The 2017 WHO classification includes a new provisional entity of indolent T-lymphoproliferative disorders of the gastrointestinal tract (ITLPD-GIT). We investigated GI involvement of peripheral T-cell lymphoma (PTCL). Eighty-two patients were diagnosed with PTCL during 2007-2017. Eleven patients (13 %) had histologically-confirmed GI tract involvement 3 monomorphic epitheliotropic intestinal lymphoma (MEITL), 3 extranodal NK-/T-cell lymphoma nasal type (ENKL), 2 PTCL, not otherwise specified, 1 adult T-cell leukemia-lymphoma, 2 ITLPD-GIT. Three patients each had lesions in the small intestine and multiple lesions, two each in the stomach and colon, and one in the duodenum. Six of the 11 patients remained alive. No perforation/stenosis was observed after chemo-radiotherapy, although one patient with ENKL developed gastric bleeding during chemotherapy. One patient with ITLPD-GIT (CD4-/CD8+/Ki67Low) with a colonic lesion showing diffuse edema and multiple aphtha by endoscope and diarrhea, initially diagnosed with MEITL, had active but stable disease after various chemotherapies for 1 year and no therapy for the next 5 years. Another patient with ITLPD-GIT (CD4+/CD8+/Ki67Low) with a localized gastric lesion and slight epigastralgia was in remission for 1 year after radiation. In conclusion, about 10 % of PTCLs were complicated by GI tract lesions and most had a poor prognosis. ITLPD-GIT should be considered as a differential diagnosis based on histology and clinical course. Local complications after chemo/radiotherapy in PTCL with GI involvement were not frequent. Intrinsically disordered regions are often involved in allosteric regulation of multidomain proteins. They can act as disordered linkers to connect and interact with domains, resulting in rather complex allosteric mechanism and novel protein behavior. Therefore, it is necessary to analyze the diverse functions of disordered linkers in order to better understand allostery and relevant regulation process. Here we summarize recent advances in understanding the function of linkers and the advantages of adopting mutlidomain architecture with disorder linkers. Concurrently, medical training continually adapts to trends in medical practice so that future physicians are prepared to enter practice. In this paper, we examine the rise of clinical skills training courses and assessments in medical school, highlighting the changing role of emotion in training about communication in the doctor - patient relationship. Drawing on an interpretive analysis of interviews with and ethnographic observations of medical students and residents from two United States medical schools, we elaborate the concept of clinical empathy to describe the character of emotional engagement in the contemporary clinical encounter. In the analysis we show how standards of emotional conduct are taught in medical school, how clinical empathy is operationalized in the patient encounter, and how clinical empathy may be used instrumentally to smooth the physician's work. Finally, we position the consistent performance of clinical empathy as a form of emotional labor, expanding the reach of studies of emotional labor in professions. INTRODUCTION Previous studies have shown that formal social participation may reduce the risk of developing chronic conditions. Yet, the underlying mechanisms are largely unknown. In this study, we assessed the potential mediating roles of quality of life and depressive symptoms using longitudinal data. METHOD We analyzed nationally representative data from three consecutive waves (2011, 2013, 2015) of the SHARE survey, including 28,982 adults from 12 European countries aged 50 years and above at baseline. Measures were self-reported and included formal social participation (i.e. active participation within volunteer organizations, educational institutions, clubs, religious organizations, or political/civic groups), quality of life (CASP-12), depressive symptoms (EURO-D), and chronic conditions. Structural equation modeling was used to construct a focused longitudinal path model. RESULTS Formal social participation at baseline was inversely associated with the number of chronic conditions at 4-year follow-up. We identified two significant longitudinal mediation patterns 1) formal social participation predicted higher levels of quality of life, which in turn, predicted lower levels of chronic conditions; and 2) formal social participation predicted lower levels of depressive symptoms, which, in turn, also predicted lower levels of chronic conditions. CONCLUSIONS Formal social participation functions as a protective factor against the onset or development of chronic conditions. Therefore, studies of insects associated with hanging cadavers are important for developing entomological tools capable of estimating the postmortem intervals in such cases. We conducted a two-year experiment, in the spring, summer, and autumn of 2012 and 2013, studying the entomofauna of 12 hanging and 12 ground, decomposing pig carcasses in a hornbeam-oak forest, in western Poland. Both carcass treatments showed colonization by similar insect species. Among the 224 taxa collected, 61 and 52 were minimally abundant (≥10 individuals) during the adult stage on at least one hanging and at least one ground carcass, respectively. During the spring and autumn, more adult flies were attracted to hanging carcasses, whereas during the summer, their numbers were similar on both hanging and ground carcasses. Adult beetles were higher in number on hanging carcasses during the spring, but during the summer and autumn, they were slightly more abundany patterns were observed among the adult insects and beetle larvae that colonized hanging carcasses. Fly larvae that colonized ground carcasses had more breaks within the presence periods than fly larvae colonizing hanging carcasses. Neither treatment significantly affected the time of insect appearance on carrion. Most species appeared at the same time or at within one- or two-day intervals on both treatments. Seasonal differences in insect appearance were found, with most species colonizing quickly during the summer. The implications of these findings for forensic entomology are discussed. As the accessibility and utility of virtual databases of skeletal collections continues to grow, the impact that scan processing procedures has on the accuracy of data obtained from virtual databases remains relatively unknown. This study quantifies the intra- and inter-observer error generated from varying computed tomography (CT) scan processing protocols, including re-segmentation, incrementally varying thresholding value, and data collectors' selection of the threshold value on a set of virtual subadult pelves. Four observers segmented the subadult ossa coxarum from postmortem CT scans of the fully-fleshed bodies of eleven individuals of varying ages. Segmentation protocol was set, with the exception of each observer selecting their own thresholding value for each scan. The resulting smoothed pelvic surfaces were then compared using deviation analyses. Root mean square error (RMSE), average distance deviation, and maximum deviation distances demonstrated that thresholding values of ∼50 HU (Hounsfield units) are easily tolerated, the surfaces generated are robust to error, and threshold value selection does not systematically vary with user experience. The importance of consistent methodology during segmentation protocol is highlighted here, especially with regards to consistency in both selected thresholding value as well as smoothing protocol, as these variables can affect subsequent measurements of the resultant surfaces. V.BACKGROUND Tobacco, alcohol, and cannabis use are prevalent behaviors that pose considerable health risks. Genetic vulnerability and characteristics of the neighborhood of residence form important risk factors for substance use. link2 Possibly, these factors do not act in isolation. This study tested the interaction between neighborhood characteristics and genetic risk (gene-environment interaction, GxE) and the association between these classes of risk factors (gene-environment correlation, rGE) in substance use. METHODS Two polygenic scores (PGS) each (based on different discovery datasets) were created for smoking initiation, cigarettes per day, and glasses of alcohol per week based on summary statistics of different genome-wide association studies (GWAS). link3 For cannabis initiation one PGS was created. These PGS were used to predict their respective phenotype in a large population-based sample from the Netherlands Twin Register (N = 6,567). Neighborhood characteristics as retrieved from governmental registration systems were factor analyzed and resulting measures of socioeconomic status (SES) and metropolitanism were used as predictors.
