Light is one of the main factors modulating the biosynthesis of specialized metabolites, determining the cascade response activated by photoreceptors and the consequent modulation of expressed genes and biosynthetic pathways. Recent developments in light emitting diode (LED) technology have enabled improvements in artificial light applications for horticulture. In particular, the possibility to select specific spectral light compositions, intensities and photoperiods has been associated with altered metabolite content in a variety of crops. This review aims to analyze the effects of indoor LED lighting recipes and management on the specialized metabolite content in different groups of crop plants (namely medicinal and aromatic plants, microgreens and edible flowers), focusing on the literature from the last 5 years. The literature collection produced a total of 40 papers, which were analyzed according to the effects of artificial LED lighting on the content of anthocyanins, carotenoids, phenols, tocopherols, glycosides, and terpenes, and ranked on a scale of 1 to 3. Most studies applied a combination of red and blue light (22%) or monochromatic blue (23%), with a 16 h day-1 photoperiod (78%) and an intensity greater than 200 μmol m-2 s-1 (77%). These treatment features were often the most efficient in enhancing specialized metabolite content, although large variations in performance were observed, according to the species considered and the compound analyzed. The review aims to provide valuable indications for the definition of the most promising spectral components toward the achievement of nutrient-rich indoor-grown products. © 2021 The Authors. Health systems make a sizeable contribution to national emissions of greenhouse gases that contribute to global climate change. The UK National Health Service is committed to being a net zero emitter by 2040, and a potential contribution to this target could come from reductions in patient travel. Achieving this will require actions at many levels. We sought to determine potential savings and risks over the short term from telemedicine through virtual clinics.

During the severe acute respiratory syndrome coronavirus 2 (SARS-2-CoV) pandemic, scheduled face-to-face epilepsy clinics at a specialist site were replaced by remote teleclinics. We used a standard methodology applying conversion factors to calculate emissions based on the total saved travel distance. A further conversion factor was used to derive emissions associated with electricity consumption to deliver remote clinics from which net savings could be calculated. Patients' records and clinicians were interrogated to identify any adverse clinical adverse clinical outcomes over the short term. However, these outcomes will likely vary with clinic locations, medical specialties and conditions.The outcome of patients with Hodgkin lymphoma (HL) has improved significantly in recent years, and now attention is increasingly being focused on the well-being of these young patients. This study aimed to analyse the influence of HL and its treatment on the spermatogenic status of 46 male HL patients with available spermiograms, treated between 2008 and 2016. Analysing prognostic factors at diagnosis, we found that the number of spermatozoa was reduced in stage III-IV; motility and vitality were reduced in stage III-IV and in the presence of B symptoms; and abnormal forms were increased in patients with elevated erythrocyte sedimentation rate (ESR) and low albumin. Furthermore, we found that haematopoietic stem cell transplantation (HSCT) was associated with a severe impairment of fertility in terms of sperm motility. In HL-treated patients who did not undergo HSCT we found a statistically significantly improved fertility in terms of motility. In this study, we found that HSCT induced infertility in the majority of male patients with HL, but that first-line treatment could improve the impaired fertility status caused by disease. Further studies are needed in larger case series to investigate risk factors for impaired fertility at HL diagnosis and after treatment.Increasing use of innovative items in operational assessments has shedded new light on the polytomous testlet models. In this study, we examine performance of several scoring models when polytomous items exhibit random testlet effects. Four models are considered for investigation the partial credit model (PCM), testlet-as-a-polytomous-item model (TPIM), random-effect testlet model (RTM), and fixed-effect testlet model (FTM). The performance of the models was evaluated in two adaptive testings where testlets have nonzero random effects. The outcomes of the study suggest that, despite the manifest random testlet effects, PCM, FTM, and RTM perform comparably in trait recovery and examinee classification. The overall accuracy of PCM and FTM in trait inference was comparable to that of RTM. TPIM consistently underestimated population variance and led to significant overestimation of measurement precision, showing limited utility for operational use. The results of the study provide practical implications for using the polytomous testlet scoring models.
This study was undertaken to characterize trajectories of antiseizure medication (ASM) adherence in adults with newly treated epilepsy and to determine predictors of trajectories.

