Low-dose radiation exposure to Canadians is exponentially increasing due to the influx of diagnostic imaging and medical procedures that utilize radiation. Despite the use of medical radiation since 1896, the standardized acceptable dose for the Canadian public is still debated. The current annual dose limit for the public is set at 1 millisievert (mSv). This set dose limit intrinsically restricts the use of medical radiation for diagnosis due to concerns of public health.
This systematic review is in the form of a retrospective meta-analysis of previous experimental studies and observational reviews of low-dose radiation health effects. A database search using PubMed and Medscape identified 1,296 articles using the terms "low-dose radiation", "radiation hormesis", "radiation safety", "dose exposure", and "medical radiation". Full text articles were excluded for the following reasons radiation dose level not <100mSv, results of radiation effects not included, or no inclusion of biologic effects on livif research, it is proposed that the current dose-response extrapolation for radiation-related health risks cannot be linearly based on the effects at high doses. By altering this knowledge, we could effectively improve patient diagnosis and public health by redefining the restrictions of current radiation limits within diagnostic imaging.Sunspots have played a key role in aiding our understanding of magnetohydrodynamic (MHD) wave phenomena in the Sun's atmosphere, and it is well known they demonstrate a number of wave phenomena associated with slow MHD modes. Recent studies have shown that transverse wave modes are present throughout the majority of the chromosphere. Using high-resolution Ca II 8542 Å observations from the Swedish Solar Telescope, we provide the first demonstration that the chromospheric super-penumbral fibrils, which span out from the sunspot, also show ubiquitous transverse motions. We interpret these motions as transverse waves, in particular the MHD kink mode. We compile the statistical properties of over 2000 transverse motions to find distributions for periods and amplitudes, finding they are broadly consistent with previous observations of chromospheric transverse waves in quiet Sun fibrils. The very presence of the waves in super-penumbral fibrils raises important questions about how they are generated, and could have implications for our understanding of how MHD wave energy is transferred through the atmosphere of a sunspot. Systemic lupus erythematosus is a chronic autoimmune disease with varied and unpredictable levels of disease activity. The ability to self-manage lupus is important in controlling disease activity. Our objective was to determine levels of patient activation toward self-management in lupus.
We used baseline results from the MyLupusGuide
study that had recruited 541 lupus patients from ten centers. We used the Patient Activation Measure (PAM), a validated self-reported tool designed to measure activation towards self-management ability, as our primary variable and examined its association with demographic, disease-related, patient-provider communication and psychosocial variables captured in our study protocol. Univariable and multivariable linear regressions were performed using linear mixed models, with a random effect for centers.
The average age was 50±14 years, 93% were female, 74% were Caucasian and the average disease duration was 17±12 years. The mean PAM score was 61.2±13.5 with 36% of participants scoring in the two lower levels, indicating low activation. Variables associated with low activation included being single, lower physical health status, lower self-reported disease activity, lower self-efficacy, use of more emotional coping and less distraction and instrumental coping strategies, and perceived lack of clarity in patient-doctor communication.

Recently, the poly(ADP-ribose) polymerase inhibitor (PARPi) olaparib has been approved for maintenance therapy after successful platinum-based chemotherapy in patients with germline mutations in BRCA1 and BRCA2. Approval was based on the POLO study that has shown a significant improvement in progression-free survival for patients with metastatic PDAC after at least 4 months of platinum-based chemotherapy. Hopefully, this first biomarker-directed targeted therapy for a relevant subgroup of pancreatic cancer patients is only the beginning of an era of personalized therapy for pancreatic cancer. The potential role for PARPi in improving survival in patients with pancreatic cancer containing somatic tumor mutations has yet to be established. Multiple studies investigating whether PARPi therapy might benefit a larger group of pancreatic cancer patients with homologous recombination repair deficiency and whether combinations with chemotherapy, immunotherapy, or small molecules can improve efficacy are currently underway. We here review the molecular basis for PARPi therapy in PDAC patients and recent developments in clinical studies.The aim of this study was to survey the present status and patterns of reirradiation (Re-RT) practice using external beam radiotherapy in Japan. We distributed an e-mail questionnaire to the Japanese Society for Radiation Oncology partner institutions, which consisted of part 1 (number of Re-RT cases in 2008-2012 and 2013-2018) and part 2 (indications and treatment planning for Re-RT and eight case scenarios). Of the 85 institutions that replied to part 1, 75 (88%) performed Re-RTs. However, 59 of these 75 institutions (79%) reported difficulty in obtaining Re-RT case information from their databases. The responses from 37 institutions included the number of Re-RT cases, which totaled 508 in the period from 2009 to 2013 (institution median 3; 0-235), and an increase to 762 cases in the period from 2014 to 2018 (12.5; 0-295). A total of 47 physicians responded to part 2 of the survey. Important indications for Re-RT that were considered were age, performance status, life expectancy, absence of distant metastases and time interval since previous radiotherapy. In addition to clinical decision-making factors, previous total radiation dose, volume of irradiated tissue and the biologically equivalent dose were considered during Re-RT planning. From the eight site-specific scenarios presented to the respondents, >60% of radiation oncologists agreed to perform Re-RT. Re-RT cases have increased in number, and interest in Re-RT among radiation oncologists has increased recently due to advances in technology. However, several problems exist that emphasize the need for consensus building and the establishment of guidelines for practice and prospective evaluation.Rejected water from sludge processing in wastewater treatment plants (WWTPs) is very harmful due to its high concentration of ammonia nitrogen and phosphorus. It is therefore necessary to find a low-cost and convenient technique to simultaneously remove ammonia nitrogen and phosphorus from rejected water. link3 In this study, natural granular zeolite was modified by NaCl and La(OH)3 to obtain a new material (Na@La-MZP), with several advantages compared with powdered zeolite. Na@La-MZP could remove 92.61% ammonia nitrogen (50 mg/L) and 99.01% phosphate (60 mg/L) at the optimal conditions of dosage 12.5 g/L, initial pH 6.0 and reaction time 12 hours, which enabled the effluent to satisfy the discharge standard (GB 18918-2002) for municipal WWTPs in China. The maximum adsorption capacity of Na@La-MZP was determined as 17.92 mg NH4+-N/g and 9.53 mg P/g by the Langmuir isotherm. Pseudo-second-order kinetics could well illustrate the adsorption process and show that the ammonia nitrogen and phosphate can be degraded by chemical reaction.
