about the prevalence of aortic aneurysms among people living with HIV (PLWH). We investigated whether HIV status is independently associated with having aortic aneurysms. Furthermore, we determined risk factors associated with aortic aneurysms in PLWH.

PLWH aged ≥40 years (n = 594) were recruited from the Copenhagen Comorbidity in HIV Infection study and matched for age and sex with uninfected controls (n = 1188) from the Copenhagen General Population Study. Aortic dimensions were assessed using contrast enhanced computed tomography. Aortic aneurysms were defined according to the European Society of Cardiology guidelines, i.e. an aortic dilation of ≥50% or an infrarenal aortic diameter of ≥30 mm. Among PLWH and uninfected controls, the median (interquartile range) age was 52 (47-60) and 52 (48-61) and 88% and 90% were male, respectively. We found 46 aneurysms in 42 (7.1%) PLWH and 31 aneurysms in 29 (2.4%) uninfected controls (P < 0.001). PLWH had a significantly higher prevalence of ascending aortic aneurysms and infrarenal aortic aneurysms. In an adjusted model, HIV was independently associated with aortic aneurysms (adjusted odds ratio; 4.51 [95% confidence interval 2.56-8.08], P < 0.001). Within PLWH, obesity and hepatitis B co-infection were associated with aortic aneurysms.

Formerly incarcerated Americans are believed to have increased risk of hypertension and cardiovascular disease (CVD), yet the impact of lower-level criminal legal system exposures, such as arrests, on cardiovascular health are less clear.

We explored the relationship between lifetime history of arrest and self-report of ever having been diagnosed with hypertension or a heart condition using data from the 2018 National Survey on Drug Use and Health (NSDUH). Survey-weighted Poisson regression models with robust variance, adjusted for age group, sex, race/ethnicity, education, past year mental illness, smoking history, and past year substance use disorder, were used to estimate adjusted prevalence ratios.

Among the 13,583 respondents, 17.0% reported a history of arrest, among whom 45.2% also reported a hypertension diagnosis and 24.4% reported a heart condition. Among those without a history of arrest, 46.7% reported a hypertension diagnosis and 25.2% reported a heart condition diagnosis. The adjusted models did not show evidence that history of arrest is associated with self-reported hypertension (adjusted PR 1.0; 95% CI 0.93, 1.07, p=0.937) or self-reported heart condition (1.0; 95% CI 0.91, 1.11, p=0.915).

Of critical importance to harnessing these benefits is the metadata which describes general and other domain specific attributes (e.g., collection location, isolate type, etc.) of a sample. Unlike the sequence data, the metadata is often incomplete and lacks adherence to an international standard. We describe the problem posed by such variable and incomplete metadata in terms of interpretative labor costs (the time and energy necessary to make sense of the signal in the genetic data), and the impact such metadata has on foodborne outbreak detection and response. Improving the quality of sequence-associated metadata would allow for earlier detection of emerging food safety hazards and allow faster response to foodborne outbreaks.
Systemic lupus erythematosus (SLE) has been described as an independent risk factor for the development of cardiovascular (CV) disease. Recently, the QRESEARCH risk estimator version 3 (QRISK3) calculator has been launched for CV risk assessment in the general population. 296 patients with SLE were recruited. The presence of subclinical atherosclerosis was evaluated by carotid ultrasound to identify carotid plaque and the thickness of the carotid intima media (cIMT). QRISK3 and SCORE were calculated. The relationship of QRISK3 and SCORE with each other and with the presence of subclinical carotid atherosclerosis (both carotid plaque and cIMT) was studied.

There was no correlation between SCORE and QRISK3 in patients with SLE (Rho Spearman r= -0.008, p= 0.90). Although QRISK3 showed a statistically significant correlation with cIMT (Rho Spearman r = 0.420, p= 0.000), this relationship was not found between SCORE and cIMT (Rho Spearman r= -0.005, p= 0.93). The discrimination capacity of QRISK3 for the presence of carotid plaque was statistically significant and superior to that of SCORE (AUC 0.765 [95% CI 0.711-0.820] vs 0.561 [95% CI 0.494-0.629], p= 0.000).

To identify clinical and serological features that distinguish patients with systemic lupus erythematosus (SLE) who require single as opposed to repeated rituximab (RTX) cycles.

All 175 SLE patients followed-up at University College hospital from 2000 onwards were retrospectively reviewed. They were divided into a one RTX cycle and multiple-cycle groups (2 or more). Patients included had a follow-up of at least 3 years after their first RTX cycle, unless they needed a second infusion sooner.

131 patients were included; 44 (33.6%) received one cycle of RTX and 87 (66.4%) received two or more. The former were older at diagnosis (31.4 vs 21 years, p< 0.001) and at first RTX infusion (39.9 vs 29 years, p< 0.001). This group of patients had more organs/systems involved (p= 0.044), more leukopenia, lymphopenia and thrombocytopenia (p= 0.001, <0.0001 and 0.003 respectively) and lower C3 levels (p= 0.035). They also had fewer immunosuppressive (IS) drugs before RTX therapy compared with those who required multiple RTX cycles (p= 0.003). There was no statistical difference in the clinical and serological response after the first RTX cycle between both groups.Furthermore, patients who had received more IS treatments were more likely to require more than one cycle of RTX infusions (p= 0.007).

RTX is an effective option for SLE patients with severe flares. Patients who received more immunosuppressive drugs are more likely to receive more than one set of RTX infusions. This suggests that RTX is best used for SLE patients with no history of refractory disease.

However, few studies have examined dynamic functional connectivity (DFC). Thus, we employed resting-state fMRI with independent component analysis (ICA) and DFC analysis to investigate the alterations in FC within and between RSNs in 56 individuals with OB and 46 normal-weight (NW) controls. ICA identified six RSNs, including basal ganglia (BG), salience network (SN), right executive control network/left executive control network, and anterior default-mode network (aDMN)/posterior default-mode network. The DFC analysis identified four FC states. OB compared with NW had more occurrences and a longer mean dwell time (MDT) in state 2 (positive connectivity of BG with other RSN) and also had higher FC of BG-SN in other states. Body mass index was positively correlated with MDT and FCs of BG-aDMN (state 2) and BG-SN (state 4).
