Synthetic complexes provide useful models to study the interplay between the structure and spectroscopy of the different Sn-state intermediates of the oxygen-evolving complex (OEC) of photosystem II (PSII). Complexes containing the MnIV4 core corresponding to the S3 state, the last observable intermediate prior to dioxygen formation, remain very rare. Toward the development of synthetic strategies to stabilize highly oxidized tetranuclear complexes, ligands with increased anion charge were pursued. Herein, we report the synthesis, electrochemistry, SQUID magnetometry, and electron paramagnetic resonance spectroscopy of a stable MnIV4O4 cuboidal complex supported by a disiloxide ligand. The substitution of an anionic acetate or amidate ligand with a dianionic disiloxide ligand shifts the reduction potential of the MnIIIMnIV3/MnIV4 redox couple by up to ∼760 mV, improving stability. The S = 3 spin ground state of the siloxide-ligated MnIV4O4 complex matches the acetate and amidate variants, in corroboration with the MnIV4 assignment of the S3 state of the OEC.Atmospheric aerosol over the Arabian Sea is significantly impacted by the long-range transported mineral dust from the surrounding continents. This transported mineral dust is hypothesized and tested during several studies to see the impacts on the surface ocean biogeochemical processes and subsequently to the Carbon cycle. It is, thus important to quantify dust contributions and their fluxes to the Arabian Sea. Here we assess temporal variability of dust concentration, their elemental characteristics as well as quantify their dry and wet deposition fluxes over the North-eastern Arabian Sea. The dust concentrations were found to vary from 59 to 132 µg m-3 which accounts for 50% to 90% of total mass during dusty days. However, its contribution during pre and post dust storms ranges between 6% and 60%. Relatively higher dust dry deposition flux of 28 ± 7 mg m-2 day-1 (range 20-44) is estimated for dusty days compared to pre and post dusty days (range 0.4-22 mg m-2 day-1). In contrast to dry deposition fluxes, sce water biogeochemical processes including primary productivity, in the Arabian Sea. Furthermore, modelling studies have highlighted on the role of dust (containing Fe) in fueling and enhancing primary productivity in the Arabian Sea. However, quantification of dust deposition fluxes (wet and dry) on seasonal time scale is missing in the literature. This paper aims to partially fulfil this research gap by providing a long-term data of wet and dry deposition fluxes over the northeastern Arabian Sea. We have also discussed their seasonal variability and factors affecting this flux. Thus, this study will be valuable contribution to the aeolian research community and have significant implication toward the role of aeolian deposition to the surface water biogeochemical processes in the Arabian Sea.Maximal rate of force development in adult humans is determined by the maximal motor unit discharge rate, however, the origin of the underlying synaptic inputs remains unclear. Here, we tested a hypothesis that the maximal motor unit discharge rate will increase in response to a startling cue, a stimulus that purportedly activates the pontomedullary reticular formation neurons that make mono- and disynaptic connections to motoneurons via fast-conducting axons. Twenty-two men were required to produce isometric knee extensor forces "as fast and as hard" as possible from rest to 75% of maximal voluntary force, in response to visual (VC), visual-auditory (VAC; 80 dB), or visual-startling cue (VSC; 110 dB). Motoneuron activity was estimated via decomposition of high-density surface electromyogram recordings over the vastus lateralis and medialis muscles. Reaction time was significantly shorter in response to VSC compared with VAC and VC. The VSC further elicited faster neuromechanical responses including a greater number of discharges per motor unit per second and greater maximal rate of force development, with no differences between VAC and VC. We provide evidence, for the first time, that the synaptic input to motoneurons increases in response to a startling cue, suggesting a contribution of subcortical pathways to maximal motoneuron output in humans.NEW & NOTEWORTHY Motor unit discharge characteristics are a key determinant of rate of force development in humans, but the neural substrate(s) underpinning such output remains unknown. Nanoparticles may enable encapsulation of drug or mRNA molecules in the form of carrier nanoparticles or can provide higher bioavailability in the form of drug nanoparticles. The ultimate goal in microfluidic approaches is the production of nanoparticles with narrow size distributions while avoiding contaminations and achieving sufficiently high throughput. To achieve this, a novel microfluidic precipitation device was developed and realized by two-photon polymerization mixing elements were designed in such a way that the liquids undergo a repeated Smale horseshoe transformation resulting in an increased interfacial area and mixing times of less than 10 ms. These elements and an additional 3D flow focusing ensure that no organic phase is exposed to the channel walls. The integration of a fluidic shield layer in the flow focusing proved to be useful to delay the precipitation process until reaching a sufficient distance to the injection nozzle. Lipid nanoparticle preparation with different concentrations of castor oil or the hard fat Softisan® 100 were performed at different flow rates and mixing ratios with and without a shield layer. Flow rates of up to 800 μl min-1 and organic phase mixing ratios of up to 20% resulted in particle sizes ranging from 42 nm to 166 nm with polydispersity indices from 0.04 to 0.11, indicating very narrowly distributed, and in most cases even monodisperse, nanoparticles. The occurrence of fouling can be completely suppressed with this new type of mixing elements, as long as Dean vortices are prevented. Moreover, this parameter range in the horseshoe lamination mixer provided a stable and continuous process, which enables a scalable production.
Surgery remains an important treatment option for renal hyperparathyroidism (rHPT). The number of long-term outcome studies of parathyroidectomy is limited.

