Solitary confinement is still used in prisons in the USA, despite its links to poor health. Past research suggests that there may be disparities by race, ethnicity, sex and mental disorders regarding who is placed in solitary confinement, although nationwide studies have been sparse.

To explore possible disparities by race, ethnicity, sex, sexual orientation, adverse childhood experiences and mental disorders in solitary confinement as a disciplinary action for adults incarcerated in USA prisons.

Data come from a recently released national survey of 24,848 adults incarcerated in the USA-the 2016 Survey of Prison Inmates. Logistic regression models were used to identify disparities in the use of disciplinary action and solitary confinement as a disciplinary action, while controlling for type of rule violation.

After controlling for rule violation type, solitary confinement was used as a disciplinary action at higher rates for people who were multiracial, as compared to white (aOR=1.30), male, as comparders in prison or transfer to health facilities.Lupus erythematosus is an autoimmune disease that often affects the skin. Cutaneous manifestations are generally subdivided into different subtypes, including acute, subacute, and chronic courses. Management of lupus erythematosus cutaneous manifestations during pregnancy remains a clinical challenge until nowadays. To date, no recommendations have been published specifically for the treatment of this condition in pregnant women, so therapeutic strategies are mainly based on recommendations for general population and other rheumatologic and dermatologic diseases during pregnancy. This challenge is compounded by a lack of evidence-based studies, as clinical trials in pregnant women are considered unethical in many circumstances, so data are often extrapolated from low-evidence sources. The aim of this article consists in review currently evidence of treatment of lupus erythematosus cutaneous lesion in pregnant women.The aim of our retrospective study was to evaluate the efficacy of a continuous therapy with a lower dosage of gemcitabine compared to those usually administered in patients with cutaneous T cell lymphomas (CTCL). Twenty-two patients received different dosages of gemcitabine. Dosage and schedule of the drug were chosen on the basis of clinical features. Gemcitabine was given at 1000 mg every 15 days in 13 patients (four mycosis fungoides [MF], nine Sezary syndrome [SS]); at 1000 mg at days +1, +8, +15 in six cases (three MF, three SS). All patients had been previously treated four patients had received both skin directed and systemic treatments. Eighteen patients had received photopheresis, IFN, chemotherapy and immunotherapy. The objective response rate (CR + PR) among all patients was 54.5% (12 of 22 patients) with a CR of 4.5% (one of 22 patients) and a PR of 50% (11 of 22 patients). Patients with SS had an ORR of 61.5% (eight of 13 patients) with one CR (7%) and seven PR (53.8%); patients with MF showed an ORR of 55.6% (five of nine patients) but no patients experienced CR (0%). The schedule with the highest efficacy and the lowest toxicity profile was 1000 mg every 15 days. Median progression free survival and overall survival in all patients were 17 and 45 months respectively. Gemcitabine was generally well tolerated. We have demonstrated that a much lower dose of gemcitabine (1000 mg once every 15 days) in patients with advanced-stage and refractory CTCL can lead to a durable response, with tolerable and manageable adverse effects.Meningioma-associated protein 30 (MAC30) is a vital modulator of malignant transformation in numerous cancers. Yet, the involvement of MAC30 in nasopharyngeal carcinoma (NPC) is undetermined. This study was devoted to the study of the function of MAC30 in NPC, along with the molecular mechanism of its involvement in disease progression. This study showed an elevated level of MAC30 in NPC tissues and cell lines. Proteasome inhibitor MAC30 silencing impeded the proliferation, colony formation, and invasion of NPC cells, while MAC30 upregulation had the opposite effects. MAC30 regulated the Wnt/β-catenin pathway via affecting the protein kinase B (Akt)/glycogen synthase kinase-3β axis. Akt inhibition markedly abrogated the MAC30-mediated activation of the Wnt/β-catenin pathway. Restoration of β-catenin reversed MAC30 silencing-induced tumor-inhibiting effects. Knockdown of MAC30 weakened the tumorigenic potential of NPC cells in vivo. This study elucidates the fact that inhibition of MAC30 produces antitumor effects in NPC via affecting the Akt/Wnt/β-catenin pathway.In this study, two natural clay minerals were combined with hydrogels to study the influence of natural adsorbents on the adsorption performance of hydrogels. Here, we separately doped bentonite and vermiculite and discussed their mechanical properties and adsorption properties. It was found that the compressive performance of the hydrogel added with clay increased by 21.6% and the swelling performance decreased or increased to varying degrees. Regarding the adsorption performance of hydrogels, it can be seen from the adsorption Langmuir isotherm model that the adsorption capacity of clay-hydrogels is improved to varying degrees (6.6%-15.8%) compared with non-clay-hydrogels, and clay-hydrogels have different degrees of improvement (6.6%-15.8%). The hydrogel has a removal efficiency of more than 95% for low concentrations of heavy-metal ions and dyes. In addition, the clay-hydrogel has low cost and is easy to prepare, and can be recycled many times. Therefore, the material is of great significance for the treatment of pollutants. PRACTITIONER POINTS The effect of natural clay on the adsorption performance of hydrogels was studied. Clay can enhance the compression and adsorption properties of hydrogels. Genetic variations in innate immune genes have been found to be associated with Covid-19 infections. A single-nucleotide polymorphism (SNP) in a promoter region of tumor necrosis factor alpha (TNF-α) gene, TNF-α -308G>A, increases expression of TNF-α protein against infectious diseases leading to immune dysregulations and organ damage. This study aims to discover associations between TNF-α -308G>A SNP and Covid-19 infection. