The study of drug release kinetics revealed that the release of 5-FU from CSNPs followed a diffusion controlled pattern. Oil-in-water (O/W) high internal phase emulsions (HIPEs) are widely used in foods, pharmaceuticals and cosmetics due to the high drug loading ratio, specific rheological behaviors and long shelf life. However, protective performance of active components within HIPEs maintains a low level. Herein, a series of carboxymethylated enzymatic hydrolysis lignin (EHL-CM-x) were synthesized by nucleophilic substitution and applied as macromolecular surfactant to stabilize the O/W HIPEs. It was found that EHL-CM-x combined with a small dosage of alkyl polyglycoside (APG) are able to stabilize HIPEs with 87 vol% soybean oil under neutral condition, which could be recognized as the highest internal phase reported in foods and pharmaceuticals. As a bioactive compound carrier, such EHL-CM-x stabilized HIPEs enable to provide outstanding UV, thermal and oxidation protection for sensitive natural extracts. The residual drug level obtained in this work is more than two times other gliadin/chitosan hybrid particles and sulfomethylated lignin stabilized HIPEs after UV irradiation. In vitro experiments showed that the minimum inhibitory concentration of curcumin within HIPEs against S. aureus and E. coli was 3.13 mg/mL and 12.5 mg/mL, respectively. Such lignin stabilized HIPEs could be potentially used in various areas, especially those with high stability and biosafety requirements. Ticks inject serine protease inhibitors (serpins) into their feeding sites to evade serine protease-mediated host defenses against tick-feeding. This study describes two highly identitical (97%) but functionally different Amblyomma americanum tick saliva serpins (AAS41 and 46) that are secreted at the inception of tick-feeding. We show that AAS41, which encodes a leucine at the P1 site inhibits inflammation system proteases chymase (SI = 3.23, Ka = 5.6 ± 3.7X103M-1 s-1) and α-chymotrypsin (SI = 3.18, Ka = 1.6 ± 4.1X104M-1 s-1), while AAS46, which encodes threonine has no inhibitory activity. Similary, rAAS41 inhibits rMCP-1 purified from rat peritonuem derived mast cells. Consistently, rAAS41 inhibits chymase-mediated inflammation induced by compound 48/80 in rat paw edema and vascular permeability models. Native AAS41/46 proteins are among tick saliva immunogens that provoke anti-tick immunity in repeatedly infested animals as revealed by specific reactivity with tick immune sera. Of significance, native AAS41/46 play critical tick-feeding functions in that RNAi-mediated silencing caused ticks to ingest significantly less blood. Importantly, monospecific antibodies to rAAS41 blocked inhibitory functions of rAAS41, suggesting potential for design of vaccine antigens that provokes immunity to neutralize functions of this protein at the tick-feeding site. We discuss our findings with reference to tick-feeding physiology and discovery of effective tick vaccine antigens. V.We reported a new approach for development of lignin-based carbon nanofibers (LCNFs) from two lignin-derived bio-oil (LB) obtained from catalytic depolymerization reaction combined with the organic solvent gradient separation process (including ethanol-soluble LB (EL) and tetrahydrofuran-soluble LB (TL)). This approach was particularly unique and translatable as it uses small molecule TL with high reactivity and low heterogeneity obtained via stalk lignin depolymerization to produce good morphologies and mechanical performances of LCNFs mixed with polyacrylonitrile under 85% high replacement rates. We first detailed characterization studies such as 2D-HSQC NMR, 31P NMR, FT-IR, GPC, TGA and DSC were conducted to understand the properties of EL and TL. Then, the goal of the influence of EL and TL on the LCNFs morphologies and mechanical properties were accomplished by using SEM, XRD, Raman and material testing machine. As compared with EL, the TL had excellent linear structure with relatively less total phenolic-OH groups and heterogeneity, which could improve lignin structure with better alignment along PAN, and thus increased the lignin replacement rates and mechanical performances of LCNFs. Degradation of sulphated polysaccharides from Grateloupia livida (GLP) was studied using H2O2-Vc combined with microwave treatment. Optimal conditions were determined after the Box-Behnken response surface analysis. The best degradation conditions were as follows microwave power 250 W, microwave temperature 54 °C, the concentration of H2O2 3 mM, degradation time 33 min. Under these conditions, the DPPH scavenging activity of the degraded GLP (DGLP) was 71.39 ± 0.17% at a concentration of 6.0 mg/mL. The degradation method is fast and effective, which is beneficial to improve the antioxidant activity of polysaccharides. Two sulphated polysaccharide constituents (DGLP-1 and DGLP-2) were isolated from DGLP and characterized, and their antioxidant activities were determined. The two polysaccharides had high sulphate content (sulphate content 25.18-25.26%), contained galactose as the major monosaccharide, and had molecular weights of 158.49 ± 0.43 and 18.06 ± 0.92 kDa, respectively. Antioxidant assays suggested that DGLP-1 and especially DGLP-2 possessed good antioxidant properties. STUDY OBJECTIVE To investigate the factors associated with poorer oncologic outcomes in patients undergoing laparoscopic radical hysterectomy (LRH) for early stage cervical cancer. DESIGN Multicenter retrospective study. SETTING Three gynecologic oncology referral centers. PATIENTS Patients with International Federation of Gynecology