This was a retrospective cohort study using Medicare. We included beneficiaries with newly treated epilepsy (one or more ASM and none in the preceding 2years, plus International Classification of Diseases codes) in 2010-2013. We calculated the proportion of days covered (proportion of total days with any ASM pill supply) for 8 quarters or until death. Group-based trajectory models characterized and determined predictors of trajectories.

We included 24923 beneficiaries. Models identified fourgroups early adherent (60%), early nonadherent (18%), late adherent (11%), and late nonadherent (11%). Numerous predictors were associated with being in the early nonadherent versus early adherent group non-White race (e.g., Black, odds ratio [OR] = 1.7, 95% confidence interval [CI] = 1.5-1.8), region (e.g., South vs. Northeast OR = 1.2, 95ghted a modifiable risk factor for early nonadherence lacking a neurologist. These data may guide future interventions aimed at improving ASM adherence, in terms of both timing and target populations.We present a generic workflow combining physiology-based computational modeling and in vitro data to assess the clinical cholestatic risk of different drugs systematically. Changes in expression levels of genes involved in the enterohepatic circulation of bile acids were obtained from an in vitro assay mimicking 14 days of repeated drug administration for ten marketed drugs. These changes in gene expression over time were contextualized in a physiology-based bile acid model of glycochenodeoxycholic acid. The simulated drug-induced response in bile acid concentrations was then scaled with the applied drug doses to calculate the cholestatic potential for each compound. A ranking of the cholestatic potential correlated very well with the clinical cholestasis risk obtained from medical literature. The proposed workflow allows benchmarking the cholestatic risk of novel drug candidates. We expect the application of our workflow to significantly contribute to the stratification of the cholestatic potential of new drugs and to support animal-free testing in future drug development.
Multiparous labor inductions are typically successful, and the process can be rapid, starting from a ripened cervix with a predictable response to amniotomy and oxytocin infusion. Outpatient Foley catheter labor induction in multiparas with unripe cervixes is a feasible option as the mechanical process of ripening is usually without significant uterine contractions and well tolerated. Labor contractions can be initiated by amniotomy and titrated oxytocin infusion in the hospital for well-timed births during working hours as night birth are associated with adverse events. ML355 We sought to evaluate outpatient compared with inpatient Foley catheter induction of labor in multiparas for births during working hours and maternal satisfaction.

A randomized trial was conducted in the University of Malaya Medical Center. A total of 163 term multiparas (no dropouts) with unripe cervixes (Bishop score ≤5) scheduled for labor induction were randomized to outpatient or inpatient Foley catheter. Primary outcomes were delivee rupture to delivery interval were significantly shortened in the outpatient group. The rate of maternal satisfaction was high in both groups and no significant differences were found.
The trial failed to demonstrate the anticipated increase in births during working hours with outpatient compared with inpatient induction of labor with Foley catheter in parous women with an unripe cervix. Hospital stay and membrane rupture to delivery interval were significantly shortened in the outpatient group. The rate of maternal satisfaction was high in both groups and no significant differences were found.
Chronic pain is one of the most common health problems for older adults worldwide and is likely to result in lower quality of life. Living in a different culture may also influence chronic pain and quality of life in older adults. The purpose of this study was to explore how multifaceted elements affect chronic pain and quality of life in older Koreans living in Korea and in older Korean-Americans (KAs) living in the USA.

We conducted a secondary data analysis of data from 270 adults aged 65years or over (138 Koreans and 132 KAs). We compared the effects of multifaceted elements on pain and quality of life by testing structural equation models (SEMs) for each group, using a maximum likelihood estimation and bootstrapping.

SEMs for both Korean and KAs showed that age and depressive symptoms directly affected quality of life. The number of comorbidities and depressive symptoms had mediating effects on quality of life through chronic pain in both groups. In older Koreans only, perceived financial status directly affected quality of life. In older KAs only, sleep quality indirectly affected quality of life through chronic pain.

The data showed that multimorbidity and depressive symptoms play critical roles for explaining chronic pain in older Koreans and KAs and ultimately negatively influence quality of life. Future intervention program to improve quality of life in older adults with chronic pain should consider the different cultural aspects affecting quality of life for Koreans and KAs.
