Probiotics are a big food and pharmaceutical industry today. The most widely used probiotics are Lactobacillus spp, Bifidobacterium spp, strains of Enterococcus spp, Streptococcus spp and yeasts such as Saccharomyces pp. Among the best-known indications are Clostridioides difficile colitis and diarrhea due to antibiotics, but probiotics are also proposed as adjuvants for atopic syndromes, autoimmune diseases, prevention and treatment of obesity, diabetes and cystic fibrosis, where dysbiosis play an important role. However, their use remains debated due to a lack of powerful data proving their effectiveness and their often-overlooked side effects. The extraordinary development of medicine with the advent of solid organ and bone marrow transplants, chemotherapy and immunotherapy, as well as the explosion of invasive procedures (« foreign material ») has made our medicine dependant to the use of antibiotics. The overuse of « empirical » antibiotics in breeding and medicine has favored the emergence and rapid dissemination of multidrug resistant pathogens (MDRs). This leaded clinicians today in a difficult situation. They should limit their empirical use of « broad-spectrum antibiotics », although they face a higher risk of MDRs. To help them in this task, anti-microbial stewardship programs have been put in place, emphasizing the use of « hospital antibiograms » and rapid and reliable microbiological diagnosis.Cephalosporins belong to the betalactam group and are frequently prescribed in both out and inpatient settings. Their broad spectrum of activity allows a varied use in most medical specialties, ranging from preoperative prophylaxis to treatment of febrile agranulocytosis. There are currently five generations of cephalosporins, mainly differentiated according to their structure, spectrum of activity and side-effect profile. So-called siderophore cephalosporins are active against many multiresistant bacteria, especially in cases of complicated urinary tract infections or ventilator-associated pneumonia. This article intends to review some general clinical principles in prescription and monitoring of patients treated with cephalosporins. The restocking of the endangered Kanglang white minnow ( Anabarilius grahami) in Lake Fuxian, China, has been conducted for 13 years. However, few studies have reported on the effectiveness of the captive breeding and release of this species. Here, we investigated variations in morphology, including body shape and skeletal deformities, and genetic features among hatchery-born and recaptured A. grahami from Lake Fuxian. Results showed that current hatchery-reared fish displayed a stubbier body shape than their wild conspecifics from the 1980s. Furthermore, high skeletal deformity ratios were found in two aquafarms (Luchong, 50%; Haikou, 45.2%), and the release of malformed fish elevated the skeletal deformity rate of wild stocks found near the Lake Fuxian release sites (west coast, 19.0%; east coast, 12.5%). Based on variations in the cytochrome b (cyt b) gene, existing A. grahami populations showed relatively high haplotype diversity and low nucleotide diversity. Hatchery populations exhibited reduced genetic variations based on microsatellite markers and reintroduction led to markedly lower genetic diversity around the west coast release sites of Lake Fuxian. Analysis of molecular variance (AMOVA) of cyt b and microsatellite analysis showed that the greatest genetic variations were found within populations, and genetic distance and Bayesian clustering analysis showed that the 14 populations clustered into one group. Based on morphological and genetic tests, we discuss corresponding recommendations, including release size, feed formulations, breeding strategies, and release tags, to minimize potential risks and improve hatchery practices for better restocking of this species.Species of the spider family Leptonetidae Simon, 1890 from China are revised based on molecular and morphological data analyses. A new genus, Jingneta Wang & Li gen. nov., is erected, with Leptoneta cornea Tong & Li, 2008 as the type species. Twenty-two Chinese species previously assigned to the genus Leptoneta Simon, 1872 are revised, with eight transferred to Falcileptoneta Komatsu, 1970, seven transferred to Jingnetagen. nov., five transferred to Leptonetela Kratochvíl, 1978, and one species each transferred to Longileptoneta Seo, 2015 and Masirana Kishida, 1942. Eight new species are described i.e., Falcileptoneta shuanglong Wang & Li sp. nov. (♂), Jingneta caoxian Wang & Li sp. nov. (♂♀), J. jingdong Wang & Li sp. nov. (♂♀), Longileptoneta gutan Wang & Li sp. nov. (♂♀), L. huangshan Wang & Li sp. nov. (♂♀), L. shenxian Wang & Li sp. nov. (♂♀), L. yeren Wang & Li sp. nov. (♂), and L. zhuxian Wang & Li sp. nov. (♂♀). In total, 127 leptonetid species from six genera are documented from China nine species of Falcileptoneta, nine species of Jingnetagen. nov., 101 species of Leptonetela, six species of Longileptoneta, one species of Masirana, and one species of Rhyssoleptoneta Tong & Li, 2007.A new species of the genus Amolops, Amolopsputaoensissp. nov., is described from northern Myanmar. The new species can be distinguished from its congeners by the following characters (1) dorsolateral fold distinct; (2) upper-lip stripe white; (3) male body size 37.6-40.2 mm; (4) ground color of dorsal surface brown, flank green, small warts on dorsum; (5) two internal subgular vocal sacs present; (6) HL slightly shorter than HW; (7) two palmar tubercles present, supernumerary tubercles and outer metatarsal tubercle absent; (8) tympanum smaller than half of eye diameter; (9) vomerine teeth present; (10) tibiotarsal articulation reaching beyond snout tip; (11) supratympanic fold indistinct; (12) pineal body present; (13) finger webbing absent, presence of circummarginal groove on tip of first finger; (14) nuptial pads present. The population from Myanmar represented a distinct maternal lineage within the Amolops monticola group and was recovered as a sister taxon to Amolops aniqiaoensis with strong support (100) based on concatenated data. Average uncorrected pairwise distances ( P-distances) between the specimens from Myanmar and other species in the genus ranged from 2.69% (vs. A. aniqiaoensis) to 12.24% (vs. A. indoburmanensis) for 16S rRNA, 6.14% (vs. A. aniqiaoensis) to 15.79% (vs. A. panhai) for COI, and 9.66% (vs. A. aniqiaoensis) to 19.52% (vs. A. afghanus) for ND2.
