We aimed to examine the effectiveness of a consumer web-based app linked to primary care electronic health records (EHRs). CONNECT was a multicenter randomized controlled trial involving patients with or at risk of cardiovascular disease (CVD) recruited from primary care (Clinical Trial registration ACTRN12613000715774). Intervention participants received an interactive app which was pre-populated and refreshed with EHR risk factor data, diagnoses and, medications. Interactive risk calculators, motivational messages and lifestyle goal tracking were also included. Control group received usual health care. Primary outcome was adherence to guideline-recommended medications (≥80% of days covered for blood pressure (BP) and statin medications). Secondary outcomes included attainment of risk factor targets and eHealth literacy. In total, 934 patients were recruited; mean age 67.6 (±8.1) years. At 12 months, the proportion with >80% days covered with recommended medicines was low overall and there was no difference between the groups (32.8% vs. 29.9%; relative risk [RR] 1.07 [95% CI, 0.88-1.20] p = 0.49). There was borderline improvement in the proportion meeting BP and LDL targets in intervention vs. control (17.1% vs. 12.1% RR 1.40 [95% CI, 0.97-2.03] p = 0.07). The intervention was associated with increased attainment of physical activity targets (87.0% intervention vs. 79.7% control, p = 0.02) and e-health literacy scores (72.6% intervention vs. 64.0% control, p = 0.02). In conclusion, a consumer app integrated with primary health care EHRs was not effective in increasing medication adherence. Borderline improvements in risk factors and modest behavior changes were observed.Pseudomonas aeruginosa is an opportunistic pathogen capable of stably adapting to the antiseptic octenidine by an unknown mechanism. Here we characterise this adaptation, both in the laboratory and a simulated clinical setting, and identify a novel antiseptic resistance mechanism. In both settings, 2 to 4-fold increase in octenidine tolerance was associated with stable mutations and a specific 12 base pair deletion in a putative Tet-repressor family gene (smvR), associated with a constitutive increase in expression of the Major Facilitator Superfamily (MFS) efflux pump SmvA. Adaptation to higher octenidine concentrations led to additional stable mutations, most frequently in phosphatidylserine synthase pssA and occasionally in phosphatidylglycerophosphate synthase pgsA genes, resulting in octenidine tolerance 16- to 256-fold higher than parental strains. Metabolic changes were consistent with mitigation of oxidative stress and altered plasma membrane composition and order. Randomized sham-controlled clinical trial.
The objective of this study is to investigate the effects of repetitive transcranial magnetic stimulation (rTMS) compared to sham stimulation, on the development of lower limb muscle strength and gait function during rehabilitation of spinal cord injury (SCI).

Twenty individuals with SCI were randomized to receive rTMS (REAL, n = 11) or sham stimulation (SHAM, n = 9) and usual care for 4 weeks. rTMS (20 Hz, 1800 pulses per session) or sham stimulation was delivered over leg M1 Monday-Friday before lower limb resistance training or physical therapy. Lower limb maximal muscle strength (MVC) and gait function were assessed pre- and post intervention. Lower extremity motor score (LEMS) was assessed at admission and at discharge.
One individual dropped out due to seizure. More prominent increases in total leg (effect size (ES) 0.40), knee flexor (ES 0.29), and knee extensor MVC (ES 0.34) were observed in REAL compaaining effect.

Our objective was to assess the safety and feasibility of autologous mucosal olfactory ensheathing cell (OEC) and bone marrow mesenchymal stem cell (MSC) co-transplantation in people with chronic, complete (American Spinal Injury Association (ASIA) Impairment Scale (AIS) classification A) spinal cord injury (SCI).
This study was performed at Shohada Tajrish Hospital, Tehran, Iran.
Three individuals with the traumatic SCI of the thoracic level were enrolled. They received the autologous OEC and MSC combination through the lumbar puncture. All adverse events and possible functional outcomes were documented performing pre- and post-operative general clinical examination, magnetic resonance imaging (MRI), neurological assessment based on the International Standard of Neurological Classification for SCI, and functional evaluation using Spinal Cord Independence Measure version III (SCIM III).
No serious safety issue was recorded during the 2 years of follow-up. MRI findings remained unchanged with no neoplastic tissue formation. AIS improved from A to B in one of the participants. SCIM III evaluation also showed some degrees of progress in this participant's functional ability. The two other research participants had negligible or no improvement in their sensory scores without any changes in the AIS and SCIM III scores. No motor recovery was observed in any of the participants.
Overall, this 2-year trial was not associated with any adverse findings, which may suggest the safety of autologous OEC and bone marrow MSC combination for the treatment of human SCI.
