Photodynamic therapy (PDT) is a useful treatment modality for premalignant skin lesions. We sought to describe PDT utilization on a national level in the Medicare population post-CPT code revision in 2018 to better understand trends in volume and distribution as well as what factors may influence service performance.

We used the 2018-2019 Medicare Physician and Other Supplier Public Use File to assess PDT services performed during this period.

We found that there was an increasing trend of PDT utilization in the Medicare population. While there was less PDT performed without direct involvement of a healthcare professional, an increasing number of PDT services requiring debridement were performed for severe lesions. Although the majority of PDT volume was attributable to dermatologists, non-physician clinicians assumed greater involvement in PDT services. Dermatologists practicing in academic and non-metropolitan settings were less likely to be directly involved with PDT delivery.

There is a considerable volume of PDT performed among Medicare beneficiaries, with several utilization trends that may be explained by geographic location and practice setting, among other factors. selleck chemical These findings provide insight to PDT service distribution on a national level and highlight practice patterns that may influence PDT delivery.

Skin conditions manifest differently on darker skin tones, yet lighter skin tones are usually overrepresented in dermatology educational resources. Our aim was to explore the representation of skin tones within dermatology social media posts in 2019.

This is a retrospective review of posts made on Instagram, YouTube, and Twitter by the top 2019 dermatology social media influencers. Twenty-one accounts were included. Eight posts were randomly chosen per account and graded by the Fitzpatrick scale (I-VI).

Overall, Fitzpatrick Type II was the most represented skin type (53%), while Fitzpatrick Type VI was the least represented (0%). On Instagram, 79.7 percent of posts represented light skin tones (Fitzpatrick Types I-III) and 20.3 percent represented dark skin tones (Fitzpatrick Types IV-VI). On YouTube, 93.75 percent of posts represented light skin tones and 6.25 percent represented dark skin tones. On Twitter, 73.2 percent of posts represented light skin tones and 26.8 percent represented dark skin tones.

This study is limited by the subjective nature of assigning Fitzpatrick skin types and by only analyzing posts from dermatology social media influencers in 2019.

Within the analyzed posts, darker skin tones were represented less frequently than lighter skin tones; we encourage a wider range of skin tone representation within educational resources.

Clinical studies on acne vulgaris in skin of color are limited; thus, it is likely that treatment recommendations to patients with darker skin types are drawn from trial data based on Caucasian skin. The aim of this study was to systematically review the effectiveness and tolerability of treatments used to treat acne vulgaris in patients with skin of color. A literature search was performed in the PubMed, Embase, and Scopus bibliographic databases, with a total of 1,477 retrieved articles, of which 1,316 were excluded after initial screening. Of the 93 studies assessed, 55 studies met our inclusion criteria (28 randomized controlled trials, 4 cohort studies, 6 post-hoc analyses, and 12 other interventional trials). The studies reported a total of 21,202 patients. Most studies explored topical therapies (23 studies) and photodynamic therapy (13 studies). Other treatments included laser/light therapy, systemic therapy, chemical peels, and radiofrequency and microneedling. In general, the different treatment modalities offered an improvement in lesion count and were well tolerated, with no report of major adverse events. However, due to limited evidence, we were unable to draw firm conclusions from the results of this review to guide decisions in practice, particularly with respect to long-term outcomes, in patients with skin of color and acne vulgaris.
Subantibiotic dose doxycycline (SDD40), formulated as a modified-release 40mg capsule administered once daily, is used to treat inflammatory lesions of rosacea. In order to investigate whether the patient's weight or lesion severity impacts clinical outcomes with using SDD40, the efficacy and safety of SDD40 in treating rosacea were evaluated in randomized controlled studies (RCTs).

Phase II, III, and IV RCTs, and a subsequent meta-analysis were described. For all studies, the primary efficacy endpoint was the change in total inflammatory lesion count (papules, pustules, and nodules) from baseline to Week 16. For one of the studies, body weights were categorized by BMI (body mass index). Secondary efficacy endpoints included the change in Investigator's Global Assessment (IGA). Safety was assessed by monitoring adverse events (AEs).

