This scoping review aimed to investigate the presence of discordance or concordance in the perceptions of end-of-life (EOL) care quality between consumers (i.e. patients aged over 60 in their last years of life and/or their informal caregivers) and clinicians, to inform further improvements in end-of-life care service delivery.

A scoping review of qualitative and quantitative studies was systematically undertaken by searching for English language publications in MEDLINE database and manual reference search of eligible articles. Thematic analysis was employed to identify and extract common concordance and discordance themes leading to the development of analytical constructs. Articles were eligible for inclusion if they reported on consumers' (i.e. older patients aged 60 + years in their final years of life and/or their informal caregivers) and clinicians' (doctors, nurses, social workers, etc.) perspectives on quality of medical, surgical or palliative/supportive care administered to older adults in the l" care and the need to prevent over-treatment are essential for their satisfaction with care and their ability to embrace the concept of a good death.
While progress has been made on promoting patient autonomy and respecting the family role in representing patient's best interest, gaps remain in terms of care coordination, communication of prognosis, public understanding of the meaning of goals of care including de-escalation of management and enactment of advance care directives by clinicians for people with diminished decision capacity. Public understanding of the meaning of "comfort" care and the need to prevent over-treatment are essential for their satisfaction with care and their ability to embrace the concept of a good death.Developmental coordination disorder (DCD) is a neurodevelopmental disorder occurring in 5-6% of school-aged children. Converging evidence suggests that dysfunction within cortico-striatal and cortico-cerebellar networks may contribute to motor deficits in DCD, yet limited research has examined the brain morphology of these regions. Using T1-weighted magnetic resonance imaging the current study investigated cortical and subcortical volumes in 37 children with DCD, aged 8 to 12 years, and 48 controls of a similar age. Regional brain volumes of the thalamus, basal ganglia, cerebellum and primary motor and sensory cortices were extracted using the FreeSurfer recon-all pipeline and compared between groups. Reduced volumes within both the left and right pallidum (Left F = 4.43, p = 0.039; Right F = 5.24, p = 0.025) were observed in children with DCD; however, these results did not withstand correction for multiple comparisons. These findings provide preliminary evidence of altered subcortical brain structure in DCD. Future studies that examine the morphology of these subcortical regions are highly encouraged in order replicate these findings.Plastics are synthetic polymers known for their outstanding durability and versatility, and have replaced traditional materials in many applications. Unfortunately, their unique traits ensure that they pose a major threat to the environment. While literature on freshwater microplastic contamination has grown over the recent years, research undertaken in rapidly developing countries, where plastic production and use are increasing dramatically, has lagged behind that in other parts of the world. In the South East Asia (SEA) region, basic information on levels of contamination is very limited and, as a consequence, the risk to human and ecological health remains hard to assess. This review synthesises what is currently known about microplastic contamination of freshwater ecosystems in SEA, with a particular focus on Malaysia. The review 1) summarises published studies that have assessed levels of contamination in freshwater systems in SEA, 2) discusses key sources and transport pathways of microplastic in freshwaters, 3) outlines what is known of the impacts of microplastic on freshwater organisms, and 4) identifies key knowledge gaps related to our understanding of the transport, fate and effects of microplastic.Persistent organic pollutants (POPs) continue to threaten the environment and human health. We have investigated levels of polychlorinated biphenyls (PCBs) and organochlorinated pesticides (OCPs) in breast milk samples. A questionnaire was also obtained from the study participants. A total of 48 healthy lactating mothers (mean age 29.5±0.8 years) living in Istanbul volunteered to participate in this study. High-resolution analyses of several OCPs and PCB congeners were done by gas chromatography. The levels of seven major PCB congeners (28, 52, 101, 118, 138, 153, and 180) and eight OCPs (α-benzenehexachloride, β-benzenehexachloride, δ-benzenehexachloride, hexachlorobenzene (HCB), and 2,4'-dichlorodiphenyldichloroethylene (2,4DDE), 4,4'-dichlorodiphenyldichloroethylene (4,4DDE), 2,4'-dichlorodiphenyltrichloroethane (2,4DDT), and 4,4'-dichlorodiphenyltrichloroethane (4,4DDT)) were determined. The analysis showed that the highest levels of PCBs were observed in PCB 52 (22.99±8.78 ng/g lipid), PCB 101 (12.22±7.8 ng/g lipid), PCB 28 (11.44±5.16 ng/g lipid), and PCB 153 (1.70±0.74 ng/g lipid). The highest OCPs detected were 4,4DDT (3.33±2.05 ng/g lipid) and 4,4DDE (0.86 ± 0.39 ng/g lipid), and the lowest was observed in HCB (0.016 ± 0.01 ng/g lipid). Our findings show that traces of PCBs and OCPs are still present in breast milk of lactating women living in Istanbul, and these pollutants decline in multipara women compared to primipara mothers. We also suggest that breast milk is a useful and representative biological tool for