Vascular endothelial growth factor (VEGF) family members contribute greatly to the development and angiogenesis of hypervascular hepatocellular carcinoma (HCC). We have previously shown that Dicer inhibited HCC growth. In this study, we aimed to determine the relationship between Dicer and VEGF in HCC.

Gain-of-function studies were performed to determine the effect of different treatments on the proliferation, migration, and invasion of HCC cells. Expression of VEGF-A in xenograft tumor tissues was analysed using Western blotting, and that of CD31 using immunohistochemical analysis.

We found that Dicer inhibited proliferation, migration and invasion of HCC cells by suppressing VEGF-A expression. Interestingly, VEGF-A165, which is the most prominent VEGF-A isoform, counteracted Dicer-induced inhibition of HCC cells. In addition, a monoclonal anti-VEGF antibody (bevacizumab) enhanced Dicer-induced inhibition of HCC in vitro and in vivo. Further, immunohistochemical analysis of CD31 indicated bevacizumab and Dicer synergized to reduce tumor microvessel density.

Our data demonstrated that Dicer enhanced bevacizumab-related inhibition of HCC cell via the VEGF pathway; therefore, Dicer in coordination with bevacizumab may provide another potential approach for HCC therapy.

To investigate the relationship between urinary stone type and the type of crystals in the urine.

This retrospective study involved 485 patients with urinary stones treated at King Saud University Medical City from May 2015 to June 2017. Clinical data were obtained from medical records. Different statistical analysis methods were applied, including basic contingency analysis, analysis of variance, logistic regression, discriminant analysis, partition modeling, and neural network evaluations.

Of 485 patients, 47 had crystals detected by urinalysis. The most common type of crystal was calcium oxalate (n = 31), which had the highest association with calcium oxalate stones. see more Uric acid crystals (n = 8) were associated with uric acid stones. The neural network model used for determining the sensitivity and specificity showed an R-square value of 0.88, with an area under the curve of 0.94 for calcium oxalate, 0.94 for carbonate apatite, and 1.0 for uric acid.

The predictive algorithm developed in the present study may be used with a patient's clinical parameters to predict the stone type. This approach predicts the stone types associated with certain patient characteristics with a high sensitivity and specificity, indicating that the models may be a valuable clinical tool in the diagnosis, management, and monitoring of stone diseases.

Unlike IgE mediated food allergies that may result in multi-organ system anaphylaxis, the non-IgE mediated food allergies primarily affect the gastrointestinal tract. This review outlines the clinical manifestations, epidemiology, pathophysiology, and management of non-IgE-mediated food allergies. An updated literature search of selected non-IgE-mediated food allergies was conducted for this review using PubMed database to the current year (2021). Reviewed disorders include food protein-induced enterocolitis syndrome (FPIES), food-protein enteropathy (FPE), food protein-induced allergic proctocolitis (FPIAP), and eosinophilic gastrointestinal disorders (EGIDs) such as eosinophilic esophagitis (EoE). While extensive gains have been made in understanding FPIES, FPIAP, FPE, and EoE, more information is needed on the pathophysiology of these food allergies. Similarities among them include involvement of innate immunity, T-lymphocyte processes, alteration of the intestinal lumen at the cellular level with the appearance of inflammatory cells and associated histologic changes, and specific cytokine profiles suggesting food-specific, T-cell, and immune-mediated responses. While FPIES and FPIAP typically resolve in early childhood, EGIDs typically do not. Emerging new therapies for EoE offer promise of additional treatment options. Further studies identifying the immunopathogenesis, associated biomarkers, and mechanisms of tolerance are needed to inform prevention, diagnosis and management.
Scurvy is a rarely seen in pediatric patients nowadays, seen more in those with developmental delay, autism or those who are severely malnourished. Epiphyseal separations are known to occur in scurvy, but only a few such cases have been reported in children with cerebral palsy. The diagnosis is often misleading since other morbidities as trauma, malignancies, coagulopathies, septic arthritis, osteomyelitis, or rheumatologic disorders are often considered at first. We report the case of 4-year-old female child with cerebral palsy in whom the initial concern was septic arthritis/osteomyelitis based upon clinical presentation, ultrasonic and magnetic resonance imaging, led to a surgery revealing subperiosteal hematomas.

Scurvy as a differential diagnosis for trauma, osteomyelitis, septic arthritis will always be a bane for orthopedic surgeons. A heightened awareness is needed to avoid unnecessary surgery, unnecessary tests, and procedures and to be able to start treatment for a potentially fatal but easily curable disease.
Femoral head fractures are uncommon. A femoral head fracture associated with an ipsilateral neck of femur fracture is classified as a Pipkin's Type III fracture and is exceedingly rare. It is a high-energy injury associated with an extremely poor prognosis. This has led some authors to advocate primary arthroplasty for these injuries. The aim of reporting this case is that in young patients even with severely displaced Pipkin's Type III injuries, it is reasonable to opt for internal fixation of these fractures. It is possible to obtain good function for a significant amount of time as evidenced by the 5-year follow-up in this case.

A 25-year-old male presented 4 days after a motorbike accident. He was diagnosed as a femoral head fracture with neck fracture of the left side. This fracture is classified as type III Pipkin fracture or OTA type 31C.3. This case was managed with open reduction and internal fixation utilizing the trochanteric flip osteotomy and multiple screws. It is worth to report the first case of Pipkin type three fracture having satisfactory functional outcome after 5 years of follow-up.

Opening-wedge high tibial osteotomy (OWHTO) is a widely used procedure to improve knee function, decrease knee pain and delay arthroplasty in unicompartmental knee osteoarthritis in young patients. However, faulty technique can lead to patella baja and maltracking of extensor mechanism- leading to poor outcome. Modified biplanar downsloping OWHTO - where tibial tuberosity (TT) remains attached to proximal osteotomy fragment prevents patella baja by preventing TT to slide down during opening of osteotomy. Biplanar osteotomy also prevents rotation of distal tibial fragment. Biplanar along with downsloping osteotomy both factors combined prevents patella baja and change in quadriceps vector angle and hence patellar tracking is not altered leading to a better functional result.

A 20 years male patient with right knee pain while walking for 2 years. Knee inspection shows varus deformity and varus thurst gait. No evidence of ligamentous laxity and meniscal injury. He was treated with modified biplanar downsloping OWHTO. Overall 12 patients were operated between 2018 and 2020 with same technique in a similar profile of patients.

Outcome of OWHTO can be improved and untoward complications such as patella baja which invariably occurs when TT remains attached to distal osteotomy fragment or when osteotomy passes above TT. This can be easily avoided with this downsloping OWHTO technique to give a better outcome in all 12 cases.

Segmental tibia fractures with extensive soft tissue injuries are rare and surgical intervention is challenging with no definitive treatment strategies.

A 52-year-old man presented with closed right segmental tibia and fibula fracture with extensive blistering of skin caused due to road traffic accident. Distal pulses were palpable and there were no signs of compartment syndrome and other systemic injuries. In the presence of extensive blistering, a monoplanar external fixator was applied within 24 h of injury. 3 weeks later, skin condition was conducive for internal fixation, and closed intramedullary multi-locking nailing was performed using the external fixator for reduction. Fracture healed at 15 months and patient had an excellent functional outcome with full knee range of motion at 2-years follow-up without any complications.

Fixator-assisted nailing is a simple, minimally invasive, and easily reproducible technique that is useful in reducing the fracture and preventing axial rotation of the intercalary segment minimizing the damage to the periosteal blood supply. Our case also highlights the importance of temporary external fixator in soft tissue healing and making the skin conducive for internal fixation.