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) was used for genotyping a general Kurdish population and Covid-19 patients. The homozygous mutant (AA) genotype was found to be rare in the current studied population. Interestingly, the heterozygous (GA) genotype was significantly (p value = 0.0342) higher in the Covid-19 patients than the general population. This suggests that TNF-α -308G>A SNP might be associated with Covid-19 infections. Further studies with larger sample sizes focusing on different ethnic populations are recommended.We investigate the effect of extreme temperatures on mortality and emergency hospital admissions, and whether local social care allows to mitigate their adverse effects. We merge monthly administrative data on mortality and hospital discharge from Italian municipalities for the period 2001-2015 with daily data on local weather conditions, and yearly data on disaggregated municipal expenditure. We compare two different measures of temperature shocks, one using the conventional approach based on absolute levels (without accounting for regional heterogeneity) and the other based on deviations from local mean temperatures. The former approach shows noisy evidence of an increase in mortality due to extremely hot days while the latter approach shows a large increase in mortality and hospital admissions for cardiovascular and respiratory diseases due to both cold and hot days. These effects are mostly driven by the oldest age group and partially by young children. Then, we report evidence of a mitigating effect of social expenditure on the impact of extremely hot and cold days on both emergency hospital admission and mortality rates. A back of the envelope calculation suggests that the additional social care expenditure is fully compensated by the benefits arising from the lower impact of temperature shocks.General practices are rooted in the local community and considered to be particularly well-positioned for engaging in preventive and health-promoting activities. The overall aim of the scoping review is to identify priorities and gaps in research published in the past 20 years on preventive and health-promoting activities provided by general practitioners or their teams in general practices in Germany. MEDLINE and Embase databases were systematically searched in November 2020. Papers were selected in dual-review mode and extracted in single-review mode. Data analysis was finished by May 2021. In total, 530 papers were included in the synthesis. Little research has been carried out into collaboration opportunities both within the general practice team and in communities as a whole, with specialists (18%), hospitals (9%), and health insurance companies (6%) being the most frequent cooperation partners of GPs. 15%-20% of papers each dealt with 'early detection', 'information provision' and 'cardiovascular prevention'. Secondary (53%) and tertiary prevention (43%) was more often the subject of research than primary (39%) and quaternary prevention (15%). Healthy subjects (26%) were less often studied than people with pre-existing conditions (42%) and risk factors (48%). Little information was available on preventive activities in terms of gender, young people, migration background, housing conditions or educational background. Personal counselling (15%) was the most frequently described approach to health promotion in general practices, along with printed information materials (10%). This scoping review provides information on which to base targeted interventions and future research that can contribute towards transforming general practices into promoters of health within the community.Many data on echiurid anatomy and ultrastructure are obtained for Bonellia viridis and extrapolated to other species. The ultrastructure of the axial blood vessels, which has been described as an "osmotic pump," is regarded as one of the unusual features of echiurids. In this study, the ultrastructure of the proboscis blood vessels in females of B. viridis is described, illustrated by accurate schemes, and a new reconstruction of the axial blood vessel is suggested. The walls of the axial and lateral vessels of the proboscis are formed by myoepithelial cells, which are connected to each other via adherence junctions, underlined by basal lamina, and therefore form a true epithelium. Apical, middle, and basal parts of the myoepithelial cells form long, thin projections, which extend to the connective tissue (in axial vessel) or coelomic canals (in lateral vessels) and to the lumen of the vessels. The presence of such projections may evidence active cellular transport. Similarity in the fine structure of the myoepithelial cells of axial and lateral blood vessels evidence their common origin from myoepithelial cells of the coelomic lining.