Overall, this 2-year trial was not associated with any adverse findings, which may suggest the safety of autologous OEC and bone marrow MSC combination for the treatment of human SCI.Heterogeneous patient populations, complex pharmacology and low recruitment rates in the Intensive Care Unit (ICU) have led to the failure of many clinical trials. Recently, machine learning (ML) emerged as a new technology to process and identify big data relationships, enabling a new era in clinical trial design. In this study, we designed a ML model for predictively stratifying acute respiratory distress syndrome (ARDS) patients, ultimately reducing the required number of patients by increasing statistical power through cohort homogeneity. From the Philips eICU Research Institute (eRI) database, no less than 51,555 ARDS patients were extracted. We defined three subpopulations by outcome (1) rapid death, (2) spontaneous recovery, and (3) long-stay patients. A retrospective univariate analysis identified highly predictive variables for each outcome. All 220 variables were used to determine the most accurate and generalizable model to predict long-stay patients. Multiclass gradient boosting was identified as the best-performing ML model. Whereas alterations in pH, bicarbonate or lactate proved to be strong predictors for rapid death in the univariate analysis, only the multivariate ML model was able to reliably differentiate the disease course of the long-stay outcome population (AUC of 0.77). We demonstrate the feasibility of prospective patient stratification using ML algorithms in the by far largest ARDS cohort reported to date. Our algorithm can identify patients with sufficiently long ARDS episodes to allow time for patients to respond to therapy, increasing statistical power. Further, early enrollment alerts may increase recruitment rate.Compared to any single hydroclimatic variable, joint extremes of multiple variables impact more heavily on the society and ecosystem. In this study, we developed new joint extreme indices (JEIs) using temperature and precipitation, and investigated its spatio-temporal variation with observed records across Indian mainland. Analysis shows an alarming rate of change in the spatial extent of some of the joint extreme phenomena, tending to remain above normal. For example, above normal hot nights and wet days events expands at a rate of 0.61% per year considering entire Indian mainland. If the historical trend continues at the same rate, consecutive cold and wet day events will drop below the threshold of mean value observed in the base line period (1981-2010) everywhere in the country by the end of the twenty-first century. In contrast, the entire country will be covered by hot nights and wet days events only (frequency of occurrence will cross the threshold of mean value observed in the base line period). This observation is also supported by the CMIP6 climate model outputs. It is further revealed that extremes of any single variable, i.e. either precipitation or temperature (e.g., Extreme Wet Days, Consecutive Wet Days, Hot Nights, and Cold Spell Duration Index), do not manifest such an alarming spatial expansion/contraction. This indicates that the consideration of the joint indices of hydroclimatic variables is more informative for the climate change impact analysis.Extreme public health interventions play a critical role in mitigating the local and global prevalence and pandemic potential. Here, we use population size for pathogen transmission to measure the intensity of public health interventions, which is a key characteristic variable for nowcasting and forecasting of COVID-19. By formulating a hidden Markov dynamic system and using nonlinear filtering theory, we have developed a stochastic epidemic dynamic model under public health interventions. The model parameters and states are estimated in time from internationally available public data by combining an unscented filter and an interacting multiple model filter. Moreover, we consider the computability of the population size and provide its selection criterion. With applications to COVID-19, we estimate the mean of the effective reproductive number of China and the rest of the globe except China (GEC) to be 2.4626 (95% CI 2.4142-2.5111) and 3.0979 (95% CI 3.0968-3.0990), respectively. The prediction results show the effectiveness of the stochastic epidemic dynamic model with nonlinear filtering. The hidden Markov dynamic system with nonlinear filtering can be used to make analysis, nowcasting and forecasting for other contagious diseases in the future since it helps to understand the mechanism of disease transmission and to estimate the population size for pathogen transmission and the number of hidden infections, which is a valid tool for decision-making by policy makers for epidemic control.Herein, for the first time the carbon dots (CDs) were synthesized by reflux method from sawmill waste material. We also represent a novel strategy based on fluorescent CDs for determination of ponceau 4R and allura red dyes in soft drinks. Interestingly, both the dyes were sensitive and showed effective fluorescence quenching of the CDs owing to the interaction between them. The analytical applicability of CDs were evaluated for detection of both the dyes with a good linear relationship between the concentration range of 0.0 to 3.0 µg mL-1 and having detection limit 0.45 and 0.47 µg mL-1 for allura red and ponceau 4R dyes respectively. Meanwhile, the potential application of this novel fluorescent probe for dyes determination in real samples was validated in different soft drink samples with good accuracy and precision. Thus, these findings provides new insights for the potential risk assessment of both the dyes. Moreover, CDs acted as an excellent fluorescent material in cellular imaging owing to their cellular uptake and localization.
