Bacteriological Look at Gingival Crevicular Liquid within Teeth Reconditioned Using Repaired Dental care Prostheses: A good Within Vivo Research.
Purpose to describe a clinical case of ocular hypertension (OHT) in Axenfeld-Rieger Syndrome (ARS). Method Observational case report of a 43-year-old woman with background of OHT. The data was collected originally with a standardized electronic medical record. A complete ophthalmologic examination was performed. Results In the biomicroscopy, a posterior embryotoxon, iris atrophy with absence of crypts and irregularity of pigmentation, and discoria in OU were observed. Gonioscopy revealed an open angle with a prominent and anterior displaced Schwalbe line. Ocular fundus (OF) demonstrated small and oblique papillae, with normal neurorretinal ring. Functional tests were normal. The patient did not present systemic pathologies, so the diagnosis of Rieger anomaly was made. The IOP control was achieved with aqueous humor suppressants. Conclusions Glaucoma is the main cause of visual morbidity in patients with ARS, therefore a complete periodic ophthalmological exam is a priority. Abbreviations ARS = Axenfeld-Rieger Syndrome, RP = retinitis pigmentosa, IOP = Intraocular Pressure, BCVA = Best Corrected Visual Acuity, OR = right eye, OS = left eye, OU = both eyes, OF = ocular fundus, OCT = optical coherence tomography, VF = visual field, TBC = trabeculectomy. Objective To present a case of unilateral optic disc drusen, initially mis-diagnosed as optic neuritis, which led to chronic systemic administration of steroids and the development of hypercortisolism. Methods A 22-year-old female was referred because of the lack of improvement of the manifestations of optic neuritis despite the chronic use of systemic steroids. Presence of unilateral optic disc edema was initially observed, associated with ipsilateral scotomata and increased ipsilateral latency time in visually evoked potentials (VEP). Results A CT scan, A and B ultrasonography and autofluorescence of the optic disc confirmed the diagnosis of optic disc drusen. Conclusions In cases of optic disc edema, the possibility of drusen should always be examined even if functional disturbances, such as scotomas in visual fields or electrophysiological findings, are present, to avoid potential toxicity from unnecessary medications. The article describes a case of Goldenhar syndrome that had been diagnosed by an ophthalmologist in a medical consultation by school bullying due to a choristoma. A 15-year-old male patient, who had a nodular lesion with hair in the inferior temporal corneal-limbo-conjunctival of the left eye, was reported. He also had a facial asymmetry, with mild mandibular hypoplasia and malformation of the left external ear, where only an auricular appendage was formed. He denied similar family history and the history of genetic diseases, but revealed that his mother had used ibuprofen during the first 3 months of pregnancy and had gestational diabetes mellitus. Excisional biopsy of the eye lesion was performed and revealed a dermoid cyst. After the exegesis and with adequate multidisciplinary monitoring, the patient reported being very satisfied with the aesthetic result, returning with more confidence to his daily activities. That was a typical case of Goldenhar syndrome that has remained undiagnosed and untreated for more than a decade due to a lack of diagnosis despite its classic presentation. The delay in the approach resulted in social stigma and profound psychosocial damage. The importance of disseminating the correct knowledge of this pathology and of having an early multidisciplinary approach in these patients is emphasized, since the impact on the quality of life is significantly high. Objective To describe the results of toric intraocular lens (IOL) implantation in three atypical cases (four eyes) with cataract and corneal astigmatism one with bilateral keratoconus, one with pellucid marginal degeneration and one with buphthalmos due to congenital glaucoma. Methods Three patients (four eyes) with corneal astigmatism (one with bilateral keratoconus, one with pellucid marginal degeneration and one with buphthalmos due to congenital glaucoma) underwent cataract surgery by standard phacoemulsification and the implantation of toric IOLs in the capsular bag. The presence of corneal astigmatism was identified by automated keratometry and confirmed by Scheimpflug-based corneal tomography. The toric IOL implanted in all cases was a single-piece AcrySof Toric IOL (Alcon Laboratories, Inc.). Postoperative visual acuity, the reduction in the refractive astigmatism, the spherical equivalent (SE) and the rotational stability of the toric IOL were recorded for all the patients. Results Visual acuity incratism in atypical situations such as mild to moderate keratoconus, pellucid marginal degeneration and buphthalmos due to congenital glaucoma. Predicting the refractive outcome is difficult in atypical cases and the surgeon should have accuracy and consistency in the preoperative measurements, for achieving satisfactory postoperative results. We present the case of a 12-year-old boy with bilateral papilledema, relating moderate symptomatology and without an important medical history. Ophthalmological examination revealed a significant alteration of visual acuity, an important papilledema and macular edema in both eyes. Further investigations excluded infectious and autoimmune diseases, intracranial masses and congenital affliction. Because of an elevated opening pressure in lumbar puncture procedure, the diagnosis of intracranial hypertension was confirmed. After two weeks of treatment with corticosteroids, carbonic anhydrase inhibitor and hyperosmotic drug, the