We investigated the possible significance of rare genetic variants to response to valproic acid (VPA) and ethosuximide (ETX) in patients with absence epilepsy. Our primary hypothesis was that rare CACNA1H variants are more frequent in ETX-non-responsive patients compared to ETX-responsive. Our secondary hypothesis was that rare variants in GABA-receptor genes are more frequent in VPA-non-responsive patients compared to VPA-responsive.

We recruited patients with absence epilepsy treated with both VPA and ETX, and performed whole exome sequencing in order to investigate the potential role of rare variants in CACNA1H, other voltage-gated calcium channel (VGCC) genes, or GABA-receptor genes in predicting response to ETX or VPA.

Sixty-two patients were included; 12 were ETX-responsive, 14 VPA-responsive, and 36 did not have a clear positive response to either medication. At treatment start, the estimated risk of a paradoxical reaction to lamotrigine was 7.9 % in JME patients (n = 190). For all GGE patients (incl. JME), the estimated risk of clinically relevant seizure aggravation under treatment with lamotrigine was 3.7 % (1.8 % for zonisamide and 0.8 % for levetiracetam). In conclusion, clinical significant aggravation of seizure frequency is common in lamotrigine-treated JME patients but rare in patients with other GGE subsyndromes or under treatment with other recommended anti-seizure medication.Polyaromatic hydrocarbons (PAH) are persistent pollutants of great concern due to their potential toxicity, mutagenicity and carcinogenicity. A biotechnological approach to remove PAH from soil was evaluated in this work using a laccase mediator system. Initially, laccase was produced by fungal co-cultivation, using kiwi peels as substrate. The produced laccase was applied to PAH contaminated soil to evaluate its efficiency on enzymatic bioremediation. Results showed that laccase mediator system was effective in the degradation of pyrene, fluorene, chrysene and a lower extension anthracene. Mediators improved the PAH degradation and natural mediators (ferulic acid and p-coumaric acid) were as effective as the synthetic mediator ABTS. However, the process was not effective in the benzo[a]pyrene degradation, one of the most recalcitrant and toxic PAH. This low degradation rate could be related to the low activity of the laccase mediator system in an environment lacking water. To overcome this issue, a PAH contaminated soil degradation system was developed in packed bed reactor (PBR) fed with laccase/mediator. Continuous flow of laccase/mediator improved the PAH degradation, achieving 74.8 %, 71.9 %, 72.2 %, 81.8 % and 100 % degradation for fluorene, anthracene, phenanthrene, chrysene and pyrene, respectively. This system was able to degrade 96 % benzo[a]pyrene, which was 90 % higher than the degradation in batch system. Results indicated that the produced laccase as well as the fed-batch degradation system developed in PBR could be successfully applied in the degradation of soil PAH pollutants, with the advantage of achieving higher degradation rates than in simple batch, as well as being a faster and simpler process than microorganism bioremediation.

For the spouses, the level of education (p<0.001), the previous childbirth experience (p<0.001), and feelings about the forthcoming childbirth (p<0.001), were significantly associated with prenatal fear of childbirth. Spearman's correlation test results indicated a significant positive correlation between prenatal fear of childbirth among pregnant women and their spouses (r=0.182, p<0.001).

The majority of the patients with pulmonary embolism (PE) are those with normotensive PE. Right ventricular dysfunction (RVD) and myocardial injury markers are associated with mortality although they have a low predictive impact. Here, we aim to study the performance characteristics of jugular vein diameter to predict 30-day mortality.

In this prospective, observational cohort study, we included normotensive patients who were diagnosed with PE using computed tomography angiography or scintigraphy in the emergency service. The demographic characteristics, blood pressures, pulses, shock indexes, troponin and lactate levels, echocardiography findings, and internal jugular vein diameters (IJV) of the patients were recorded. Testing characteristics of IJV in predicting 30-day mortality were studied.

The mean age of the 81 patients was 66.8±16.9years and 37% of them were male. Age, shock index, lactate, RVD, PESI, and IJV diameters during inspiration and expiration were indicators for 30-day mortality. The cut-off value obtained using the ROC curve for mortality was an IJV-exp-AP of ≤8.9mm (sensitivity,73.3%; specificity,92.4%; +LR,9.68; -LR,0.29; NPD,93.8%; PPD,68.7%; area under the curve, 0.76; 95% confidence interval, 0.65-0.84; p=0.004).

To compare the image quality of subtracted and nonsubtracted images obtained using volumetric interpolated breath-hold exam (VIBE) and free breathing T1 weighted Golden-angle Radial Sparse Parallel (GRASP).

We retrospectively evaluated 27 consecutive patients who underwent MRI for the evaluation of renal masses. Contrast enhanced VIBE and free breathing GRASP imaging were performed, and subtraction images generated. Two radiologists performed quantitative and qualitative evaluations of image quality of nonsubtracted and subtracted data sets. Statistical analysis was performed using the Wilcoxon signed-rank test, paired t-test and kappa statistics.

VIBE images scored statistically higher for the following parameters in the coronal and axial plane sharpness, streak artifact, image noise, and overall image quality for standard and subtracted images (all P values P<0.001). GRASP images had significantly less subtraction artifact in the coronal (P=0.042) plane with a similar trend in the axial plane (P=0.079). Interreader Kappa values for qualitative images scores were fair to good (0.23-0.71). Quantitative subtracted GRASP images had significant less subtraction artifact compared to VIBE in the anterior-posterior (3.9mm SD 2.6mm versus 5.8mm SD 3.6mm, P=0.010), and craniocaudal direction (4.4mm SD 2.9mm versus 7.0mm SD 5.3mm, P=0.010); a trend was seen in the left-right direction (2.6mm SD 1.4mm versus 4.0mm SD 3.9mm, P=0.084).
