The study has provided percentile values useful to monitor the physical fitness status of children and adolescents.The risk of a severe course of COVID-19 is increased in patients suffering with Neuromuscular disorders (NMD) due to the following comorbidities muscular weakness of the chest and diaphragm, use of ventilator supports and/or presence of tracheostoma, weak airway clearance, cardiac involvement, rhabdomyolysis, comoribities, steroid and immunosuppressant treatments. NMD display varying levels of disability in people with the same diagnosis, thus it is difficult to give COVID-19 related general recommendations. Present advicess were designed for patients, caregivers, general neurologists and non-specialist medical providers. They address frequently asked questions and basic service requirements and are supported by a series of in-depth references. In this truly unprecedented situation, the clinical management of neuromuscular patients during the COVID-19 epidemics - taking into account the related difficulties (patients who have suspended ERT, difficulty in contacting the doctors, etc.) - we propose to use a telemedicine device, i.e. the AIGkit application (AIGkit app), promoted and developed in 2018 by Fabrizio Seidita on behalf of the Italian Glycogenosis Association (AIG). The app was born to allow patients with Pompe disease to receive as far as possible continuous monitoring of their health. The support of all colleagues of the Italian Association for Myology (AIM) should extend its use to all NMD patients and beyond.There is a lack of commercially available low-cost technologies to assess gait clinically in non-controlled environments. As a consequence of this, there has been poor massification of motion measurement technologies that are both objective and reliable in nature. Advances about the study of gait and its interpretation in recent years using inertial sensors have allowed proposing acceptable alternatives for the development of portable and low-cost systems that contribute to people's health in places and institutions that cannot acquire or maintain the operation of commercially available systems. A system based on a custom single Inertial Measurement Unit and a mobile application is proposed. Thus, an investigation is carried out using methodologies and algorithms found in the literature in order to get the main gait events and the spatial-temporal gait parameters. Twenty healthy Chilean subjects were assessed using a motion capture system simultaneously with the proposed tool. The results show that it is possible to estimate temporal gait parameters with slight differences respect gold--standard. We reach maximum mean differences of -2.35±5.02[step/min] for cadence, 0.03±0.04[sec] for stride time,0.02±0.03[sec] for step time, ±0.02[sec] for a single support time, 0.01±0.02[sec] for double support time and 0.01±0.03[m] for step length. As a result of experimental findings, we propose a new technological tool that can perform gait analysis. Our proposed system is user-friendly, low-cost, and portable. Therefore, we suggest that it could be an attractive technological tool that healthcare professionals could harness to objectively measure gait in environments that are either within the community or controlled. We also suggest that the tool could be used in countries where advanced clinical tools cannot be acquired. Therefore, we propose in this paper that our system is an attractive, alternative system that can be used for gait analysis by health professionals worldwide.The present study focuses on an innovative approach in measuring the mechanical oscillations of pre-loaded Achilles tendon by using Mechanotendography (MTG) during application of a short yet powerful mechanical pressure impact. This was applied on the forefoot from the plantar side in direction of dorsiflexion, while the subject stood on the ball of the forefoot on one leg. Participants with Achilles tendinopathy (AT; n = 10) were compared to healthy controls (Con; n = 10). Five trials were performed on each side of the body. For evaluation, two intervals after the impulse began (0-100ms; 30-100ms) were cut from the MTG and pressure raw signals. The intrapersonal variability between the five trials in both intervals were evaluated using the arithmetic mean and coefficient of variation of the mean correlation (Spearman rank correlation) and the normalized averaged mean distances, respectively. The AT-group showed a significantly reduced variability in MTG compared to the Con-group (from p = 0.006 to p = 0.028 for different parameters). The 95% confidence intervals (CI) of MTG results were disjoint, whereas the 95% CIs of the pressure signals were similar (p = 0.192 to p = 0.601). We suggest from this work that the variability of mechanical tendon oscillations could be an indicative parameter of an altered Achilles tendon functionality.Elderly people perform more slowly movements of everyday life as rising from a chair, walking, and climbing stairs. This is in the first place due to the loss of muscle contractile force which is even more pronounced than the loss of muscle mass. In addition, a secondary, but not negligible, component is the rigidity or increased stiffness which requires greater effort to produce the same movement and limits the range of motion of the joints. Oxaliplatin order In this short review, we discuss the possible determinants of the limitations of joint mobility in healthy elderly, starting with the age-dependent alterations of the articular structure and focusing on the increased stiffness of the skeletal muscles. Thereafter, the possible mechanisms of the increased stiffness of the muscle-tendon complex are considered, among them changes in the muscle fibers, alterations of the connective tissue components, i.e., extracellular matrix (ECM), aponeurosis, tendon and fascia, and remodeling of the neural pattern of muscle activation that increases antagonist co-activation.Hamstring strain injuries remain among the most problematic and most frequent sport injuries. Two of the most effective methods for prevention