Serum fentanyl concentration and the safety and efficacy of once-a-day fentanyl citrate patch were investigated in pediatric and adolescent patients with cancer pain.

In this open-label, uncontrolled study, cancer patients aged 2-19 years being treated with strong opioid analgesics were switched to fentanyl citrate patch for 2 weeks. Serum fentanyl concentration was measured at steady state, and severity of pain was evaluated.

Eleven patients (four patients aged 2-5 years and seven patients aged 6-19 years) were enrolled. No patient received a dose exceeding 2 mg. Mean serum fentanyl concentrations after administration of 0.5 mg, 1 mg, and 2 mg were 144 pg/mL (n=4), 277pg/mL (n=3), and 2070pg/mL (n=4), respectively. All patients were included in the efficacy and safety analysis, but one patient was excluded from the pharmacokinetic analysis because blood was sampled on the day after blood transfusion. A subgroup analysis showed that the mean serum fentanyl concentration tended to be higher in pre-school patients (aged 2-5years) than in school-aged and adolescent patients (aged 6-19years) and than in reports of adult patients (aged 20years and above) who received the same dose. No respiratory adverse events were observed, and pain was well controlled.

Fentanyl citrate patch tended to result in a higher serum fentanyl concentration in pre-school patients than in school-aged, adolescent, and adult patients who received the same dose. The patch provided adequate pain control, was well tolerated, and did not cause respiratory adverse events.

Since May 2018, a 6-year post‑marketing surveillance (PMS) has been underway to evaluate the safety and effectiveness of letermovir for cytomegalovirus (CMV) prophylaxis in Japanese patients with allogenic hematopoietic stem-cell transplantation (allo-HSCT). The interim PMS data for 461 patients collected as of March 2021are reported in this publication.

The case report forms (CRFs) were drafted in part by the Japanese Data Center for Hematopoietic Cell Transplantation (JDCHCT) using data elements in the Transplant Registry Unified Management Program (TRUMP) and sent to individual HSCT centers to decrease burden of reporting. These CRFs were completed by physicians in the respective HSCT centers and sent to MSD K.K., Tokyo, Japan.

MiR-106a-5p and PTPN12 mRNA expressions were determined by quantitative real-time polymerase chain reaction (qRT-PCR). siRNA was used to knockdown PTPN12 expression in HCC cells, and the multiplication, migration, and invasion of HCC cells were determined by cell counting kit 8 (CCK-8) and Transwell assays. The interaction between PTPN12 and miR-106a-5p was verified by dual-luciferase reporter gene assay and RNA immunoprecipitation (RIP) assay. In the present study, we demonstrated that PTPN12 expression in HCC tissues and cells was significantly decreased, which was associated with the tumor size, TNM stage, and lymph node metastasis of HCC patients. Functionally, knocking down PTPN12 significantly promoted the multiplication, migration, invasion, and epithelial-mesenchymal transition (EMT) of HCC cells. PTPN12 was identified as the direct target of miR-106a-5p, and its expression was negatively modulated by miR-106a-5p. Besides, PTPN12 counteracted the promoting effects of miR-106a-5p on the viability, migration, invasion, and EMT of HCC cells. In conclusion, this study substantiates that PTPN12 inhibits the growth, migration, invasion, and EMT of HCC cells, and miR-106a-5p contributes to its dysregulation in HCC.
The prevalence of hypertension (HTN) is increasing in the middle-aged population, but few studies have examined the prevalence and risk factors associated with hypertension in this age group.

Toestimate the prevalence of HTN and its associated demographic risk factors in the middle-aged population (30-59years) of Iran.

In this cross-sectional study, 4112 middle-aged people from five provinces of the country were selected by multi-stage sampling and entered into the study. HTN or high blood pressure (HBP) was defined as systolic blood pressure ≥140mmHg and diastolic blood pressure ≥90mmHg. The relationship between demographic risk factors and the prevalence of HTN was assessed by multivariate logistic regression in Stata software version 14.

The prevalence of HTN was 17.8% (95% CI 16.5-19.0) and was higher in women than men (19.1% vs. 16.5%). The prevalence of HTN in three studied age groups including 30-39, 40-49 and 50-59years were 7.2, 18.7 and 38.0%, respectively. The frequency of HTN in urban residents was higher than rural residents (18.4% vs. 15.6%). The chance of developing hypertension in urban residents (OR 1.25), overweight (OR 3.94) and obese persons (OR 7.54) was higher.

Hypertension has a relatively high prevalence in the middle-aged Iranian population. According to our results, age, living in urban regions, being illiterate, overweight and obesity are independent risk factors for HBP. It seems that the administration of prevention programs in general population especially in younger ages based on existing risk factors is a necessity.

Many mountain SES are experiencing rapid environmental and socio-economic change, demanding viable action for conservation to sustain ecosystem services for the benefit of their communities. This paper is a synthesis of 71 case studies of mountain-specific SES, submitted to the International Partnership for the Satoyama Initiative (IPSI) that identifies major drivers of change, associated impacts, and response strategies. We find that overexploitation, land use change, demographic change, and the regional economy are the most prevalent drivers of change in the IPSI mountain SES, leading to negative consequences for biodiversity, livelihoods, indigenous knowledge, and culture. To counter these challenges in the study SES, stakeholders from the public, private, and civil society sectors have been implementing diverse legal, behavioral, cognitive, technological, and economic response strategies, often with strong community participation. We outline the lessons learned from the IPSI case studies to show how community-based approaches can contribute meaningfully to the sustainable management of mountain landscapes.Understanding the nexus between practices and ecosystems is a critical challenge for sustainability, but it is unclear how ecological sciences have explored the question. To bring clarification, we conducted an analysis of ecology literature dealing with agricultural and forestry practices (AF practices), scanning a total of 27 556 references. Scientometric analyses showed that AF practices were addressed by 5.5% of ecology literature, and that this proportion increased from 2.5 to 8.1% between 1956 and 2017. Content analyses showed that research has mainly focused on monospecific systems in the Global North, predominantly using plot-level experimental approaches. Temporal monitoring, real-world practices and their social context were poorly investigated. This analysis stresses the need to reinforce research in complex agroecosystems, in particular in non-Western countries. Multilevel and spatio-temporal approaches, as well as participatory research, should also be encouraged to build a social-ecological understanding and formulate more grounded, relevant policy recommendations for sustainability.At the mid-twentieth century the pace of the transformation of the Swedish forest increased. Gender differences in risk factors and treatment outcomes for type 2 diabetes mellitus (T2DM) may exist. We used the REALI European database to investigate whether there were gender-specific differences in baseline characteristics and clinical outcomes among patients with inadequately controlled T2DM initiated on insulin glargine 300 U/ml (Gla-300).

Data were pooled from 14 multicentre, prospective, interventional and non-interventional studies. link3 Impact of gender on glycaemic control, insulin dose, body weight and hypoglycaemia was evaluated after 12 and 24weeks of Gla-300 treatment.

Women (N = 3857) were older than men (N = 4376) (median age, 65.0 versus 63.0years), with greater mean body mass index (32.5 versus 31.6kg/m
) and lower median estimated glomerular filtration rate (77.5 versus 84.0ml/min/1.73 m
). Peripheral arterial disease and a history of myocardial infarction were more frequent in men (20.1% versus 11.7% and 12.0% versus 5.8%, respectively). At baseline, mean haemoglobin A1c (HbA1c) wa in baseline characteristics, Gla-300 treatment improved glycaemic control, with overall low hypoglycaemia incidences in both men and women. However, women had statistically significantly lower HbA1c reductions than men, although these differences were clinically modest.
