Post-dural puncture headache is a common complication after spinal anesthesia for women who undergo cesarean delivery. Intravenous (IV) dexamethasone has been used to reduce the incidence and severity of PDPH with controversial results. This Systemic review and meta-analysis aimed to assess the effects of IV dexamethasone on PDPH.

This study is reported as per Preferred Reporting Items for Systematic and Meta-analysis. The primary outcome was the incidence and severity of PDPH. The secondary outcome variables were the postoperative total analgesic requirement and incidence of nausea and/or vomiting. Twelve randomized controlled trials with a total of 1548 women were included.

Intravenous (IV) dexamethasone had no effect on the incidence of PDPH (OR=0.64; CI, 0.39 to 1.05; I
=71%, P=0.08). Intravenous dexamethasone did not show a significant difference in the incidence of PDPH at 24h at 48h, and within one week postoperatively with p-values of less than 0.05. In a random-effect model, a pooled analysis showed that IV dexamethasone had no effect on the severity of PDPH in VAS (MD=0.78; CI, -2.27 to 0.71; I
=98%, P=0.30).

Intravenous dexamethasone failed to decrease the incidence and severity of PDPH in women who underwent cesarean delivery under spinal anesthesia.

COVID-19, a global pandemic, affects neurosurgical care in Indonesia. This study has objective to propose guideline and algorithm recommendation for the management of TBI patients during this pandemic, which can be used flexibly at neurosurgery centers, both in Indonesia and throughout the world.

We performed retrospective Cohort analysis from TBI database at tertiary public general hospitals. All neurotrauma cases from mid-February until mid-August 2020 was included in this study. The chronology of COVID-19 pandemics impact in Indonesia was defined by early period from mid-February until end of May 2020, and late period are latter. All subjects undergone the screening and perioperative measures that based on our proposes scoring system and algorithm as follows.

There are many guidelines that explain screening methods in neurosurgery patients in general, as well as neurotrauma in particular. But here, we proposed our own scoring and screening algorithm that has been developed based on conditions in Indonesia. In total of 757 neurotrauma cases data were collected from the pandemic starts in Indonesia.

Screening is a crucial initial step in this pandemic period, not only for COVID patients, but also all patients who enter the emergency room. The use of PPE is a necessity in several neurosurgery centers, especially with high COVID-19 case rates.

The management of neurotrauma patients with suspected and confirmed COVID-19 requires special attention, starting from admission of the patient in ER. Rapid scoring and screening are important and the highest level of PPE is mandatory during patient care.

The three-part question addressed was is breast-conserving surgery feasible after neoadjuvant chemotherapy for locally advanced breast cancer? Using the reported search, 19 articles were found, out of these 6 studies were deemed to be suitable to answer the question. The outcomes assessed were local recurrence rate. The best evidence showed that breast conserving surgery is safe in terms of local recurrence.
Post-operative pulmonary complications are common after exploratory laparotomy. Good abdominal muscle functioning is essential for forced exhalation and effective coughing. However, the impact of a laparotomy on abdominal muscle activity remains uncertain. The study aimed to assess abdominal muscle activity during forced exhalation following elective laparotomy.

A was carried out on those undergoing (n=30) their first elective laparotomy. Abdominal muscle activity, as percentage maximal voluntary contraction (%MVC), was assessed during forced exhalation using surface electromyography (EMG) for transverse abdominis (TrAb), external oblique (EO), and rectus abdominis (RA) pre-operatively and up to seven days post-operatively. Peak expiratory flow rate (PEFR) was assessed during the forced exhalation maneuver. Median %MVC was used to represent the trends and Z-scores to report the change from the baseline activity. Spearman's correlation was used for the correlation between %MVC and PEFR.

