The purpose of this study was to investigate whether neurosurgical intervention for traumatic brain injury (TBI) is associated with reduced risks of death and clinical deterioration in a low-income country with a relatively high neurosurgical capacity. The authors further aimed to assess whether the association between surgical intervention and acute poor outcomes differs according to TBI severity and various patient factors.
Using TBI registry data collected from a national referral hospital in Uganda between July 2016 and April 2020, the authors performed Cox regression analyses of poor outcomes in admitted patients who did and did not undergo surgery for TBI, with surgery as a time-varying treatment variable. Patients were further stratified by TBI severity using the admission Glasgow Coma Scale (GCS) score mild TBI (mTBI; GCS scores 13-15), moderate TBI (moTBI; GCS scores 9-12), and severe TBI (sTBI; GCS scores 3-8). Poor outcomes constituted Glasgow Outcome Scale scores 2-3, deterioration in TBI seve the need for closer monitoring of this population. These results highlight the importance of accurate, timely clinical evaluation throughout a patient's admission and can inform decisions about whether and when to perform surgery for TBI when resources are limited.
Gauging fitness remains a challenge among older adults with hematologic malignancies, and interventions to restore function are lacking. We pilot a structured exercise intervention and novel biologic correlates of aging using epigenetic clocks and markers of immunosenescence to evaluate changes in function and clinical outcomes.
Older adults (n=30) with hematologic malignancy actively receiving treatment were screened and enrolled in a 6-month exercise intervention, the Otago Exercise Programme (OEP). The impact of the OEP on geriatric assessment metrics and health-related quality of life were captured. Clinical outcomes of overall survival and hospital utilization (inpatient length of stay and emergency department use) in relationship to geriatric deficits were analyzed.
Older adults (median age, 75.5 years [range, 62-83 years]) actively receiving treatment were enrolled in the OEP. Instrumental activities of daily living and physical health scores (PHS) increased significantly with the OEP interventio utilization.
The OEP was effective in improving quality of life, and geriatric tools predicted survival and hospital utilization among older adults with hematologic malignancies.

The etiology of gambling disorder is complex and multifaceted as it is governed by multiple and interrelated factors. In this context and in light of the pervasive financial crisis in Greece, we conducted a study in order to explore adolescents' gambling involvement in Athens region and also to identify the socio-economic characteristics of adolescents who have engaged into gambling activities. Students were recruited from a sample of schools in Athens area. For the assessment of gambling involvement in adolescents, the Diagnostic and Statistical Manual of Mental Disorders-IV Multiple Response Adapted for Juveniles Questionnaire (DSM-IV-MR-J) was administered. Additional self-constructed questions enquired about students' socio-demographic and economic characteristics. Our results indicate that adolescents that had problem with gambling or had at least one pathological item on DSM were moociety during this rough time period. Interventions should prioritize endowing adolescents with the necessary coping skills for dealing with daily obstacles or life adversities productively and without losing their self-control.Atrial fibrillation (AF) is a common sustained cardiac arrhythmia, which is expected to expand enormously due to population ageing. The aim of the present study was to explore the significance of AF patients' needs and the associated factors as well as the impact of anxiety and depression on these needs. The sample of the study included 215 AF hospitalized patients. Data were collected by the completion of a questionnaire which, in addition to patients' characteristics, it also included the Hospital Anxiety and Depression Scale (HADS) and the questionnaire "Needs of hospitalized patients with coronary artery disease", that is evaluating the significance of the following 6 categories of needs a) for support and guidance, b) for information from medical and nursing staff, c) for communication with other patients and family, d) for individualization of care and participation, e) to trust the medical and nursing staff and f) to meet emotional and physical needs. The statistical significance level was p70 years and those having no children (p=0.039 and p=0.029, respectively) while the need for communication with other patients and family was considered more significant by women (p=0.028). The present findings emphasize the importance of providing care tailored to meet the needs of AF patients who experience anxiety and depression.Repetitive Transcranial Magnetic Stimulation (rTMS) is an innovative, non-invasive and well tolerated method that could be used as a treatment option for a variety of neuropsychiatric disorders. A large number of studies, for more than 30 years, have demonstrated