Appropriate decisions by medical technicians at a trauma scene may influence a patient's prognosis. Emergency life-saving technicians (ELSTs) are certified specialists trained with the knowledge to provide advanced techniques for prehospital emergency care in Japan. However, the benefit of treatment by ELSTs compared to basic emergency medical technicians (BEMTs) remains unclear. The aim of this study is to determine whether treatment by ELSTs improves outcomes for trauma patients.

We retrospectively reviewed the Japan Trauma Data Bank for the years 2004 to 2017. Patients transferred to the hospital directly from the trauma scene and at least 16 years old were included in this study. The following criteria were used to exclude patients; presence of burns, untreatable severe traumas, unknown ELST attendance, and missing prognosis. We compared two groups (ELST group patients transported by emergency medical services (EMS) with the presence of at least one ELST; BEMT group patients transported only by BEMTs)CI 1.14-1.97), and shorter transportation time (adjusted OR 1.21, 95% CI 1.03-1.41).

Pheochromocytomas are rare tumors of the adrenal gland. Intestinal pseudo-obstruction is a very rare presentation of a functioning catecholamine-secreting tumor. We present a case of intestinal pseudo-obstruction due to a large functioning pheochromocytoma.

A 29-year-old female presented with abdominal distension, pain, nausea, and vomiting with constipation for 3weeks. She was hypertensive and diabetic and was on multiple medications. She reported frequent spells of severe headaches, palpitations, night sweats, and a 17kg weight loss over 6months. She had pallor, dyspnea, marked abdominal distension, and diminished bowel sounds. Her blood pressure was high at 200/120mmHg. She had tachycardia (pulse 120bpm) and tachypnea (35pm). Serum metanephrine levels were significantly elevated, measuring 1203pg/ml. Abdominal CT showed a heterogeneous, hyper-vascular mass near the upper pole of the left kidney, measuring 10.75cm×8.72cm. Open left adrenalectomy was performed through an anterior subcostal approach to remove the tumor with the left adrenal gland. Histopathological examinations were consistent with pheochromocytoma.

Autochthonous yeast isolates, Saccharomycodes ludwigii and Hanseniaspora uvarum, were selected due to their generated products of alcohol and 2-phenylethyl acetate, respectively. The fermentation kinetic parameters of selected autochthonous yeasts as single and co-cultures in chaptalized pineapple juice were investigated comparing to commercial Saccharomyces cerevisiae. The ethanol production rate of S'codes ludwigii (0.104%(v/v)/h) during the initial stage of fermentation was relatively slower compared to those of S. cerevisiae (0.129%(v/v)/h) but increased during middle through final stages with similar ethanol content to the commercial S. cerevisiae (~12%(v/v)). In pineapple juice, fructose was firstly assimilated, S'codes ludwigii (K = 0.405) and S. cerevisiae (K = 0.552), while glucose was secondly used, S'codes ludwigii (K = 0.281) and S. cerevisiae (K = 0.217) for first-order kinetic model. In co-cultures, the two isolated strains displayed synergistic behaviours during fermentation. S'codes ludwigii supported the growth of H. uvarum so that it generated more desirable volatile organic compounds (VOCs) at an early stage. Interestingly, the VOCs could not be produced in co-cultures of H. uvarum with the commercial strains. Then, S'codes ludwigii further completed the alcoholic fermentation through final stage. The fermentation performances of co-cultured autochthonous yeasts demonstrated a new approach for successful pineapple winemaking over S. cerevisiae. In addition, growth kinetics and fermentation behaviour, as observed in this study, could be a key information in development of potential substrates and strains for future alcoholic fermentation.A novel antimicrobial chitosan-gelatin based edible coating fortified with papaya leaves and thyme extract was prepared for improving the quality and shelf-life of chicken breast fillet and Kareish cheese during chilled storage at 4 ± 1 °C. The samples were dipped for 10 min in distilled water (control), chitosan-gelatin (CG), chitosan-gelatin +2% papaya leaves extract (CG + P) and chitosan-gelatin +2% thyme extract (CG + Th). The coated and uncoated samples were examined periodically for sensory attributes, pH, TBARs, total aerobic mesophilic (TAM), total Enterobacteriaceae (TE), and total yeasts and molds counts (TYM). Sensory evaluation revealed that chicken breast fillet and Kareish cheese samples coated with CG + P were the best in terms of tenderness, juiciness, body & texture and flavor. CG + Th exhibited the highest antimicrobial and antioxidant effect, followed by CG + P. The results of microbiological, physicochemical and sensory analysis of this study demonstrated that the application of CG + P or CG + Th could be a promising method for increasing the shelf life and improving the quality of chicken breast fillet and Kareish cheese.Most vertebrates exhibit sexual dimorphisms in size, colour, behaviour, physiology and many others. The Chinese soft-shelled turtle (Pelodiscus sinensis) male individuals reach a larger size than females which produce significant economic implications in aquaculture. However, the mechanisms of sex determination and plastic patterns of sex differentiation in P. sinensis remain unclear. Here, comparative transcriptome analysis on male and female embryonic gonads prior to gonad formation and stages mediated gonadal differentiation of P. sinensis were performed to characterize the potential sex-related genes and their molecular pathways in P. sinensis. A total of 6369 differentially expressed genes (DEGs) were identified from day 9 and day 16 and assigned to 626 GO pathways and 161 KEGG signalling pathways, including ovarian steroidogenesis pathway, steroid hormone biosynthesis pathways, and the GnRH signalling pathway (P less then 0.05). Moreover, protein interaction network analyses revealed that Akr1c3, Sult2b1, Sts, Cyp3a, Cyp1b1, Sox30 and Lhx9 might be key candidate genes for sex differentiation in P. sinensis. These data provide a genomic rationale for the sex differentiation of P. sinensis and enrich the candidate gene pool for sex differentiation.Family caregivers to persons living with dementia increasingly provide complex care tasks, though most (53%) do so without any training. "Complex care" includes medical/nursing tasks, as well as personal care tasks that require disease knowledge. Of the 67% of dementia caregivers who provide complex care, nearly half worry about making a mistake. To help caregivers feel more confident when providing complex care tasks, we developed and conducted a pilot study of the 4-week Learning Skills Together psychoeducation program (LST; N=35).