and rehabilitation of the hamstring strain injuries are classic eccentric training using the Nordic hamstring exercise (NHE) and eccentric training in a lengthened position using the glider exercise. Both exercises have disadvantages that could be fixed by adding hip flexion during the NHE. Thus, the purpose of the study was to compare peak hamstring activity (measured by electromyography) between three eccentric exercises the standard NHE, the modified NHE and the glider. Differences were statistically tested with the analysis of the variance for repeated measurements and the paired 2-tailed post-hoc t-test. Hamstring activity during the modified NHE was significantly lower compared to the NHE and significantly higher compared to the glider. The results indicate that implementing the modified NHE could increase the effectiveness of already established rehabilitation protocols and help reduce the risk of hamstring (re-)injury.Cancer cachexia has been reported to be directly responsible for at least 20% of cancer deaths. Management of muscle wasting in cancer-associated cachexia appears to be of pivotal importance for survival of patients. In this regard, it would be interesting to identify before its patent appearance eventual functional markers of muscle damage, to plan specific exercise protocols to counteract cachexia. The muscle function of 13 oncologic patients and 15 controls was analyzed through i) analysis of the oxidative metabolism, indirectly evaluated trough dosage of blood lactate levels before and after a submaximal incremental exercise on a treadmill; ii) analysis of strength and, iii) endurance, in both lower and upper limbs muscles, employing an isokinetic dynamometer. Statistical analyses were carried out to compare the muscle activities between groups. Analysis of oxidative metabolism during the incremental exercise on a treadmill showed that patients performed a shorter exercise than controls. Lactate levels were significantly higher in patients both at baseline and after the task. Muscle strength analysis in patients group showed a reduction of Maximum Voluntary Contraction during the isometric contraction and, a tendency to fatigue during endurance task. Data emerging from this study highlight an impairment of muscle oxidative metabolism in subjects affected by a pre-cachexia stage of cancer. A trend of precocious fatigability and an impairment of muscle strength production were also observed. This evidence underlines the relevance of assessing muscle function in order to develop novel rehabilitative approaches able to counteract motor impairment and eventually to prevent cachexia in these patients.Duchenne Muscular Dystrophy is a devastating disease caused by the absence of a functional rod-shaped cytoplasmic protein called dystrophin. Several avenues are being developed aimed to restore dystrophin expression in boys affected by this X-linked disease. However, its complete cure is likely to need combinational approaches which may include regimes aimed at restoring muscle mass. Augmenting muscle growth through the manipulation of the Myostatin/Activin signalling axis has received much attention. However, we have recently shown that while manipulation of this axis in wild type mice using the sActRIIB ligand trap indeed results in muscle growth, it also had a detrimental impact on the testis. Here we examined the impact of administering a powerful Myostatin/Activin antagonist in two mouse models of Duchenne Muscular Dystrophy. We report that whilst the impact on muscle growth was not always positive, both models showed attenuated testis development. Sperm number, motility and ultrastructure were significantly affected by the sActRIIB treatment. Our report suggests that interventions based on Myostatin/Activin should investigate off-target effects on tissues as well as muscle.Sjögren's syndrome (SS) is a complex autoimmune disease that predominantly affects the exocrine glands, notably the salivary and lacrimal glands, resulting in dryness of the mucosa recognized as xerostomia. Chief oral complications reported by patients consist of high caries rate, burning sensation of the mucosa, early tooth loss, intensified tooth wear and repetitive failure of dental restorations. In particular, due to the decreased salivary flow, conventional removable prostheses might irritate the mucosa and lead to painful ulcerations at the borders of the denture. Implant-supported prostheses offer a unique solution to the difficulties experienced by edentulous patients with Sjögren's syndrome. This research showed no signs of peri-implantitis or peri-implant mucositis during 7-years following the placement of implants. The present study indicates that successful long-term maintenance of dental implants can be also achieved in SS patients.Fibrin sealant as a promising agent for providing scaffold and efficient hemostasis is widely accepted in several specialties. However, the outcome of Fibrin sealants in lumbar annuloplasty after endoscopic discectomy has not been evaluated in patients with disc herniation. The goal of this study was to evaluate the efficacy, response, and probability of future recurrence rates in herniated nucleus pulposus (HNP) with the use of fibrin sealant in conjunction with endoscopic disc surgery. A total of 35 patients (28 men, 7women) were evaluated, including 18 patients who underwent endoscopic discectomy alone and 17 patients that received fibrin sealant at the site of annulus tear and endoscopic discectomy. All patients were followed through both clinical and imaging methods for an average of 10.5 months. Primary outcome measure was defined as lumbar decompression approved by imaging and symptom alleviation after endoscopic spinal discectomy with Visual Analogue Scale (VAS) score ≤ 4 (cut-off point). Median size of annular tearing was significantly lower in the endoscopic discectomy group (median, 3) (minimum, 2; maximum, 5); however, the corresponding factor in the endoscopic discectomy plus fibrin sealant group was significantly larger (median, 6) (minimum, 5; maximum, 10), with P less then 0.
