Cloud computing has led to dramatic growth in the volume, variety, and velocity of cancer data. However, cloud platforms and services present new challenges for cancer research, particularly in understanding the practical tradeoffs between cloud performance, cost, and complexity. The goal of this study was to describe the practical challenges when using a cloud-based service to improve the cancer clinical trial matching process.

We collected information for all interventional cancer clinical trials from ClinicalTrials.gov and used the Google Cloud Healthcare Natural Language Application Programming Interface (API) to analyze clinical trial Title and Eligibility Criteria text. An informatics pipeline leveraging interoperability standards summarized the distribution of cancer clinical trials, genes, laboratory tests, and medications extracted from cloud-based entity analysis.

There were a total of 38,851 cancer-related clinical trials found in this study, with the distribution of cancer categories extract-like the Google service in this study-should not be used for automated clinical trial matching unless they can perform effective extraction and classification of the clinical, genetic, and medication concepts central to precision oncology research. A strong understanding of the practical aspects of cloud computing will help researchers effectively navigate the vast data ecosystems in cancer research.
Adherence to tamoxifen citrate among women diagnosed with metastatic breast cancer can improve survival and minimize recurrence. This study aimed to use real-world data and machine learning (ML) methods to classify tamoxifen nonadherence.

A cohort of women diagnosed with metastatic breast cancer from 2012 to 2017 were identified from IBM MarketScan Commercial Claims and Encounters and Medicare claims databases. Patients with < 80% proportion of days coverage in the year following treatment initiation were classified as nonadherent. Training and internal validation cohorts were randomly generated (41 ratio). Clinical procedures, comorbidity, treatment, and health care encounter features in the year before tamoxifen initiation were used to train logistic regression, boosted logistic regression, random forest, and feedforward neural network models and were internally validated on the basis of area under receiver operating characteristic curve. The most predictive ML approach was evaluated to assess featurdherence. Screening at treatment initiation may support personalized care, improve health outcomes, and minimize cost. Baseline claims may not be sufficient to discriminate adherence. Further validation with enriched longitudinal data may improve model performance.
Acute care imposes a significant burden on patients and cancer care costs. We examined whether an advanced practice provider-driven, cancer-specific urgent care center embedded within a large tertiary academic center decreased acute care use among oncology patients on active therapy.

We conducted a quasi-experimental study anchored around the Oncology Extended Care Clinic (OECC) opening date. We evaluated two parallel 4-month periods a post-OECC period that followed a 5-month run-in phase, and the identical calendar period 1 year earlier. Our primary outcomes included all emergency department (ED) presentations and hospital admissions during the 3-month window following the index provider visit. We used Poisson models to calculate absolute pre-OECC
post-OECC rate differences.

