Brain abscesses (BA) form approximately 8% of intracranial masses in developing and 1-2% in western countries. Fungal BA (FBA) are aggressive and represent a catastrophic manifestation compared to protozoan and bacterial BA. Diagnosis of FBA is rare and usually done postmortem.
The present retrospective study analyses the clinico-mycological aspects of FBA presented to our neurosurgical services over a period of 38 years, from January 1979 to April 2017.
Patients diagnosed as definitive cases of FBA were included in the study. Clinico- demographic and microbiological data were collected from medical records. BA pus was examined for fungal etiology using standard microbiological procedures.
During the period of 38 years out of total 2,916 brain abscesses, 29 cases of FBA were diagnosed with an overall incidence rate of 0.99% per year. Cladophialophora bantiana (44%) was the most predominant isolate followed by Aspergillus spp and others. Male preponderance was seen with a malefemale ratio of 4.81. Therngal infection. Advance in the diagnostic modalities show promising in diagnosis of FBA. High index of suspicion with early diagnosis, prompt antifungal therapy and aggressive surgical management is required as FBA are associated with high mortality rate.
The long-term health effects of physical child abuse are well documented in self-report, retrospective studies. However, there have been few longitudinal, multimethod studies on physiological responses to stress and the onset of chronic disease, thereby slowing the advancement of prevention and intervention programs.
This study used survey data from an extended longitudinal study to examine prospective and retrospective associations between measures of physical child abuse and adult health in the 40s.
Data are from an ongoing longitudinal study of the correlates and consequences of child maltreatment that began in the 1970s with a sample of 457 children.
Bivariate correlations and multiple regression models with covariates were used to assess associations between measures of physical child abuse and outcomes of self-reported health in adulthood.
Physical child abuse, measured retrospectively, was moderately related to reports of overall health, as well as a number of adult health problems and conditions, such as back and chest pain, hypertension, and certain forms of cancer. Associations were also observed for lifetime alcohol problems and past-year doctor and emergency room visits. Fewer associations between prospective parent self-report measures of physical child abuse and adult health were identified, although child welfare (official record) reports performed similarly to retrospective measures.
This study adds important information on the long-term health effects of child physical abuse, as well as measurement differences in the prediction of adult health outcomes. Conclusions drawn from prospective and retrospective studies of abuse are at best inconsistent, and possibly incompatible.

Forearm fractures are common pediatric injuries. Most displaced or angulated fractures can be managed via closed reduction in the operating room or in the Emergency Department (ED). Previous research has shown that emergency physicians can successfully perform closed reduction within ED; however, the fracture morphology amendable to ED physician reduction is unclear. The aim of this study is to detail the fracture characteristics associated with successful reduction by ED physicians.
We conducted a retrospective study of children (aged <18 years) presenting to the ED of a tertiary care children's hospital (annual census 90,000) between January 2018 and December 2018 with closed distal and midshaft forearm fractures requiring reduction. Data collected included patient demographics, fracture morphology, management, and complications. Successful ED physician reduction was based on predefined criteria. Orthopedic referrals included those patients sent directly to the operating room, closed reductions perfos are more likely to need treatment by orthopedics.
In this series, fractures most amenable to reduction by ED clinicians include distal greenstick fractures, whereas midshaft and completely displaced fractures are more likely to need treatment by orthopedics.
Wearing a lumbosacral orthosis (LSO) is known to influence spine mechanics, but less is known about how LSOs affect motor control. Whether the use of a LSO can negatively affect motor control of the lumbar spine is still under debate.
The current study examined the immediate effects of two flexible LSOs (extensible and non-extensible) on the anticipatory postural adjustments that prepare the spine for a predictable perturbation.
A comparative study using a repeated measures design in a laboratory setting.
Healthy controls (n=20) and participants with low back pain (n=40) performed a rapid arm flexion/extension cycle with and without these LSOs. The latency between the activations of the shoulder and different back (iliocostalis lumborum) and abdominal (rectus abdominis, internal and external obliques) muscles, as measured with surface electromyography, was used as the outcome.
The effects, which were comparable between groups and between LSOs, were mixed, with some muscles showing significantly (p ˂etrimental effects to the control of lumbar stability.Exocrine glands in the legs of social insects are found throughout all leg segments, but studies of exocrine glands in legs of solitary insects are very limited. We discovered a novel gland at the apex of the fore, mid and hind femurs from six representative species of Cicadidae, which we propose to name as the epithelial femoral gland. The epithelial femoral gland is located between the paired apodemes and the articulation membrane within the apex of the femur, which faces the proximal articulation region of the tibia. The epithelial femoral gland in the midlegs is less developed than that in the fore- and hindlegs within a species. The glandular cells belong to class-1, which contain a large amount of rough endoplasmic reticulum, secretory vesicles and Golgi bodies, indicating that these cells may produce a proteinaceous secretion. Details of the epithelial femoral gland at the ultrastructural level suggest that it may function to produce nourishing substances to the joint between femur and tibia. The less developed epithelial femoral gland in the midlegs and the slight difference in the glands between fore- and hindlegs within a species could be related to the functional differentiation of the corresponding legs in cicadas. Further studies of exocrine glands in the legs of cicadas and other Cicadomorpha insects may improve our understanding of the structural and functional divergence of legs in hemipteran insects.
We sought to investigate aortic stenosis (AS) progression rate (pr) with the comparison between bicuspid aortic valve (BAV) and tricuspid aortic valve (TAV) morphology.
We compared ASpr in patients with BAV and TAV examined by transthoracic echocardiography (TTE) in the years 2004-2019.
Data from 363 TTEs in 161 AS patients (median age 70 [61-77] years; 63% men; 25% with BAV; 20% with severe AS) performed at different time points (median time interval 10 months) was analyzed. We assessed changes of AS severity with peak velocity through aortic valve (Vmax), mean/peak pressure gradients (MG/PG), aortic valve area by planimetry and continuity equation (AVAce). We compared pr (defined as parameter change per year) between the BAV and the TAV groups. Onvansertib clinical trial BAV patients showed faster ASpr with odds ratio 3.467 and 95% confidence intervals 1.36 to 8.86, moreover, expressed as a quicker AVAce decrease 0 (-0.4-0.0) in the BAV vs. 0 (-0.15 - 0.0) cm
/year in the TAV group, p=0.02. Furthermore, in BAV, female sex was associated with lower ASpr (p=0.01), and in the whole group a larger aortic diameter was a predictor of faster progression (p<0.001).
The ASpr, expressed as a decrease in the AVAce, was faster in BAV. Moreover, ASpr depends on both valve morphology being faster in BAV and Vmax increase. Furthermore, the female sex was related to slower pace of AVA reduction in BAV subgroup whereas the larger baseline aortic diameter associated to faster AS progression in the whole studied group.

These approaches will help advance our knowledge on cognition in primates, including humans, and therapy for human neurological and psychiatric disorders.Maternal gestational diabetes mellitus (GDM) is one of the most common medical complications of pregnancy, which can adversely affect the short- and long-term health of mothers and newborns. In recent years, several studies have revealed the early impact of maternal hyperglycemia on fetal growth trajectory and birth weight abnormalities in GDM-exposed pregnancies. However, an intense debate continues regarding the mode and optimal timing of diagnosis and treatment of this condition. The purpose of this review is to provide a brief overview of the understanding of GDM and its implications for fetal growth, addressing the modulatory role of medical nutrition therapy and available pharmacological antidiabetic agents (i.e. insulin, metformin, and glyburide), and to identify gaps in current knowledge toward which future research should be directed.
