Despite many studies linking various risk factors to the association between gestational diabetes and subsequent type 2 diabetes, little is known about how food insecurity affects their association. This study is a secondary data analysis of 9,505 US women aged 20 years or older who had at least 1 live birth; we used cross-sectional data from the National Health and Nutrition Examination Survey (NHANES) from 2007 through 2018. The main outcome was a diagnosis of type 2 diabetes in the subsequent years after the first live birth. We used multivariable survey-weighted negative binomial regressions to examine whether the association between gestational diabetes and subsequent type 2 diabetes differed by food security status, with and without adjusting for health behavior factors.

Gestational diabetes was significantly associated with subsequent type 2 diabetes (incidence rate ratio [IRR], 2.57; 95% CI, 2.45-2.69). The association between gestational diabetes and subsequent type 2 diabetes was significantly different by food security status (IRR, 2.34; 95% CI, 2.23-2.45 among food-secure women; IRR, 2.99; 95% CI, 2.70-3.28 among food-insecure women).

However, investments in CH4 reduction strategies are primarily aimed at liquid dairy manure, whereas stockpiled solids remain a large source of CH4. Here, we measure the CH4 and net greenhouse gas reduction potential of dairy manure biochar-composting, a novel manure management strategy, through a composting experiment and life-cycle analysis. We found that biochar-composting reduces CH4 by 79%, compared to composting without biochar. In addition to reducing CH4 during composting, we show that the added climate benefit from biochar production and application contributes to a substantially reduced life-cycle global warming potential for biochar-composting -535 kg CO2e Mg-1 manure compared to -194 kg CO2e Mg-1 for composting and 102 kg CO2e Mg-1 for stockpiling. If biochar-composting replaces manure stockpiling and complements anaerobic digestion, California could meet SB 1383 with 132 less digesters. When scaled up globally, biochar-composting could mitigate 1.59 Tg CH4 yr-1 while doubling the climate change mitigation potential from dairy manure management.Natural killer (NK) cells are the key cells of the innate immune system that share many characteristics with T lymphocytes; their activation, however, is based on the integration of a range of activatory and inhibitory signals via receptors recognizing recurrent pathogen-associated molecular patterns. Two important populations of NK cells with differing functions are recognized CD56bright and CD56dim. NK cells have the potential to recognize and kill malignant plasma cells, which offers therapeutic opportunities. We used mass cytometry to examine the phenotype and function of NK cell subsets from patients with newly diagnosed multiple myeloma (NDMM). We show that NK cells in NDMM are shifted toward a CD56bright but dysfunctional cytotoxic phenotype, which exhibits selective loss of cytokine production. The CD56dim subset has features of exhaustion with impaired proliferation, upregulation of programmed cell death protein 1, and loss of T-cell immunoglobulin and mucin domain 3 expression. Poor expression of NK cell activation markers is seen and is associated with inferior long-term survival. These results suggest that NK cell exhaustion is already present by the time of myeloma diagnosis and likely contributes to the loss of immunologic control of malignant plasma cells. Restoring NK cell function via immune-directed therapies offers a route to restoring immunologic control in multiple myeloma.Living in a disadvantaged neighborhood is associated with poor health outcomes. Blood or Marrow Transplant (BMT) survivors remain at risk of chronic health conditions requiring anticipatory management. We hypothesized that among BMT survivors, neighborhood disadvantage was associated with poor self-reported routine healthcare utilization and health. We leveraged data from BMTSS - a retrospective cohort study examining long-term outcomes among individuals surviving ≥2y following BMT at three institutions between 1974-2014. Participants in this analysis completed the BMTSS survey (sociodemographics; chronic health conditions; time since routine check-up; self-reported health). The Area Deprivation Index (ADI) represented neighborhood disadvantage; this composite indicator of 17 census measures is a percentile rank (0=least deprived to 100=most deprived). Multivariable ordered logit regression adjusted for clinical factors and individual-level sociodemographics, modeling associations between ADI, time since routine check-up, and self-reported health. Among 2,857 survivors, median ADI was 24 (interquartile range 10-46). Adjusting for self-reported individual-level socioeconomic indicators and chronic health conditions, patients in more disadvantaged neighborhoods had higher odds of reporting longer intervals since routine check-up (ORADI_continuous=1.007, p less then 0.001) and poorer health status (controlling for time since check-up; ORADI_continuous=1.005, p=0.003). Compared to patients living in the least disadvantaged neighborhood (ADI=1), patients in the most disadvantaged neighborhood (ADI=100), had twice the odds (ORADI=1.007^99=2.06) of reporting no routine visits and 1.65-times the odds of reporting poor health (ORADI=1.005^99=1.65). In BMT survivors, access to healthcare and health status are associated with area disadvantage. Data of 100 ITP patients were analysed. Complete blood count findings of the patients at certain time points were evaluated to classify the disease as acute, persistent and chronic. An effect of sex on chronicity was not observed ( P  = 0.166). Of the patients enrolled in the study, 38% ( n  = 38) had chronic course. The mean age of patients with the chronic course was 7 ± 4.1 years, which was significantly higher than the other groups ( P  = 0.007). Sixty-five percent ( n  = 13) of the patients presenting with mucosal bleeding and 27.4% ( n  = 20) of the patients presenting with skin bleeding became chronic ( P  = 0.008). MPV was found to be significantly high in chronic ITP patients ( P  = 0.049). Monocytosis was noted in 80% of the patients at diagnosis. Intravenous immunoglobulin was used in 84% of the patients with acute ITP; 33% of them developed chronic ITP. The age at diagnosis, presence of mucosal bleeding and increased MPV on admission were high-risk factors for the development of the chronic course. Monocytosis was detected in 80% of the patients on admission, and it may play a role in the pathogenesis of ITP.
