51; 95% CI [-6.84, -0.18]) P = 0.04. Regarding time to seizure cessation after midazolam (n = 326) or BDZ (n = 322) administration, there is no significant difference between the two groups (mean difference = -0.03; 95% CI [-1.30, 1.25]), P = 0.97 and both are equally effective for controlling acute seizures (odds ratio = 1.06; 95% CI [0.43, 2.63]; n = 737).

In children with acute seizures, IN midazolam is equally effective in aborting seizure and decreases the total time from hospital arrival and cessation of seizures, eventually leading to faster cessation of seizure as compared to IV/rectal BDZ.

According to the International League Against Epilepsy (ILAE) criteria, epilepsy can be diagnosed after one unprovoked (or reflex) seizure when there is a ≥60% of seizure recurrence in the next decade. The application of this diagnostic criterion, however, is challenging because the risk of recurrence based on different etiologies is not easily retrievable from the literature.

To assess etiologies that permit a diagnosis of epilepsy after a single unprovoked seizure.

We conducted a systematic review of the literature search using PubMed, Scopus, and Cochrane library from January 1950 to December 2020 with the keywords recurrence, risk of recurrence, absolute risk, risk ratio, risk, seizures, epilepsy, structural, infectious, metabolic, immune, and genetic. We included articles that reported estimates of risks of a subsequent unprovoked seizure. Etiologies were categorized according to the ILAE epilepsy classification. The quality of the evidence was evaluated with PRISMA. Descriptive statistics were userovoked seizure based on low strength of evidence and moderate quality of cohorts. In recent years, fluoroquinolones have been found present in important water resources and food sources which compromises the food quality and availability, thereby, causing risks to the consumer. Despite the recent advancement in the development of analytical instrumentation for routine monitoring of fluoroquinolones in water, food, and biological samples, sample pre-treatment is still a major bottleneck of the analytical methods. Therefore, fast, selective, sensitive, and cost-effective sample preparation methods prior to instrumental analysis for fluoroquinolones residues in environmental, food and biological samples are increasingly important. Solid-phase extraction using different adsorbents is one of the most widely used pre-concentration/clean-up techniques for analysis of fluoroquinolones. Molecularly imprinted polymers (MIPs) serve as excellent effective adsorbent materials for selective extraction, separation, clean-up and preconcentration of various pollutants in different complex matrices. Therefore, synthesis of MIPs remains crucial for their applications in sample preparation as this offers much-needed selectivity in the extraction of compounds in complex samples. In this study, the progress made in the synthesis of MIPs for fluoroquinolones and their applications in water, food and biological samples were reviewed. The present review discusses the selection of all the elements of molecular imprinting for fluoroquinolones, polymerization processes and molecular recognition mechanisms. In conclusion, the related challenges and gaps are given to offer ideas for future research focussing on MIPs for fluoroquinolones.Automation of analytical sample preparation and testing is always attractive due to its high efficiency and continuous operation, but it can also be very challenging due to the operational complexity and method development. Studies focused on method development in automated systems are extremely rare and little has been published on the subject. In this paper, we investigate several unique and advanced aspects of this subject. The example product is selected containing two APIs to be tested by a single method to be productive. The product strengths of the two APIs are significantly different, making development of a single method much more challenging. In addition, we use a "green chemistry" approach, avoiding any organic solvent in the dissolving media in order to achieve an environmentally friendly solution. The selection and optimization of several key operational parameters are then discussed. The successful method is achieved using a new strategy called "Automated Online Standard (AO-STD)" methodology. This novel approach is compared to the manual operation procedure. The proposed approach can substantially reduce the method transfer time, generally considered the biggest obstacle to practical deployment of automated systems. The automated system applied in this paper is most suitable for content uniformity testing, but is also applicable for potency assays, blend uniformity testing, and degradation product tests.Gentamicin sulfate (GEN) is an aminoglycoside antibiotic with a narrow therapeutic range of plasma concentrations. The collection of venous blood represents a significant burden for patients, especially in neonatology. Dried blood spots (DBS) obtained from capillary blood can be an alternative for drug measurements in this particular population. This study aimed to develop and validate an assay for the quantification of GEN in DBS using UHPLC-MS/MS. Total GEN concentrations were obtained by adding the individual concentrations of the GEN forms C1, C1a, and C2. The assay used a DBS disk containing approximately 17 μL of blood for GEN quantitation in the range of 0.1-40 mg L-1. Measurement accuracy for total GEN was in the range of 102.6-108.6%, inter-assay precision was 11.3-13.1% and intra-assay precision was 9.1-12.8.