human biomonitoring of POPs.Water treatment for human uses is a vital asset to economic development. This article focuses on a combination of physical, biological, and chemical processes. Methods of water treatment require concrete or steel base structures, rendering them heavy, costly, and time-consuming to establish. Water treatment systems should be economically viable and practicable in design for developing societies; hence, structural builds should be simplified. Novel structures, a series of polyethylene cyclonic ponds for water treatment, were built and tested. Tanks were equipped with a settled material central outlet, water side outlet, and a pump shower system that enabled sludge removal without mechanical intervention. A freely flowing vortex was produced, aerated, given a biological treatment, and finally mixed with a chemical treatment step. Test establishment of the batch system enabled improvement in water quality, with removal performance as follows TSS (total suspended solids) = 98%, UV254 absorbance = 50%, DOC (dissolved organic carbon) = 56%, and turbidity=95%. The batch system builds of the current study required short construction time and were economically priced. Cyclonic ponds are suited to diverse usage. The value may be felt in the improvement of water quality for human consumption, utility, and ecosystem services to counter the extensive pollution caused by oil and gas extraction and other industries.Climate change is an important environmental issue that is causing global temperatures to rise. The primary environmental targets are to reduce carbon emissions and mitigate the impacts of climate change. The refrigeration system is a major emitter of greenhouse gases because it uses refrigerants with a high global warming potential. Due to its excellent thermophysical properties, the R134a is the most commonly used refrigerant in refrigeration systems; however, its high GWP will need to be disposed of earlier. check details To achieve global environmental objectives, conventional refrigerants need to be replaced with environmentally friendly and energy-efficient refrigerants. In the present work, a mathematical simulation has been carried out to check the performance of low-GWP refrigerant mixtures as environmentally friendly alternatives for R134a in a low-temperature system. In this study, a 190-L domestic refrigerator has been considered a low-temperature system. This simulation was performed using the MATLAB software, and the REFPROP database was used to obtain thermophysical properties of the refrigerants. The results showed that the COP of HFO mixtures decreased by 4-20% compared to R134a. The exergy efficiency of the R1234ze/R134a mixture improves by 4 to 16% as compared to the other mixtures and its performance is very similar to the R134a. Due to the environmentally friendly properties and flammability aspects, R1234ze/R134a (90/10) could be a good substitute for R134a in lower temperature applications and to satisfy the Montreal and Kyoto Protocol expectations. We collected and processed Boletus edulis (King Bolete) carpophores grouped in four batches based on their developmental stage (button stage, young-white, large-white, and large-yellow). The study aimed, for the first time, to examine the B. edulis content and effect of braising and to estimate the intake of total mercury (THg) and methylmercury (MeHg) from a single meal based on whole (wet) weight (ww) and dry weight (dw). In braised carpophores, THg concentrations ranged from 0.2668 ± 0.0090 to 0.5434 ± 0.0071 mg kg-1 ww at different developmental stages, whereas crude products concentrations ranged from 0.1880 ± 0.0247 to 0.2929 ± 0.0030 mg kg-1 ww. The button stage crude carpophores were more highly contaminated with THg than at later stages of maturity, but MeHg levels were lower (p less then 0.0001). On the other hand, braised button stage carpophores showed more MeHg than at later maturity stages. MeHg contributed at 1.9 ± 0.7% in THg in crude mushrooms and at 1.4 ± 0.3% in braised meals. The effect of braising was to increase the average THg and MeHg contents in fresh mushroom meals by 52 ± 31% and 53 ± 122% respectively, but a reduction of 40 ±14% and 40 ± 49% respectively was seen on a dw basis. The potential intakes of THg and MeHg from braised meals of B. edulis studied were small and considered safe.A new electrode was constructed via the anodic electropolymerization of poly-(L-serine) (PLS) on an rGO-Nafion-modified glassy carbon electrode (GCE) for the detection of the emerging organic contaminant naproxen (NPX). The morphology, crystal phase, and surface elements of the electrode were investigated with SEM, TEM, XRD, Raman, ATR-FTIR, zeta potential, C-H-O, and XPS analyses. Results of the surface analysis showed a porous structure resembling graphene sheets inside the Nafion/GCE architecture. Various electrochemical parameters, including scan rate, pH, and NPX concentration, were studied to evaluate the performance of the electrode. The synergistic effect of PLS and rGO-Nafion greatly facilitated the catalytic oxidation of NPX on PLS/rGO-Nafion/GCE. Electrochemical NPX oxidation was a one-electron transfer and adsorption limited process. The optimal working potential was 0.92 V vs. Ag/AgCl. The oxidation current of NPX increased with the increase in the concentration of analyte and scan rate but decreased with pH. The modified electrode exhibited excellent linearity with respect to NPX concentration in the range of 4.3 to 87 μM and limit of detection of 0.23 μM (S/N = 3). The PLS/rGO-Nafion/GCE is a fast, sensitive, reliable, and economical electrode for the detection of NPX in water.