All consecutive patients with a parathyroidectomy for rHPT between 09/2009 and 04/2021 in a Belgian tertiary referral hospital were prospectively enrolled. The main outcomes were (long-term) cured proportion and postoperative morbidity (hypocalcemia, recurrent laryngeal nerve (RLN) palsy, re-intervention for bleeding, and wound morbidity).

Sixty patients with a median age of 57 years were analyzed, including 23 patients before kidney transplantation, 23 patients without kidney transplantation, and 14 patients after kidney transplantation. Median time to transplant was 15 (6-24) months after parathyroidectomy. Morbidity was low with only two non-urgent returns to theatre (wound infection and non-compressive hematoma), two temporary RLN paralyses, and no 30-day mortality. Length of hospital stay was longer in patients with parathyroidectomy before kidney transplant, due to a more severe and prolonged need for calcium supplementation. After a median follow-up of 63 months, 37 patients (62%) were still alive, and 11 patients (18%) developed a recurrence.

CT and MRI disclosed a 3.2 × 1.9 cm soft-tissue mass of the right extraconal and intraconal orbit with sphenoid bone erosion. After debulking through an upper eyelid crease incision, the tumor was diagnosed as a spindle cell/sclerosing rhabdomyosarcoma. DNA sequencing was negative for an L122R mutation in MyoD1. Spindle cell/sclerosing rhabdomyosarcoma is an uncommon variant of this neoplasm, and only 2 patients with orbital tumors have been reported in 2 case series. Spindle cell/sclerosing rhabdomyosarcomas confined to the orbit are considered to have an excellent prognosis when treated with chemotherapy and radiation therapy. Diagnosis and treatment planning rely on histology, immunohistochemistry, and molecular analysis.Eosinophilic esophagitis (EoE) has been associated with autoimmune (AI) and connective tissue disorders (CTDs), but clinical correlates and treatment response to topical corticosteroids (tCS) for patients with both conditions are not well known. We aimed to determine the prevalence and clinical features of AI/CTDs in EoE patients, and assess the response to tCS. In this retrospective cohort study of adults and children newly diagnosed with EoE in the University of North Carolina EoE Clinicopathologic database, we extracted clinical characteristics and treatment response data. We compared EoE patients with and without AI/CTDs, identified independently associated factors, and explored treatment responses. Of 1029 EoE patients, 61 (5.9%) had an AI/CTDs. The most common AI/CTDs were psoriasis/psoriatic arthritis (P/PA) (1.7%), Hashimoto's (1.2%), and rheumatoid arthritis (RA) (1%). Compared to those without AI/CTDs, AI/CTDs patients were older (35 vs. 28 years, P = 0.004), more likely to be female (51% vs. 30%, P = 0.001), have insurance (93% vs. 78%, P = 0.004) and a longer symptom duration prior to EoE diagnosis (10 vs. 7 years, P = 0.02). Older age, female sex, having insurance, and having allergic rhinitis were independently associated with AI/CTDs. AI/CTD patients with EoE were less likely to have a symptom response (47% vs. 79%, P = 0.003). Overlap between EoE and AI/CTDs was uncommon, seen in approximately 6%, with P/PA, Hashimoto's, and RA being most frequent. In conclusion, older age, female sex, having insurance, and allergic rhinitis were independently associated with AI/CTDs. EoE patients with AI/CTDs had less symptom response, with trendtowards lower endoscopic and histologic responses, to tCS therapy.Orbital penetrating injuries from pencils are rare. This report describes a case of penetrating orbital injury in a young child with a retained blue-colored pencil core foreign body, which led to rapid onset of orbital abscess requiring surgical drainage. Intraoperatively, orbital tissues were stained a bright-blue color. Histopathological study of specimen also highlighted bright blue aggregates of foreign material infiltrating the orbital tissues.
