Few studies evaluate the relationships between surgical approach, histologic margin, and overall survival in gastrointestinal stromal tumor. We test the hypothesis that margin positive resection is associated with compromised overall survival.

We queried the National Cancer Data Base to identify patients undergoing resections for gastrointestinal stromal tumors ≤3cm in size between 2010 and 2015. Multivariable logistic regression was used to identify factors associated with positive microscopic margins on final pathology. Cox proportional hazard methods were used to evaluate factors associated with overall survival.

2064 patients met inclusion criteria; 135 (6.5%) had a microscopically positive surgical margin. On multivariable regression, minimally invasive approach was not associated with risk of a positive margin (OR 1.06 95% CI [0.71, 1.59]). On Cox analysis, positive margin status was not associated with OS (R1 1.03, CI [0.46-2.31], reference R0).

There are questions regarding whether grade group (GG) 4 prostate cancer (PC) is heterogeneous in terms of prognosis. We assessed prognostic differences in PC patients within GG 4 treated with radical prostatectomy (RP).

Biochemical recurrence (BCR)-free, cancer-specific, and overall survival were analyzed in 787PC patients with GG 4 based on RP pathology (Gleason score (GS) 3+5 189, GS 4+4 500, and GS 5+3 98). Logistic regression analysis was performed to assess factors predictive of high-risk surgical pathological features. Cox regression models were used to evaluate potential prognostic factors of survival.

Within a median follow-up of 86 months, 378 patients (48.0%) experienced BCR and 96 patients (12.2%) died, 42 of whom (5.3%) died of PC. GS 5+3 was significantly associated with worse BCR-free and cancer-specific survival, as well as higher positive surgical margin, lymph node metastasis, extraprostatic extension, and non-organ-confined disease rates, than GS 3+5 and higher positive surgical margin, lymph node metastasis, extraprostatic extension, and non-organ-confined disease rates than GS 4+4 (P<0.05). GS 4+4 was significantly associated with worse BCR-free survival and higher extraprostatic extension, and non-organ-confined disease rates than GS 3+5 (P<0.05). Inclusion of the different Gleason patterns improved the discrimination of a model for prediction of all survival outcomes compared to standard prognosticators.

To summarize and update our current knowledge regarding adenomyosis diagnosis, prevalence, and symptoms.

Systematic review of PubMed between January 1972 and April 2020. Search strategy included "adenomyosis [MeSH Terms] AND (endometriosis[MeSH Term OR prevalence study [MeSH Terms] OR dysmenorrhea[text word] OR prevalence[Text Word] OR young adults [Text Word] OR adolesce* [Text Word] OR symptoms[Text Word] OR imaging diagnosis [Text Word] OR pathology[Text Word].

Articles published in English that addressed adenomyosis and discussed prevalence, diagnosis, and symptoms were included. The included articles described pathology diagnosis, imaging, biopsy diagnosis, prevalence and age of onset, symptoms, and concomitant endometriosis.

Sixteen articles were included in the qualitative analysis. The studies are heterogeneous when diagnosing adenomyosis with differing criteria, protocols, and patient populations. The prevalence estimates range from 20% to 88.8% in women who are symptomatic (average 30%-35%),Adenomyosis remains a challenge to identify, assess, and research because of the lack of standardized diagnostic criteria, especially in women who wish to retain their uterus. As noninvasive diagnostics such as imaging and myometrial biopsies continue to improve, younger women with variable symptoms will likely create criteria for diagnosis with adenomyosis. The priority should be to create standardized histopathologic and imaging diagnoses to gain a deeper understanding of adenomyosis.Hydrogen evolution from biomass photoreforming has been widely recognized as a promising strategy for relieving the pressure from energy crisis and environmental pollution, as it could generate sustainable H2 and value-added bioproducts simultaneously. Combining p-type semiconductors with n-type semiconductors to form n-p heterojunction is an effective strategy to improve the photocatalytic quantum efficiency by enhancing the separation of photogenerated electrons and holes, which could greatly facilitate the realization of such biomass photorefinery concept. However, the incompact contact between the n-type and p-type semiconductors often induces the aggregation of photogenerated electrons and holes. In this work, we design and synthesize an ultrafine n-p heterojunction TiO2-NiO core-shell structure to overcome the incompact contact in the n-p interface. When the n-p heterojunction photocatalysts are evaluated for photocatalytic water splitting and biomass lignin photoreforming respectively, the as-fabricated TiO2-NiO nanocomposite with 3.25% NiO demonstrates the highest hydrogen generation of 23.5 mmol h-1 g-1 from water splitting and H2 (0.45 mmol h-1 g-1) and CH4 (0.03 mmol h-1 g-1) cogeneration with reasonable amount of fatty acids (palmitic acid and stearic acid) production from lignin photoreforming. The excellent photocatalytic activity is ascribed to the synergistic effects of high crystallinity of TiO2 ultrafine nanoparticles, core-shell structure and n-p heterojunction with NiO nanoclusters. This present work demonstrates a simple and efficient method to fabricate ultrafine n-p heterojunction core-shell structure for noble-metal free catalyst for both water splitting and biomass photoreforming.In this work, nitrogen-doped reduced graphene oxide/nickel ferrite (NRGO/NiFe2O4) hybrid nanocomposites were synthesized by a one-pot hydrothermal method. Results manifested that the as-synthesized NiFe2O4 nanoparticles displayed hexagonal morphology with a mean size of 40 nm. Moreover, it was found that the entangled structure consisting of crumpled RGO and hexagonal NiFe2O4 nanoparticles was well constructed in the as-fabricated hybrid nanocomposites. Furthermore, the additive volumes of hydrazine hydrate and filler loadings had significant influences on the electromagnetic wave (EMW) absorption properties of obtained nanocomposites. Remarkably, the NRGO/NiFe2O4 hybrid nanocomposite with 15 mL additive volume of hydrazine hydrate showed the optimal reflection loss of -54.4 dB at 9.2 GHz (X band) under a matching thickness of 2.2 mm and maximum absorption bandwidth of 4.5 GHz (13.5-18 GHz) at a very thin thickness of only 1.5 mm. The dual absorption peaks located at the low and high frequency regions could be achieved by facilely modulating the matching thicknesses. In addition, the underlying EMW absorption mechanisms were uncovered. Therefore, our results could shed light on the design and fabrication of graphene-based dielectric/magnetic hybrid composites as light-weight and high-efficiency EMW absorbers.
Malnutrition has been reported to be associated with worse clinical outcomes in various cardiovascular diseases. We aimed to investigate the clinical significance of the nutritional status in atrial fibrillation (AF) patients aged 80 years and over receiving oral anticoagulants (OACs), focusing on the difference between direct OACs (DOACs) and warfarin treatment.

This was a retrospective and observational study. We enrolled 332 consecutive AF patients aged 80 years and over who were treated with OACs DOACs (n = 256) and warfarin (n = 76). A controlling nutritional status (CONUT) score was used to evaluate the nutritional status. The enrolled patients were divided into two groups based on the CONUT score CONUT score <5 (n = 239) and CONUT score ≥5 (n = 93) groups. We investigated the relationship between the nutritional status and clinical outcomes.

The CONUT score ≥5 group had significantly higher incidence of major bleeding (MB) compared to the CONUT score <5 group (4.6/100 person-years vs. 0.7/1 aged 80 years and over receiving DOACs and those receiving warfarin. It may be favorable to use DOACs to avoid bleeding events in those with malnutrition.