and Obstetrics 2009 stage IA (positive lymphovascular space invasion)-IB1 cervical cancer between January 2006 and June 2018. INTERVENTIONS LRH (Piver type II-III hysterectomies). Lymph-node dissection was accomplished according to the tumor characteristics. MEASUREMENTS AND MAIN RESULTS Surgical and oncologic outcomes were analyzed. Overall, 186 patients met the inclusion criteria, 16 (8.6%) experienced a recurrence, and 9 (4.8%) died of the disease (median follow-up period 37.9 months). Surgery-related complications did not influence disease-free survival. All the recurrences (16/16; 100%) occurred in patients with stage IB1 disease (p = .02), and 15 (93.7%) in cases involving tumors ≥2 cm. No association between positive lymph node and recurrence was detected (p =.82). Patients who had a preoperative diagnosis through conization (93; 50%) had a significantly lower rate of recurrence than those who underwent cervical biopsy (93; 50%) 1/93 (1.1%) vs 15/93 (16.1%); p less then .001). The subanalysis of patients with International Federation of Gynecology and Obstetrics stage IB1 cervical cancer showed that patients undergoing preoperative conization (vs cervical biopsy) were less likely to experience a recurrence (odds ratio 0.09; 95% confidence interval 0.01-0.55). CONCLUSION We confirmed that LRH was associated with a recurrence rate similar to that reported in the Laparoscopic Approach to Cervical Cancer trial. Tumor size ≥2 cm represents the most important risk factor influencing disease-free survival. However, we found that preoperative conization plays a potentially protective role in patients with an IB1 tumor. Herlyn-Werner-Wunderlich syndrome (HWWs) is a rare congenital malformation of the female urogenital track characterized by a triad consisting of didelphys uterus, obstructed hemi-vagina and ipsilateral renal agenesis. We report five consecutive cases of patients diagnosed with Herlyn-Werner-Wunderlich Syndrome (HWWs) treated in our center. Imaging studies with 2D/3D ultrasound and abdomino-pelvic MRI were obtained to confirm the diagnosis. The treatment consisted of one step surgical in-office hysteroscopic incision of the vaginal septum using 5Fr hysteroscopic bipolar electrodes. At the follow up visit, between one to two months after the initial procedure, the patients underwent diagnostic vaginoscopy with excision of exceeding vaginal tissue if needed, performed with a vaginal hysteroscopic approach. In office hysteroscopic treatment of patients diagnosed with Herlyn-Werner-Wunderlich syndrome (HWWs) is a safe and effective minimally invasive treatment modality that provides symptomatic relief and preserves fertility avoiding the cost and risks of the use of general anesthesia in an operating room setting. We recommend shifting the management of this challenging condition to the office setting. STUDY OBJECTIVE The aim of the Altis 522 study is to compare the safety and efficacy of the Altis® Single Incision Sling (SIS) to standard mid-urethral transobturator and/or retropubic slings through 36 months. In this report we present data through 12 months of follow-up. DESIGN Post-market, prospective, multi-center, non-randomized cohort design. SETTING The study is performed at 23 hospitals in the United States and Canada. PATIENTS Adult female patients with stress urinary incontinence (SUI) clinically indicated for an incontinence sling were treated (n=355). INTERVENTIONS Altis SIS is compared to any FDA cleared transobturator or retropubic sling. MEASUREMENTS Collected measures include device- and/or procedure-related serious adverse events, relevant non-serious and all adverse events; as well as, revision surgery. Objective efficacy measures include 24-hour pad weight, dryness (defined as pad weight ≤ 4.0 grams) and cough stress test (CST). Subjective outcome measures include Patient Global Impression . BACKGROUND Data are limited on the differential response to long-acting bronchodilators in older versus younger adults with asthma. OBJECTIVE To determine whether the response to tiotropium Respimat differed in older versus younger patients with asthma. METHODS Post hoc analyses of 4 randomized, double-blind, placebo-controlled studies in adults with asthma were carried out. Two studies compared tiotropium Respimat 5 μg once daily with placebo, both added to high-dose inhaled corticosteroid (ICS) plus long-acting β2-agonist (ie, severe asthma). The other 2 evaluated tiotropium Respimat 2.5 or 5 μg once daily, salmeterol 50 μg twice daily, or placebo, all added to medium-dose ICS (moderate asthma). Data were analyzed in 2 pools (1) severe and (2) moderate asthma. ABT-199 Efficacy end points trough and peak FEV1; trough forced vital capacity; Asthma Control Questionnaire total score and responder percentage, all at week 24. One set of analyses was performed with age as a continuous covariate; the second was conducted in categories less than 40, 40 to 60, and more than 60 years, with treatment-by-age subgroup interaction P values obtained. Safety was analyzed in age categories. RESULTS Across the age categories, treatment-by-age subgroup interaction P values for trough FEV1 were .13 and .77 for patients with severe and moderate asthma, respectively, not indicating significant impact of age on overall treatment effect, with this observation replicated in the 2 continuum analyses. The other end points (including safety) were also not impacted by age. CONCLUSIONS Once-daily tiotropium Respimat add-on to ICS or ICS/long-acting β2-agonist therapy was effective and well tolerated in patients with asthma independent of age.