The efficacy of SDD40 was consistent across the studies (two trials including n=72 and n=91 subjects) and meta-analysis (n=127 and n=142). SDD40 remained effective regardless of baseline disease severity and weight (with a weak correlation coefficient below 0.75); overweight or obese subjects with severe rosacea cleared at least as well if not better than those with a normal BMI and mild disease. The treatment was well tolerated with no to minimal gastrointestinal-related AEs.

Retrospective analyses have methodological limitations.

Consistency between study results including the meta-analysis supports the effectiveness and safety of SDD40, irrespective of the weight of the patient or rosacea severity based on inflammatory lesion count at baseline.

Keratoacanthomas are fast-growing cutaneous neoplasms that can be difficult to distinguish from squamous cell carcinoma, both clinically and histologically. The uncertain behavior of these neoplasms creates a challenge in management, and treatment choice often varies significantly between cases. The objective of this review is to discuss the most common and up-to-date treatment modalities used in the management of keratoacanthomas.

A literature search was performed using PubMed to access and review relevant keratoacanthoma treatment modalities published within the last 40 years. Keywords searched included "keratoacanthoma," "Grzybowski syndrome," "Ferguson-Smith syndrome," "Witten-Zac syndrome," and "Muir-Torre" syndrome.

Our search resulted in 3,408 articles, of which 67 articles were ultimately included in this review.

The current mainstay treatment of perimenstrual acne consists of systemic hormonal therapies, which can be problematic due to their side effects, stigma, or pill burden. Topical treatments are often used as well; however, data on their efficacy in treating this type of hormonal acne are limited.

We sought to evaluate the efficacy and tolerability of clindamycin phosphate and benzoyl peroxide 1.2%/3.75% combination gel in treating perimenstrual acne in adult women.

The single-group interventional pilot study was performed on 22 adult female subjects with perimenstrual acne. The subjects applied the investigational drug daily and were assessed every 14 days for a total of 99 days. Treatment success was evaluated by the investigators using the acne physician global assessment (PGA) scoring system. Drug tolerability assessment was based on the subject-reported adverse events, as well as physician-evaluated erythema, scaling, and dryness.

The study demonstrated a significant improvement in PGA score and lesion count, as well as patient-reported outcomes. The medication was well-tolerated in all subjects.

Limited sample size; lack of concurrent comparison group.

Clindamycin phosphate and benzoyl peroxide 1.2%/3.75% combination gel presents an important topical option for perimenstrual acne.

Despite Pakistan's stringent tobacco control policy, its effective implementation has always been a challenge, leading to rising tobacco consumption. The aim of the study is to determine the prevalence of tobacco consumption and associated risk factors in the adult population of deprived urban areas.

A community-based, cross-sectional survey was conducted from February to July 2019, comprising 607 adults residing in the urban squatter settlement of Lahore using a standardized questionnaire, the WHO STEPwise approach. The outcome was current use of tobacco and/or smokeless tobacco daily. Multivariable logistic regression was applied to determine factors associated with smoking and smokeless tobacco consumption.

Among 607 participants, about 64% were females, 49.3% were illiterate, 64.9% were currently unemployed, and 47.1% belonged to the low-income group. The prevalence of tobacco smoke was 10.5% (95% CI 8.07-12.93), and smokeless tobacco consumption was 8.6% (95% CI 6.38-10.82). Multivariable logistic effects of tobacco and endorsement of health warnings on tobacco packaging.
Migraines are prevalent and cause significant morbidity, decline in quality of life and healthcare costs universally. Treatment options are varied, but efficacy is limited. This review centers on Eptinezumab-jjmr, a humanized monoclonal specific to CGRP for the prevention of migraines in adults. Herein presented are the science and mechanism of action, indication and clinical evidence for use.

Migraines are severe, recurrent headaches, which are either episodic or chronic in nature. The pain is severe, often accompanied by co-morbid symptoms, such as photophobia, phonophobia, nausea and emesis, and is limiting in nature. It is a prevalent disorder that causes significant, worldwide disability, morbidity, suffering, and costs. The pathophysiology of migraines is actively studied, though recent research points to an initiating event causing migraine generation, that is then propagated by other brain regions, a significant one being the trigeminocervical complex. This is driven by biochemical transmitters, chiefly CGRP.