that is a powerful neuroscience tool for diagnostic and therapeutic purposes. rTMS is based on the phenomenon of electromagnetic mutual induction, that firstly reported by Michael Faraday in 1831. Later, in 1985, Anthony Barker and his colleagues developed the first modern transcranial magnetic stimulation (TMS) device. rTMS uses brief electromagnetic pulses generated by an insulated coil, placed over the scalp. This technique has the ability to modulate the cortical activity of the brain. Daily rTMS stimulation for several weeks has been shown to be effective in reducing symptoms of many neuropsychiatric disorders. Moreover, studies have shown that dorsolateral prefrontal cortex (DLPFC) has a crucial role in improving cognitive performance and, as a result, is a commonly used target area for the treatment of many neuropsychiatric disorders such as depression. 2008 was a significant year for TMS history, as FDA approved for first time a TMS therapy device for the clinical treatment of depression. This approval as well as the National Institute for Health and Care Excellence (NICE) recommendation on TMS for the treatment of depression in 2015 lead to the establishment of rTMS as a first-line treatment for patients that failed in at least one prior antidepressant medication, followed by FDA approvals for the treatment of migraines and OCD. This paper aims to enhance further some of the clinical usefulness of rTMS.Infliximab (IFX) is a chimeric monoclonal antibody biologic drug that works against tumor necrosis factor alpha (TNF-α) and is used to treat autoimmune diseases. This is case of a 45year old female patient who had suicidal thoughts after receiving infusions with IFX. She did not report any family psychiatric history. She was diagnosed with ulcerative colitis. She had many relapses and she was treated with azathioprine and prednisolone. After many incidents of diarrhea, she started therapy with infliximab infusions. She had totally 13 infusions during a period of 13 months. The last year and in particular during the time of Infliximab intake, she reported suicidal ideation. Due to lack of improvement in her physical symptoms, she voluntarily discontinued medication and resorted to a nutritionist and a mental health counselor, where she followed cognitive and behavioral interventions. Treatment of autoimmune disorders with infliximab raise an awareness among medical and paramedical staff involved in the care of these patients about the psychiatric side effects of the drug.
To automate diagnostic chest radiograph imaging quality control (lung inclusion at all four edges, patient rotation, and correct inspiration) using convolutional neural network models.
The data comprised of 2589 postero-anterior chest radiographs imaged in a standing position, which were divided into train, validation, and test sets. We increased the number of images for the inclusion by cropping appropriate images, and for the inclusion and the rotation by flipping the images horizontally. The image histograms were equalized, and the images were resized to a 512×512 resolution. We trained six convolutional neural networks models to detect the image quality features using manual image annotations as training targets. Additionally, we studied the inter-observer variability of the image annotation.
The convolutional neural networks' areas under the receiver operating characteristic curve were >0.88 for the inclusions, and >0.70 and >0.79 for the rotation and the inspiration, respectively. The inter-observer agreement between two human annotators for the assessed image-quality features were 92%, 90%, 82%, and 88% for the inclusion at patient's left, patient's right, cranial, and caudal edges, and 78% and 89% for the rotation and inspiration, respectively. Higher inter-observer agreement was related to a smaller variance in the network confidence.

Previously, we constructed a high-throughput sonoreactor to investigate features of the amyloid-fibril nucleation. Although the instrument substantiated the ultrasonication efficacy, several challenges remain; the key is the precise control of the acoustic field in the reactor, which directly affects the fibril-formation reaction. In the present study, we develop the optimized sonoreactor for the amyloid-fibril assay, which improves the reproducibility and controllability of the fibril formation. Using β2-microglobulin, we experimentally demonstrate that achieving identical acoustic conditions by controlling oscillation amplitude and frequency of each transducer results in identical fibril-formation behavior across 36 solutions. Moreover, we succeed in detecting the 100-fM seeds using the developed sonoreactor at an accelerated rate. Finally, we reveal that the acceleration of the fibril-formation reaction with the seeds is achieved by enhancing the primary nucleation and the fibril fragmentation through the analysis of the fibril-formation kinetics. These results demonstrate the efficacy of the developed sonoreactor for the diagnosis of amyloidoses owing to the accelerative seed detection and the possibility for further early-stage diagnosis even without seeds through the accelerated primary nucleation.