Our cohort included 2,095 patients in the pre-OECC period and 2,188 in the post-OECC period. We identified 32.6 ED visits/100 patients and 41.2 hospitalizations/100 patients in the pre-OECC period, versus 28.2 ED visits/100 patitalization rate. Barriers included clinic capacity, patient awareness, and physician comfort with advanced practice provider autonomy. Optimizing workflow and standardizing clinical pathways can create benchmarks useful for value-based payments.Age-related changes in decision making have been attributed to deterioration of cognitive skills, such as learning and memory. On the basis of past research showing age-related decreases in the ability to inhibit irrelevant information, we hypothesize that these changes occur, in part, because of older adults' tendency to give more weight to low-level, subordinate, and goal-irrelevant information than younger adults do. Consistent with this hypothesis, our findings demonstrated that young adults are willing to pay more for a product with superior end attributes than a product with superior means attributes (Study 1, N = 200) and are more satisfied after an experience with superior end than means attributes (Study 2, N = 399). Young adults are also more satisfied with a goal-relevant than with a goal-irrelevant product (Study 3, N = 201; Study 4, N = 200, preregistered). Importantly, these effects were attenuated with age. Implications for research on construal level and aging, as well as implications for policymakers, are discussed.In some Ugandan fishing communities, almost half the population lives with HIV. Researchers designate these communities "HIV hotspots" and attribute disproportionate disease burdens to "sex-for-fish" relationships endemic to the lakeshores. In this article, we trace the emergence of Uganda's HIV hotspots to structural adjustment. We show how global economic policies negotiated in the 1990s precipitated the collapse of Uganda's coffee sector, causing mass economic dislocation among women workers, who migrated to the lake. There, they entered overt forms of sex work or marriages they may have otherwise avoided, intimate economic arrangements that helped to "engineer the spread of HIV," as one respondent recounted.This study compared the sensitivity of peak running speed (PRS) measured during a submaximal run test (SRT) with a countermovement jump (CMJ) test to provide an alternate method of measuring neuromuscular fatigue (NMF). The study involved 20 semi-professional academy soccer players who performed a SRT and CMJ test, 24-hours pre-match, 24-, 48-, 72- and 96-hours post-match. Moderate decreases (effect-size (ES) ± 90% confidence intervals) were observed 24-hours post-match for CMJH; ES -0.70 ± 0.19, CMJPV; ES -0.92 ± 0.27 and PRS; ES -0.58 ± 0.11 with small decreases in CMJH; ES -0.36 ± 0.13 and PRS; ES -0.27 ± 0.23 still evident 48-hours post-match before returning to baseline 72-hours post-match. The results confirm PRS as an inexpensive alternate method of measuring NMF which, allows instantaneous real-time feedback on NMF. This can allow changes to be made immediately on-field within the post-match recovery phase without the need for further analysis or equipment.Well-being is a heterogeneous idea with inconsistent applicability to real-world circumstances. In this article, I explore A'uwẽ (Xavante) notions of social well-being from an ethnographic perspective. My data indicate many members of this Indigenous group understand wellness to involve not only health and harmony, but also certain modes of strife and inequality that are also viewed as desirable. A'uwẽ understandings of social wellness, including linkages to the environment, suggest that a broader and more locally contingent concept of social well-being than is evident in mainstream literature would benefit transcultural health efforts and policy involving Indigenous and other culturally distinct communities.The objective of this work is to propose a more effective way to prepare an in-house CO2 with known triple oxygen isotope compositions. The major experimental steps include (1) the O2 is combusted to CO2 on a graphite rod at 750 °C with Pt-catalyst for 3-4 min; and (2) converted CO2 is subsequently purified by two cryogenic traps. The results show high reproducibility of δ13C and δ18O values of the converted CO2 within 0.010-0.020 ‰ and 0.006-0.010 ‰ (1σ, SD), and the identical δ18O value within error with that of the original O2. Additionally, we have measured the triple oxygen isotope compositions of converted CO2 using an O2-CO2 Pt-catalyzed oxygen-isotope equilibration method. The measured δ17O values of CO2 show high reproducibility within 0.006 ‰ (1σ, SD), and are identical within error with the original O2 as well. Notably, our experiments also found that the O2 with heavier oxygen isotope ratios (δ18O > 40 ‰, VSMOW) might have a lesser conversion efficiency, and this effect, combined with the lighter isotope preferential fractionations during the reaction processes of O2 to CO and CO to CO2, may explain the observed lower 17O/16O and 18O/16O ratios of the converted CO2 relative to the original O2.The main objective of this study was to compare the physical response of para-footballers with cerebral palsy (CP) in official international football matches played at moderate altitude and sea level locations. Eighty-seven international CP footballers participated in this study. We divided participants according to the place of the international competition [sea level group (SLG) and moderate altitude group (MAG)], sport classes (i.e., FT1, FT2, and FT3), and match playing time (i.e., 40 min groups obtained more marked differences in the physical response variables. Para-footballers with CP who competed under altitude conditions showed a lower physical response during football matches, suggesting the implementation of specific preparation and training strategies to face the demanding environmental conditions.Cold-water immersion (CWI) is the gold standard therapy for exertional heat illness (EHS), and it is critical to perform CWI expeditiously when the core temperature exceeds 40°C; however, the treatment comes with risks, most notably hypothermia. Following a major marathon, three runners presented to our emergency department (ED) with symptomatic mild hypothermia requiring re-warming. Prior to developing hypothermia, all three were treated at the racecourse with CWI for EHS. We assessed the association between adolescent and parent gender related to subsequent adolescent drinking, binge drinking (>4 standard drinks), and alcohol-related harms.Results At mean age of 12.9 years about one in ten children report parental supply of alcohol which increases to about four in ten children by 17.8 years.
