To compare associations between individual antidepressants and newborn outcomes.
Retrospective cohort study.
Deliveries in a large, US medical system.
Women who received at least one antidepressant prescription 3 months prior to conception through delivery.
Eligible women had maternal characteristics and newborn outcomes extracted from medical record data. Exposure was defined by the timing of the prescription during pregnancy.
Newborn outcomes (any adaptation syndrome, neonatal intensive care unit (NICU) admission) were analyzed for each antidepressant and compared using standard statistics and multivariable regression compared to exposure to bupropion. Odds of outcomes based on timing of exposure were also explored.
A total of 3,694 women were analyzed. Rates of any adaptation syndrome (p<0.001), NICU admission (p<0.001), and transient tachypnea of newborn (TTN) (p=0.006) were significantly different between drugs. Infants exposed to duloxetine had the highest rates of NICU admissions (3cuss choice of individual antidepressant drugs during pregnancy.
Duloxetine and escitalopram appear to have the strongest associations with any adaptation syndrome and NICU admission whereas bupropion and sertraline tended to have among the lowest risks of these outcomes. These results can help providers and patients discuss choice of individual antidepressant drugs during pregnancy.Dispersal-the movement of an individual from the site of birth to a different site for reproduction-is an ecological and evolutionary driver of species ranges that shapes patterns of colonization, connectivity, gene flow, and adaptation. In plants, the traits that influence dispersal often vary within and among species, are heritable, and evolve in response to the fitness consequences of moving through heterogeneous landscapes. Spatial and temporal variation in the quality and quantity of habitat are important sources of selection on dispersal strategies across species ranges. While recent reviews have evaluated the interactions between spatial variation in habitat and dispersal dynamics, the extent to which geographic variation in temporal variability can also shape range-wide patterns in dispersal traits has not been synthesized. In this paper, we summarize key predictions from metapopulation models that evaluate how dispersal evolves in response to spatial and temporal habitat variability. Next, we compile empirical data that quantify temporal variability in plant demography and patterns of dispersal trait variation across species ranges to evaluate the hypothesis that higher temporal variability favors increased dispersal at plant range limits. We found some suggestive evidence supporting this hypothesis while more generally identifying a major gap in empirical work evaluating plant metapopulation dynamics across species ranges and geographic variation in dispersal traits. To address this gap, we propose several future research directions that would advance our understanding of the interplay between spatiotemporal variability and dispersal trait variation in shaping the dynamics of current and future species ranges.When listening to a speaker, we need to adapt to her individual speaking characteristics, such as error proneness, accent, etc. The present study investigated two aspects of adaptation to speaker identity during processing spoken sentences in multi-speaker situations the effect of speaker sequence across sentences and the effect of learning speaker-specific error probability. Spoken sentences were presented, cued, and accompanied by one of three portraits that were labeled as the speakers' faces. In Block 1 speaker-specific probabilities of syntax errors were 10%, 50%, or 90%; in Block 2 they were uniformly 50%. In both blocks, speech errors elicited P600 effects in the scalp recorded ERP. We found a speaker sequence effect only in Block 1 the P600 to target words was larger after speaker switches than after speaker repetitions, independent of sentence correctness. In Block 1, listeners showed higher accuracy in judging sentence correctness spoken by speakers with lower error proportions. No speaker-specific differences in target word P600 and accuracy were found in Block 2. When speakers differ in error proneness, listeners seem to flexibly adapt their speech processing for the upcoming sentence through attention reorientation and resource reallocation if the speaker is about to change, and through proactive maintenance of neural resources if the speaker remains the same.Psychotic disorders such as schizophrenia are closely related to the development of obesity and metabolic perturbations. To develop and evaluate a Zambian context-specific mentorship model that supports registered nurses completing emergency, trauma and critical care programmes in Zambia.
In Zambia, emergency and trauma and critical care nursing are relatively new specialties, with education and training programmes less than a decade old. A train the trainer mentorship programme was developed and delivered at two colleges of nursing. Ethics approval was gained in both Zambia and the UK.
Documentary data analysis and focus groups were used. Focus groups included stakeholders and nurses in practice who had completed the train the trainer programme and were using the mentorship model.
The critical review of the literature revealed there was a paucity of evidence on the role of mentors in critical care. However, national documentation identified that most post basic education programmes are at Diploma Level with limited content that focuses on bedside teaching, mentorship and assessment content.
Feedback from representatives attending the stakeholder workshops and focus groups which included participants who had completed the training programme enabled the mentorship model and workshop to be developed and evaluated.
Nurses are the backbone of healthcare systems in Africa and the world. Mentorship and assessment in practice enables nurses to develop the competence and skills to lead practice, support peers and junior colleagues.

Variable selection in linear models has been fully developed, while it has received relatively little attention for longitudinal data. Since a longitudinal study involves within-subject correlations, the likelihood function of discrete longitudinal responses generally cannot be expressed in analytically closed form, and standard variable selection methods cannot be directly applied. As an alternative, the penalized generalized estimating equation (PGEE) is helpful but very likely results in incorrect variable selection if the working correlation matrix is misspecified. In many circumstances, the within-subject correlations are of interest and need to be modeled together with the mean. For longitudinal binary data, it becomes more challenging because the within-subject correlation coefficients have the so-called Fréchet-Hoeffding upper bound. In this paper, we proposed smoothly clipped absolute deviation (SCAD)-based and least absolute shrinkage and selection operator (LASSO)-based penalized joint generalized estimating equation (PJGEE) methods to simultaneously model the mean and correlations for longitudinal binary data, together with variable selection in the mean model. The estimated correlation coefficients satisfy the upper bound constraints. Simulation studies under different scenarios are made to assess the performance of the proposed method. Compared to existing PGEE methods that specify a working correlation matrix for longitudinal binary data, the proposed PJGEE method works much better in terms of variable selection consistency and parameter estimation accuracy. A real data set on Clinical Global Impression is analyzed for illustration.Metabolic intermediates influence inflammation not only through signaling effects, but also by fueling the production of pro-inflammatory molecules. Microglial production of nitric oxide (NO) requires the consumption of NADPH. NADPH consumed in this process is regenerated from NADP+ primarily through the hexose monophosphate shunt, which can utilize only glucose as a substrate. These factors predict that glucose availability can be rate-limiting for glial NO production. To test this prediction, cultured astrocytes and microglia were incubated with lipopolysaccharide and interferon-γ to promote expression of inducible nitric oxide synthase, and the rate of NO production was assessed at defined glucose concentrations. Increased NO production was detected only in cultures containing microglia. The NO production was markedly slowed at glucose concentrations below 0.5 mM, and comparably reduced by inhibition of the hexose monophosphate shunt with 6-aminonicotinamide. Reduced NO production caused by glucose deprivation was partly reversed by malate, which fuels NADPH production by malate dehydrogenase, and by NADPH itself.
