V.OBJECTIVE To evaluate the risk of encountering unexpected uterine smooth muscle tumors of uncertain malignant potential (STUMPs) or sarcomas during surgical treatment of mesenchymal tumors of the uterus using morcellation. MATERIAL AND METHODS Data were collected retrospectively from subjects who were pathologically diagnosed with uterine leiomyoma or its variants, STUMP or other premalignant mesenchymal tumors of uterus, or sarcoma during surgical treatment between July 2014 and June 2017. RESULTS A total of 3785 women were investigated; 2824 laparoscopic procedures (74.6%) were performed, and an electronic power morcellator was used in 1636 patients (43.2%). Sixteen women (0.42%) were diagnosed with STUMP and 14 (0.37%) were diagnosed with uterine sarcoma. The incidence rate of unexpected STUMP or uterine sarcoma was 0.61% (23 of 3785 women); unexpected STUMP in 13 (0.34%), and unexpected sarcoma was in 10 (0.26%). Moreover, the unexpected leiomyosarcoma rate was 0.08% (3 in 3785). The rate of unintended morcellation of STUMPs was relatively high at 0.26% (10 in 3785), however, that for uterine sarcomas was 0.05% (2 in 3785). CONCLUSION The risks of unintended morcellation were very low for sarcomas and STUMPs, although the risk of the latter was approximately 5-fold that of the former. To reduce the unintended dissemination of tumors, patients suspected of having malignancies should be provided adequate information regarding their treatment options as well as their associated risks. Meanwhile, improved preoperative screening methods for STUMP and sarcoma should be established. V.OBJECTIVE Primary objective is to identify risk factors of endometriotic-cyst associated ovarian cancer (EAOC). Secondary objective is to evaluate the clinical characteristics of EAOC patients. MATERIALS AND METHODS A retrospective case-control study was conducted by analyzing data of patients from 1999 to 2014. Cases were endometriotic-cyst associated ovarian cancer with pathologically confirmed diagnosis. Controls were randomly selected with year-matched patients with benign ovarian endometriotic cyst. Univariate and multivariate with logistic regression analyses were used to identify patients' characteristics that were risk factors for endometriotic-cyst associated ovarian cancer. RESULTS Altogether, 158 controls and 79 EAOC cases were recruited. Mean age of the EAOC group was 13 years older than that of the control group (49 vs. 36 years). The most common stage of EAOC was stage I (59.74%). Clear cell subtype is the most commonly found in this population (60.76%). Univariate analysis showed that age ≥42 years, menopause, weight loss, cyst diameter ≥8.33 cm, presence of solid area, bilaterality and CA 125 higher than 117.6 units/ml were significant. Multivariate analysis showed that patients with age ≥42 years (OR 7.69, 95%CI 2.47, 23.87), menopause (OR 33.19, 95%CI 2.37, 465.12), weight loss (OR 11.94, 95%CI 1.52, 94.08), cyst diameter ≥ 8.3 cm (OR 10.56, 95%CI 4.39, 25.35) and presence of solid area by ultrasonography (OR 6.70, 95%CI 2.19, 22.35) were significant risk factors for EAOC. selleck compound CONCLUSION Advanced age, menopause, weight loss, cyst diameter ≥ 8.33 cm and presence of solid area from ultrasonography were important risk factors for EAOC. V.OBJECTIVE This study aimed to determine risk factors associated with the failure of uterine artery ligation at its origin following development of the retroperitoneal space (UALr) and evaluated its efficacy in decreasing estimated blood loss (EBL) during single-port total laparoscopic hysterectomy (SP-TLH). MATERIALS AND METHODS This study includes patient data collected prospectively from May 1st, 2013 to establish a registry for single-port surgery. Data for the present study were collected retrospectively from May 1st, 2013 to August 30th, 2016. Patients who underwent SP-TLH for a symptomatic benign disease. When bilateral UALr was performed successfully, the case was classified as part of the UALr success group. When only unilateral UALr was completed or UALr failed, the case was classified as part of the failure group. We compared patients' baseline characteristics and surgical outcomes between the two groups. RESULTS Bilateral UALr was successfully performed in 155 cases and failed in 64 patients. Body Mass Index (BMI) was significantly different between the two groups (24.1 kg/m2 vs. 22.86 kg/m2, p = 0.025). A BMI higher than 23.6 kg/m2 was a risk factor for UALr failure in a multivariate analysis (odds ratio = 2.42, p = 0.004). EBL was significantly lower in the UALr success group compared to the UALr failure group (100 [100.0-200.0] vs. 200 [100.0-250.0], p less then 0.001), and incidence of Hb decrease of more than 2 g/dl was higher in the UALr failure group (36.1% vs. 54.7%, p = 0.017). CONCLUSION We identified higher BMI as a risk factor for UALr failure and demonstrated the safety and efficacy of UALr in reducing blood loss during SP-TLH. V.OBJECTIVE The aim of this study was to evaluate the role of oxidative stress and reactive oxygen species (ROS) in the pathogenesis of endometriosis (EMs) and to investigate the role of antioxidant therapy on autophagy and the outcome of EMs. MATERIALS AND METHODS Experimental rats were given an peritoneal perfusion of N-acetyl-l-cysteine (NAC, 200 mg/kg) or catalase (CAT, 2000 U/mL). Immunofluorescence was then used to detect microtubule-associated protein light chain 3 (LC3). Western blotting was used to determine the levels of Beclin-1 protein while enzyme-linked immunosorbent assays (ELISAs) were used to measure ROS levels after treatment. RESULTS Fluorescent in situ hybridization showed that NAC and CAT influenced the levels of LC3, an autophagy marker; there were significantly lower levels of LC3 fluorescence in the EMs group (surgical group) of rats compared with controls (p less then 0.05).