patient had an important structural and functional ophthalmological improvement.A 68-year-old male addressed to our clinic complaining of gradual loss of visual acuity and perceptual distortions. He had a history of extrathoracic hematoma and essential hypertension. The clinical assessment revealed bilateral retinal hemorrhages and white-green foveal and extrafoveal areas. The complete blood count (CBC) suggested a hematologic disorder. Objectives The paper presents the reconstructive options of the lower eyelid region in patients who have benefited from the excision of basal cell carcinomas. selleck products Methods The study was based on the clinical and evolutionary particularities analyzed in a series of cases, the patients benefiting from excision and reconstruction for the treatment of basal cell carcinomas located at the level of the orbital region. Results Following the surgical treatment, the local evolution was favorable with a good functional recovery. The aesthetic results were strongly influenced by the stage of the neoplasm, the final aspect being satisfactory for the patients included in the study. Conclusion Early diagnosis and rapid and effective surgical treatment are associated with favorable results from a functional and aesthetic point of view. Delayed surgery and treatment of relapses are associated with increased risks and inferior results. The Romanian health care system faced a long line of chaotic reformations that proved to be less patient oriented and more expensive, leading to an increased level of burnout in physicians. The phenomenon of burnout occurs more and more often in the pandemic context, even in ophthalmology services. Among psychological and occupational factors that may cause burnout, there are also organizational factors that may decrease the burnout levels. The aim of this paper was to bring more insight into the prevention of burnout in ophthalmology physicians by applying strategies related to the organizational factors, namely, learning organization and internal marketing, in sustainable manner. We elaborated the conceptual framework on a thorough literature review and concluded that a learning organization has a positive impact on the implementation of internal marketing practices, internal marketing practices decrease the level of burnout, if appropriately applied, as well as, a learning organization has the power to decrease the level of burnout in ophthalmology physicians. Objective To analyze therapy of glaucoma among the elderly population, 65 years old and above, in Sofia city, the capital of Bulgaria. Methods Two prophylactic ophthalmic campaigns were realized in the summer of 2016 and 2017, in Sofia city. All patients passed thorough ophthalmic examinations and filled pen-and-paper questionnaires. The results and files were collected and statistically analyzed to estimate current antiglaucoma therapy among the elderly citizens of Sofia city. Results The predominant number of persons who were diagnosed with glaucoma, was treated with conservative monotherapy. Prostaglandin analogues have been prescribed to more than half of the patients in both studies. Fixed combinations have been used in 86% of the cases who needed combined therapy with two substances and in 100% of those with three medications. Conclusions The therapeutic scheme used for treating glaucoma among elderly citizens of Sofia city is similar and corresponding to practice worldwide. Conservative monotherapy is used in mean 45%. When combined therapy is necessary, fixed combinations are predominantly prescribed with a target to minimize adverse events and to ensure better patients' compliance. A tendency of usage of preservative-free formulas to benefit patients' eye health has been noticed. Abbreviations POSE = prophylactic ophthalmic screening among elderly people, GSW = glaucoma screening week, EGS = European Glaucoma Society, PEX= pseudoexfoliative syndrome, PGAs = Prostaglandin analogues, CAIs = Carbonic Anhydrase Inhibitors. Objective Oxidative stress plays an important role in the pathogenesis of diabetic retinopathy. The aim of the present study was to investigate the effect of Crocus sativus L. styles (saffron) extract on oxidative stress indices of retina in streptozotocin (STZ)-induced diabetic rats. Methods Adult male Wistar rats (n=20) were randomized into the following 4 groups (n=6-7/ group) Control group (C) normal, Control + Saffron group (CS) non-diabetic rats treated with 60 mg/ kg of saffron extract, Diabetic group (D) and Diabetic + Saffron group (DS) diabetic rats treated with 60 mg/ kg saffron extract. We determined the activity of superoxide dismutase (SOD), glutathione peroxidase (GPx) and catalase (CAT) as markers of antioxidant response, as well as malondialdehyde (MDA) as a marker of lipid peroxidation. Results Induction of diabetes caused a significant decline in the activities of CAT (76.43%), SOD (53.43%) and GPx (77.58%). MDA levels were significantly lower in the DS group (0.878 ± 0.375 nmol MDA/ mg protein) as compared to D group (1.950 ± 0.299 nmol MDA/ mg protein, p 1.8-fold increase in the activity of CAT (p less then 0.05) and a 3-fold decrease in MDA levels (p less then 0.01). Conclusions This study showed that saffron extract has a protective antioxidant action in retinas of diabetic rats. Abbreviations C = Control group, CS = non-diabetic rats diabetic rats treated with 60 mg/ kg saffron extract, D = diabetic group, DS = diabetic rats treated with 60 mg/ kg saffron extract, SOD = superoxide dismutase, GPx = glutathione peroxidase, CAT = catalase, MDA = malondialdehyde, DM = diabetes mellitus, DR = diabetic retinopathy, ROS = reactive oxygen species, STZ = streptozotocin, GSH = reduced glutathione.
