There is a paucity of data examining perioperative complications after microvascular free tissue transfer (MVFTT) reconstruction of the head and neck in this patient population.

Retrospective cohort study performed at two tertiary referral centers between August 2016 and May 2020.

Nine hundred and seventy-nine patients underwent MVFTT during the study period; of these 47 (5%) patients were taking chronic immunosuppressive medications. The most common indications for immunosuppression were solid organ transplant and autoimmune disease. Fourteen (30%) patients had surgical complications within 30 days of surgery 8 (17%) wound dehiscences, 6 (12%) hematomas, and 2 (4%) surgical site infections. There was one total and one partial flap failure with a 30-day reoperation rate of 4%.

In this paper, the focus is on the use of biological molecules found in body fluids or tissues for diagnosis and prediction of outcome in ovarian cancer patients. In cancer care, biomarkers are being used to develop personalised treatment plans for patients based on the unique characteristics of their tumour. This tailoring of care can be used to pursue specific targets identified by biomarkers, or treat the patient according to specific tumour characteristics. Surgery is one of the core treatments for ovarian cancer, whether it is offered in primary surgery or following chemotherapy in delayed surgery. Biomarkers already exist to guide the treatment of tumours with chemotherapy, but very little research has determined the value of biomarkers in tailoring surgical care for ovarian cancer. Such research is required to identify new biomarkers and assess their effectiveness in a clinical setting as well as to help identify specific tumour types to guide surgery. Biomarkers could help to determine the success of removing the disease surgically, or help to identify tumour deposits that persist after chemotherapy. All of these aspects would improve current practice. This Scientific Impact Paper highlights research that may pave the way towards bespoke surgery according to the biological characteristics of a tumour and aid gynaecological oncologists to provide surgical treatment according to individual need, rather than a blanket approach for all.
Predatory syrphids are an important functional group due to their potential for providing multiple ecosystem services. Adults feed on nectar and pollen, and can be effective pollinators, while larvae are voracious predators that can reduce aphid pressure. Still, little research has addressed their potential dual function in agroecosystems. In this study, we assessed the potential of two predatory hoverflies, Eupeodes corollae and Sphaerophoria rueppellii, for delivering concurrent pollination and biological control of Chaetospihon fragaefolii in greenhouse strawberries.

Both hoverfly species effectively pollinated strawberry flowers of two different varieties ('Elsanta' and 'Sonsation'), resulting in an increase in high-quality marketable fruits, a reduction of fruit deformities, and higher number of seeds per fruit compared to pollinator-excluded fruits. S. ruepellii had a significantly longer flower handling time than E. corollae, which translated to a more efficient pollination expressed as higher seed numbers per fruit after a single flower visit. By contrast, flowers that were open to multiple visits were more effectively pollinated by E. corollae, suggesting that E. corollae is potentially a better cross-pollinator than S. rueppellii. In addition, both hoverfly species suppressed aphid populations in strawberry (var. 'Sonata'), with S. rueppellii and E. corollae reducing aphid populations by 49% and 62%, respectively.

Part of the soil diversity are also eukaryotes that include fungi, algae, protists and Metazoa. Nowadays, soil eukaryotes are targeted with the same approaches developed for bacteria and archaea (prokaryotes). However, fundamental differences exist between domains. After providing a short historical overview of the developments of metabarcoding applied to environmental microbiology, we compile the most important differences between domains that prevent direct method transfers between prokaryotic and eukaryotic soil metabarcoding approaches, currently dominated by short-read sequencing. These include the existence of divergent diversity concepts and the variations in eukaryotic morphology that affect sampling and DNA extraction. Furthermore, eukaryotes experienced much more variable evolutionary rates than prokaryotes, which prevent capturing the entire eukaryotic diversity in a soil with a single amplification protocol fit for short-read sequencing. In the final part we focus on future potentials for optimization of eukaryotic metabarcoding that include superior possibility of functionally characterizing eukaryotes and to extend the current information obtained, such as by adding a real quantitative component. This review should optimize future metabarcoding approaches targeting soil eukaryotes and kickstart this promising research direction.
To investigate whether the Fibrosis-4 index can help stratify the risk of diabetes mellitus in patients with fatty liver disease.

Based on fatty liver disease and Fibrosis-4 index (cut-off value 1.3), we retrospectively divided 9,449 individuals, who underwent at least two annual health checkups, into four groups stratified by sex normal; high Fibrosis-4 index without fatty liver disease; low Fibrosis-4 index with fatty liver disease; and high Fibrosis-4 index with fatty liver disease.

Onset rates for diabetes mellitus in the normal, high Fibrosis-4 index without fatty liver disease, low Fibrosis-4 index with fatty liver disease and high Fibrosis-4 index with fatty liver disease groups were 1.6%, 4.3%, 6.8% and 10.2%, respectively, in men, and 0.6%, 0.9%, 5.3% and 7.0%, respectively, in women. Compared with the normal group, the high Fibrosis-4 index without fatty liver disease, low Fibrosis-4 index with fatty liver disease and high Fibrosis-4 index with fatty liver disease groups were at a significant risk for diabetes mellitus onset in both male and female participants. Furthermore, in both sexes, high Fibrosis-4 index with fatty liver disease remained a significant risk factor on multivariate analysis (high fibrosis-4 index with fatty liver disease group adjusted hazard ratio 4.03, 95% confidence interval 2.19-7.42 [men] and adjusted hazard ratio 6.40, 95% confidence interval 1.77-23.14 [women]).

Previous studies have suggested that negative schizotypy correlates with empathy, but mixed results have been shown for positive schizotypy. The valid sample consisted of 595 college students who completed the Interpersonal Reactivity Index, the Emotion Regulation Questionnaire, and the Chapman Psychosis Proneness Scales. Pearson correlations between schizotypy, emotion regulation, and empathy were performed to build potential mediating models. Structural equation modeling and bootstrap analyses were used to examine the mediation effects of emotion regulation. Our results showed that negative schizotypy was correlated with both cognitive and affective empathy whereas positive schizotypy only correlated with the Fantasy and Personal Distress subscales after multiple comparisons corrections. The Cognitive Reappraisal subscale of the Emotion Regulation Questionnaire was positively correlated with empathy whereas the Expressive Suppression subscale was negatively associated with affective empathy. More importantly, there was a significant mediating effect of cognitive reappraisal on the relationship between negative schizotypy and cognitive empathy. Our findings suggest that a higher level of negative schizotypy is associated with poorer cognitive and affective empathy, and cognitive reappraisal may mediate the effect of negative schizotypy on cognitive empathy. These findings highlight the optimal usage of cognitive reappraisal in social interactions, and may help to improve empathy, especially for individuals with a high level of negative schizotypy.
To investigate the longitudinal association between radiographic sacroiliitis progression and treatment with tumor necrosis factor inhibitors (TNFi) in patients with early axial spondyloarthritis (SpA) in a long-term inception cohort.

We included patients from the German Spondyloarthritis Inception Cohort who underwent radiographic assessment of the sacroiliac joints at baseline and at least once more during the 10-year follow-up. Two central readers scored the radiographs according to the modified New York criteria for ankylosing spondylitis. The sacroiliac sum score was calculated as a mean of the scores determined by both readers. TNFi use was assessed according to exposure in the current and/or previous 2-year radiographic interval. The association between TNFi use and radiographic sacroiliitis progression was examined by longitudinal generalized estimating equation analysis with adjustment for potential confounders.

In this long-term inception cohort, 10-year follow-up data on 737 radiographic intervals assessed in 301 patients with axial SpA (166 patients with nonradiographic axial SpA and 135 patients with radiographic axial SpA) were obtained.
