These concepts form the rationale for a recent multicentric randomized trial, Rate Control vs. Electrical Cardioversion Trial 7 - Acute Cardioversion vs. Wait and See (RACE 7 ACWAS), comparing the efficacy of delayed cardioversion, within 48 h from symptoms onset, in case of lack of spontaneous conversion, with early cardioversion in symptomatic patients with recent-onset AF. In patients presenting to the emergency department with recent-onset, symptomatic AF, a wait-and-see approach was non-inferior to early cardioversion in maintaining the sinus rhythm at 4 weeks. Nonetheless a system employing a delayed cardioversion strategy increases the costs of treatment, complicates the treatment pathway, and could represent a psychological burden for the patients. Accordingly, delayed cardioversion could not represent a practical choice for many hospitals with limited resources and without an adequate outpatient organization.Spontaneous coronary artery dissection (SCAD) has been recognized as an emergent cause of acute coronary syndrome (ACS), myocardial infarction, and sudden death. Patients mostly affected by SCAD are individuals without or with few cardiovascular risk factors, particularly young women, thus suggesting a clearly different pathophysiology than the more common atherosclerosis. Present research efforts outlined an improved characterization of the prevalence, natural history, and clinical outcome of SCAD. Intracoronary imaging has been an important asset in this condition, providing an improved diagnostic and therapeutic understanding. Current evidences suggest not only that this condition is more common than previously thought but also that the clinical management could be distinctly different from ACS secondary to atherosclerosis. Both medical and interventional treatment should consider the different cause of ACS, as well as the clinical stability of the patient, taking into account that the risk of recurrence is particularly high, predominantly during the first few days after the acute event. Stemming from new scientific evidences in terms of pathophysiology, clinical approach, therapy strategies, and follow-up of SCAD, it is important to identify spontaneous coronary dissection in the differential diagnosis of ACS.Cardiac oncology is a subspecialty of cardiology engaging cardiologists and oncologists alike, in order to provide the best possible oncologic treatment for patients at high cardiovascular risk or developing cardio-toxicity during the course of their treatment, thus avoiding discontinuing it, and aiming at improving survival and quality of life. Early diagnosis and the effectiveness of the newer cancer treatments delivered an increasing number of long-term survivors (presently almost 30 million worldwide), at high risk of developing cardiovascular diseases. This predisposition has been correlated not only to the toxic side effects of the oncologic treatment but also to a real vulnerability to the risk factors in this patients population. More recently, the SYNTAX III Revolution trial showed, in patients with three-vessel CAD with or without left main involvement, that treatment decision-making between percutaneous coronary intervention and coronary artery bypass grafting based on CCTA only has an almost perfect agreement with the treatment decision derived from invasive coronary angiography (ICA). The high degree of correlation between CCTA and ICA suggests the potential feasibility of treatment decision-making based solely on non-invasive imaging and clinical information. New research prospects have opened up for the future to demonstrate the true feasibility and safety of this innovative approach in the clinical arena.