Multiple breath washout (MBW) is increasingly used in the clinical assessment of patients with cystic fibrosis (CF). Guidelines for MBW quality control (QC) were developed primarily for retrospective assessment and central overreading. We assessed whether real-time QC of MBW data during the measurement improves test acceptability in the clinical setting.
We implemented standardized real-time QC and reporting of MBW data at the time of the measurement in the clinical pediatric lung function laboratory in Bern, Switzerland, in children with CF aged 4-18 years. We assessed MBW test acceptability before (31 tests; 89 trials) and after (32 tests; 96 trials) implementation of real-time QC and compared agreement between reviewers. Further, we assessed the implementation of real-time QC at a secondary center in Zurich, Switzerland.
Before the implementation of real-time QC in Bern, only 58% of clinical MBW tests were deemed acceptable following retrospective QC by an experienced reviewer. After the implementation of real-time QC, MBW test acceptability improved to 75% in Bern. In Zurich, after the implementation of real-time QC, test acceptability improved from 38% to 70%. Further, the agreement between MBW operators and an experienced reviewer for test acceptability was 84% in Bern and 93% in Zurich.
Real-time QC of MBW data at the time of measurement is feasible in the clinical setting and results in improved test acceptability.
Real-time QC of MBW data at the time of measurement is feasible in the clinical setting and results in improved test acceptability. The high incidence of disease in captive pangolins is a major obstacle in pangolin-conservation breeding programs. Therefore, elucidating pangolins' susceptibility to disease is the key to conservation progress. At the Pangolin Research Base for Artificial Rescue and Conservation Breeding of South China Normal University (PRB-SCNU), vitamin A deficiency was diagnosed in 14 captive Sunda pangolins. Typical eye signs included lacrimal eyes, keratopathy and a blank, milky orb. The afflicted pangolins were treated with vitamins A and D for 15-30 days; all individuals recovered. We report the detection and treatment of vitamin A deficiency in captive Sunda pangolins at the PRB-SCNU. Our results could provide guidance for the future prevention and treatment of vitamin A deficiency and associated diseases in pangolin species, both to reduce the incidence of these diseases in captive pangolins and to aid conservation efforts. In the present study, the effect of nerve stimulation on the secretory activity of the ovary of adult females was analyzed for the first time in amphibians. Results revealed that in Rhinella arenarum the stimulation of nerves that supply the gonad induced an increase in estradiol and progesterone secretion, this response showing differences during the reproductive cycle of the species. During the postreproductive period, an increase in estradiol secretion was observed while, in the reproductive period, progesterone secretion increased. Our results suggest that the sympathetic division of the autonomic nervous system would be responsible for this increase, taking into account that, under our experimental conditions, acetylcholine did not affect the endocrine activity of the gonad, while adrenaline (epinephrine) was effective in inducing steroid secretion an effect that could be due to interaction with β receptors. selleck chemical On the other hand, our data show that the association of adrenaline with follicle-stimulating hormone increased estradiol secretion during the postreproductive period, while the association of catecholamine with LH or hCG increased progesterone secretion during the reproductive period. Our results would suggest that nerve stimulation, mediated by the release of adrenaline, would act synergistically with gonadotrophins to stimulate steroid secretion.
Antineutrophil cytoplasmic antibody-associated vasculitis (AAV) is considered a chronic, relapsing condition. To date, no studies have investigated multimorbidity in AAV nationally. This study was undertaken to characterize temporal trends in multimorbidity and report excess health care expenditures associated with multimorbidities in a national AAV cohort from Scotland.
Eligible patients with AAV were diagnosed between 1997 and 2017. Each patient was matched with up to 5 general population controls. Linked morbidity and health care expenditure data were retrieved from a Scottish national hospitalization repository and from published national cost data. Multimorbidity was defined as the development of ≥2 disorders. Prespecified morbidities, individually and together, were analyzed for risks and associations over time using modified Poisson regression, discrete interval analysis, and chi-square test for trend. The relationship between multimorbidities and health care expenditure was investigated using multivariate linear regression.