% GEN was stable for 21 days at - 20 and 8 °C, but only for 24 h at room temperature. Blood Hct affected the accuracy within acceptable limits (93.8-95% at Hct% of 30, 104.3-113% at Hct% of 50). Blood spotted volume did not affect GEN measurement accuracy. Concentrations of GEN in DBS obtained after heel pricks were correlated to plasma levels in a small cohort of neonatal patients. However, percentual differences between estimated plasma concentrations and actual plasma levels presented values between - 64-35.3% (average difference of - 1.9%). The use of DBS for the measurement of GEN concentrations can increase access to TDM of this antibiotic due to the ease of sample collection and the facilitated specimen transportation logistics when testing is not available onsite.Headspace gas chromatography (HS-GC) has the potential to be used for water determination in pharmaceutical products. In this article, its use for the determination of water in solid samples is explored without the need of dissolution by means of the full evaporation technique (FET). This way, water is thermally removed from a small amount of sample which is directly weighed in the vial. This simplifies considerably the method since no diluent has to be searched and HS saturation is avoided. Blank corrections were performed to compensate for atmospheric moisture variation. U0126 Moreover, the performance of mass spectrometry (MS) and thermal conductivity detection (TCD) was compared. The method showed excellent figures of merit when working with TCD, such as R2> 0.99 and RSD less then 5% for each level of the calibration curve. Eight samples were carefully studied at different equilibration temperatures to find the optimal working conditions for each case and the results were compared to the ones obtained by Karl Fischer titration (KFT). Both methods showed restrictions and proper case by case optimization/validation turned out to be mandatory. Hyphenation with a flame ionization detector (FID) in series with the TCD showed the benefit to be useful for testing residual solvents (RS) simultaneously.Infants look longer and explore more following violations-of-expectation, but the reasons for these surprise-induced behaviors are unclear. One possibility is that expectancy violations heighten arousal generally, thereby increasing infants' post-surprise activity. Another possibility is that infants' exploration reflects the search for an explanation for the surprising event. We tested these alternatives in three experiments. First in Experiment 1 we confirmed that seeing an object violate expectations (by passing through a solid wall) increased infants' exploration of the surprising object, relative to when no expectancy violation was seen. Then in Experiment 2 we measured infants' exploration after they had seen the same violation event, but then an explanation for the event was provided (the wall was revealed to have a large hole in it). We found that providing this explanation abolished infants' surprise-induced exploration. In Experiment 3 we replicated this effect. Furthermore, we found that the longer infants looked at the explanation, the greater their reversal in exploratory preference (i.e., the more they ignored the surprising object). These findings demonstrate that preverbal infants both seek and recognize explanations for surprising events. Can the presence of unrelated flanker words change the way that lexical decisions are made to target words in the flankers task? Here we examined the impact of flanker presence on the effects of word concreteness. Target words had high or low concreteness ratings (e.g., fork, free) and were either presented in isolation or flanked to the left and right by an unrelated word (e.g., cold free cold) that was irrelevant for the task. Results revealed that the facilitatory effect of concreteness (faster responses to concrete words compared with abstract words) was significantly greater in the presence of flankers. A control experiment revealed the same pattern with pseudoword and nonword flankers. We conclude that the mere presence of flanking letter strings causes a greater depth of processing of target words. We further speculate that this might arise by flankers inducing a more "sentence-like" context by the presence of multiple, spatially distinct letter strings, that prohibits the use of more superficial decision processes and can be used to make lexical decisions to isolated words.
COPD is often accompanied by extra-pulmonary manifestations such as thrombo-embolic and hemorrhagic events, the disease is linked with atrial fibrillation (AF).

The objective of the current review was to assess the impact of chronic obstructive pulmonary disease (COPD) on outcomes of atrial fibrillation (AF).

PubMed, Scopus, Embase, and Web of Science databases were searched for studies comparing overall mortality, cardiovascular death, and other outcomes for AF patients with and without COPD. The data retrieved were subjected to both qualitative and quantitative analyses. The hazard ratios (HR) obtained for mortality in presence of COPD were pooled to meta-analyze using generic inverse variance function of RevMan 5.3 software. The association of various risk factors and HRs were pooled with 95% confidence interval (CI). The quality of the included studies was assessed using Newcastle Ottawa scale (NOS).

The hazard ratios (HR) were calculated with 95% confidence intervals (CIs). A total of seven studies were included.
