Virtual patients provide numerous learning opportunities for medical students, yet only support one-off patient interactions. To emulate general practice, allowing multiple encounters with a single patient, a new longitudinal virtual patient (LVP) was developed.

The LVP was integrated into 2nd year at a graduate-entry medical school, in the 2019/2020 academic year. Students were asked about their prior experience and expectations of LVPs, before they engaged with two consultations and one results session. Data were collected from this survey and from the engagement with the LVP. Feedback was collected and thematically grouped.

Survey responses indicated that 1.7% (2/120) had previous experience with virtual patients, and the majority expected the LVP to make a difference to their clinical reasoning. Out of the 142 students engaged with the LVP, 53% completed over 75% of the work. Informal feedback arose around accessibility, professional learning and development, and engagement with the LVP module.

The data indicate that LVPs are agreeable to medical students, with good engagement and positive reports of clinical learning.

Future work exploring reasons of engagement or lack of, will support refinement of the LVP to accommodate the learning needs of the medical students.

In the case presented here, a neonate was born at 29 weeks gestation with severe respiratory depression, pyrexia; hypotension and an elevated white blood cell count. Her mother was a 19-year-old pregnant woman who developed premature rupture of the membranes and went into labour early. Sepsis was suspected and so the neonate received dobutamine and empiric ampicillin/gentamicin. The mother reported visiting a farm during her pregnancy and so congenital brucellosis was considered a possibility. Blood cultures were positive for Gram-negative coccobacilli and serology confirmed the presence of Brucella abortus and B. meltiness. Antibiotic treatment was changed to rifampin/gentamicin/ciprofloxacin but on day 17 the baby deteriorated and gentamicin was discontinued and meropenem was added. The neonate gradually improved; meropenem was discontinued on day 24 and the baby was discharged from hospital on day 38.Objectives To prevent the occurrence of injury in a sport, exact knowledge of injury patterns is needed. To synthesize sport-specific injuries in track and field comparing elite and recreational level athletes, as well as gender. Furthermore, analyze the time loss due to injury and reduction in athletic performance. Methods Injury type-specific frequencies were recorded according to discipline, gender and performance level. Injury severity was assessed by time loss duration and performance reduction. Results 64% of athletes suffered at least one injury. In this article, we present a fast algorithm to compute the fractional Fourier power spectra of real sequences. For a real sequence of length of m = n l , we may deduce its congruence derivative sequence with a length of l. The discrete Fourier transform of the original sequence can be calculated by the discrete Fourier transform of the congruence derivative sequence. The relation of discrete Fourier transforms between the two sequences may derive the special features of Fourier power spectra of the integer and fractional periods for a real sequence. It has been proved mathematically that after calculating the Fourier power spectrum (FPS) at an integer period, the Fourier power spectra of the fractional periods related this integer period can be easily represented by the computational result of the FPS at the integer period for the sequence. Computational experiments using a simulated sinusoidal data and protein sequence show that the computed results are a kind of Fourier power spectra corresponding to new frequencies that cannot be obtained from the traditional discrete Fourier transform. Therefore, the algorithm would be a new realization method for discrete Fourier transform of the real sequence.