Pre-operatively, ve muscle, rather it is the EO that primarily contributes to forced exhalation. This should be considered while providing post-operative respiratory care. However, more research is required in this area to better understand the role of expiratory muscle training for those undergoing elective laparotomies.Diabetes mellitus (DM) has been treated with herbs for centuries and many herbs reported to exert antidiabetic activity. Laurus nobilis is an aromatic herb belonging to the Lauraceae family, commonly known as bay. This study aimed to investigate the activity of Laurus nobilis leave extracts on histopathological and biochemical changes in β-cells of streptozotocin (STZ)-induced diabetic rats. Thirty healthy adult male albino rats were included in the study and divided equally into 5 groups for 4 weeks as follow; control group (C), diabetic group (D), diabetic Laurus nobilis extract group (DLN), Laurus nobilis extract group (LN) and diabetic acarbose (DA) group. Histopathologically, D group rats exhibited various degenerative and necrotic changes in their liver, pancreas and kidney, whereas the DLN rats had nearly normal histology. Insulin immunostaining in the pancreatic beta cells was decreased in the D group compared to the C group, whereas the DLN group was similar to the C group. The glucose concentration decreased significantly in both diabetic rats treated with L. find more nobilis and acarbose (p less then 0.05). Additionally, the levels of aspartate aminotransferase (AST), gamma-glutamyltransferase (GGT) and alanine aminotransferase (ALT) enzyme were significantly decreased in both diabetic rats treated with L. nobilis and acarbose, compared to the D group (p ˃ 0.05). Outcomes of this study said that leave extracts of L. nobilis has valuable effect on blood glucose level and ameliorative effect on regeneration of pancreatic islets, it also restored the altered liver enzymes, urea, creatine kinase, total protein levels, calcium and ferritin to near normal.
We investigated positive and negative subjective well-being in relation to lower-extremity peripheral artery disease (PAD) in a sample of older adults.

4760 participants in the English Longitudinal Study of Ageing (ELSA) provided baseline data on symptomatic PAD, sociodemographic characteristics, lifestyle risk factors, and co-morbid conditions. Baseline and two-year follow-up data were available for life satisfaction, quality of life, and depressive symptoms.

Participants with PAD symptoms had lower baseline levels of life satisfaction (β = -0.03, p < .05) and quality of life (β = -0.04, p < .01), and more depressive symptoms (β = 0.03, p < .05). These associations remained statistically significant in multivariate analyses. Baseline PAD did not, however, influence well-being levels at two-year follow-up.

Greater awareness of the potential for chronic vascular morbidity to disrupt the lives of older adults is needed to inform effective multidisciplinary support and interventions that help maintain the quality of life of those affected.

The COVID-19 pandemic has highlighted the pressing need to access research outputs without the traditional economic and temporal barriers. This article explores the current scholarly communication landscape, assessing the reasons for the slower uptake of open access in SSH research. The authors discuss such frontiers as commercial interests, sources of academic prestige and discipline-specific genres. This article defines and discusses the key areas in which a research infrastructure can play a vital role in making open scholarly communication a reality in SSH (1) providing a federated and easy access to scattered SSH outputs; (2) supporting publication and dissemination of discipline-specific genres (e.g. monographs, critical editions); (3) providing help with evaluation and quality assurance practices in SSH; (4) enabling scholarly work in national languages, which is significant for local communities; (5) being governed by researchers and for researchers as a crucial factor for productive, useful and accessible services; (6) lastly, considering the needs of other stakeholders involved in scholarly communication, such as publishers, libraries, media, non-profit organisations, and companies. They conclude that a scholarly-driven, inclusive, dedicated infrastructure for the European Research Area is needed in order to advance open science in SSH and to address the issues tackled by SSH researchers at a structural and systemic level.Face masks have become the bulwark of COVID-19 prevention in the US. Between 10 April and 1 August, 2020, 33 state governors issued orders requiring businesses to require their customers and employees to wear face masks, and persons outdoors who could not social distance to do the same. We documented the policies and enforcement actions for these policies in each of the states. We used governors' orders and journalists' news reports as our sources. Our results show that the states used a variety of state and local (county and municipality) agencies to enforce business prevention behaviors and primarily local law enforcement agencies to enforce outside mask-wearing behaviours. In particular, law enforcement officers demonstrated a strong preference for educating non-mask wearers, and indicated a reluctance to resort to civil penalties that were enacted in the state orders. Businesses expressed a preference to have government agencies enforce non-mask wearing behaviours. But there was also a widespread reluctance on the part of local law enforcement to resort to legal remedies.
