Mitotane (o, p'-DDD) is a molecule that was developed many years ago for adrenal cortical carcinoma, but no suitable pediatric dosage form is available for administration to young children. Mitotane requires therapeutic drug monitoring because of its long half-life and difficulty in stabilizing plasma concentrations. Furthermore, Mitotane is a highly lipophilic drug that requires concurrent lipid administration.

We present the case of a 3-year-old girl who was diagnosed with metastatic adrenal cortical carcinoma. Due to the difficulty in administering the tablets and the non-stabilized mitotane dosages, a nasogastric tube was inserted. An administration protocol based on dispersing the tablets in whole milk was established by the pharmacy team. This led to the stabilization of the disease for at least 1.5 years.

Mitotanemia is difficult to stabilize even when treatment is administered orally. To maintain biological efficacy, we propose an easily reproducible protocol. The efficacy was stabilized at a dosage of 1500 mg per day. Mitotanemia fluctuated between 14 mg/mL, and 20 mg/mL. The implementation of this protocol prevented treatment discontinuation.

The mortality rate as high as 27% from fulminant hepatic failure has been reported following ingestion of yellow phosphorous.1 Multisystem toxicity affecting gastrointestinal, cardiac, renal and central nervous systems occurs.2 There is a paucity of literature on its haematological toxicity. In this case report, we highlight the clinically significant myelosuppression observed in a patient resulting from yellow phosphorous; a small literature review has also been done on this rare complication.

= 6), from the North West of England recruited from a suicide bereavement support group's social media. Data were analysed using thematic analysis informed by the realist framework.

Effective social support includes the prioritisation of building meaningful connections with like-minded individuals, providing a safe space for authentic self-expression aiding personal relationship maintenance. Contextual factors included Societal and cultural stigma of suicide, self-stigma and gender norms. Mechanisms influencing support seeking include Not wanting to burden loved ones due to judgement, and a lack of understanding.

Our study investigated monocyte parameters in this regard.Thirty cases each of SNTB, sputum positive cases and healthy controls were included. Full blood count and immune-phenotyping for 'monocyte parameters' by flow cytometry were performed using CD45, CD16, CD14, CD64 and HLA-DR The percentage of monocyte subsets and MFI of CD64 and HLA- DR noted and data statistically compared.The monocyte to lymphocyte ratio, percentage of intermediate monocytes, expression of CD64 on total as well as intermediate monocytes were significantly elevated in cases with SNTB as compared to healthy controls (p value, 0.07, 0.07, 0.001, 0.001 respectively). The cut off value for MFI CD64 on total and intermediate monocyte was >18 and >19.6 respectively.Thus monocyte parameters can serve as useful biomarkers for diagnosing tuberculosis in the sputum negative group.Spiritual care interventions depend upon the context in which care is provided, its institutional setting and the actors involved. In order to understand the relationship between interventions in spiritual care and the context in which care is provided, we study the spiritual interventions of military chaplains against the background of the armed forces. In our study, we demonstrate that the military context needs its own conceptualization to understand the pastoral practices of military chaplains. This article uses a qualitative comparative methodology and is based upon 13 case studies that have been generated over the course of five years by a team of eight military chaplains within the framework of the Dutch Case Study Project. The analysis results in four concepts that describe the relationship between spiritual care practices and the context of the armed forces in more detail structuring pastoral availability, positioning within the military environment, existential negotiation of being human and being a soldier and transforming military time. We conclude that the study of spiritual care in context leads to a contextualized understanding of spiritual care practices and stimulates comparison of pastoral care practices across contexts.People who experience the greatest social inequities often have poor experiences in emergency departments (EDs) so that they are deterred from seeking care, leave without care complete, receive inadequate care, and/or return repeatedly for unresolved problems. However, efforts to measure and monitor experiences of care rarely capture the experiences of people facing the greatest inequities, experiences of discrimination, or relationships among these variables. This analysis examined how patients' experiences, including self-reported ratings of care, experiences of discrimination, and repeat visits vary with social and economic circumstances. Every consecutive person presenting to three diverse EDs was invited if/when they were able to consent; 2424 provided demographic and contact information; and 1692 (70%) completed the survey.
