High circulating levels of the hormone prolactin (PRL) protect against experimental diabetic retinopathy (DR) due to the retinal accumulation of vasoinhibin, a PRL fragment that inhibits blood vessel permeability and growth. A phase 2 clinical trial is investigating a new therapy for DR based on elevating serum PRL levels with levosulpiride, a prokinetic dopamine D2 receptor blocker. Here, we tested whether levosulpiride-induced hyperprolactinemia elevates PRL and vasoinhibin in the vitreous of volunteer patients with proliferative DR (PDR) undergoing elective pars plana vitrectomy.

Patients were randomized to receive placebo (lactose pill, orally TID;
= 19) or levosulpiride (25 mg orally TID;
= 18) for the 7 days before vitrectomy. Vitreous samples from untreated non-diabetic (
= 10) and PDR (
= 17) patients were also studied.

Levosulpiride elevated the systemic (101 ± 13 [SEM] vs. 9.2 ± 1.3 ng/mL,
< 0.0001) and vitreous (3.2 ± 0.4 vs. 1.5 ± 0.2 ng/mL,
< 0.0001) levels of PRL, and both levels were directly correlated (
= 0.58,
< 0.0002). The vitreous from non-diabetic patients or from PDR patients treated with levosulpiride, but not from placebo-treated PDR patients, inhibited the basic fibroblast growth factor (bFGF)- and vascular endothelial growth factor (VEGF)-induced proliferation of endothelial cells in culture. Vasoinhibin-neutralizing antibodies reduced the vitreous antiangiogenic effect. Matrix metalloproteases (MMPs) in the vitreous cleaved PRL to vasoinhibin, and their activity was higher in non-diabetic than in PDR patients.

Levosulpiride increases the levels of PRL in the vitreous of PDR patients and promotes its MMP-mediated conversion to vasoinhibin, which can inhibit angiogenesis in DR.

To compare electrophysiological and pupillometric responses in subjects with cone-rod dystrophy due to autosomal recessive (AR)
mutations.

Four subjects with AR
dystrophy and 10 visually normal, age-similar controls participated in this study. Full-field, light- and dark-adapted electroretinograms (ERGs) were obtained using conventional techniques. Full-field, light- and dark-adapted measures of the pupillary light reflex (PLR; pupil constriction elicited by a flash of light) were obtained across a range of stimulus luminance using long- and short-wavelength light. Pupil size as a function of stimulus luminance was described using Naka-Rushton functions to derive
(maximum response) and
(pupil response sensitivity).

subjects, whereas dark-adapted ERGs were non-detectable in three subjects and markedly attenuated in the fourth. By contrast, each

One of the diagnostic features of visual crowding, radial-tangential anisotropy, has been observed both in behavioral experiments as well as in responses of the blood-oxygenation-level-dependent (BOLD) functional magnetic resonance imaging (fMRI) signal. As has been shown previously, crowding is stronger for radially arranged flankers, and this tendency is reflected in BOLD signal suppression. In the current study, we examined the effect of practice on the neural correlates of crowding. We expected that training on a crowding task would cause shrinkage of the crowding zone that would be mirrored in corresponding BOLD signal responses.

Pre- and post-training fMRI images were acquired in 17 healthy volunteers using a 3-tesla MRI scanner. Participants were trained over 4 consecutive days on a crowding task.

Comparison of the pre- and post-training behavioral data indicates a significant shrinkage of the crowding zone as a result of training. Moreover, we observed a pronounced radial-tangential anisotropy in the BOLD signal prior to training; that is, radial flankers induced a larger reduction in the BOLD signal compared to equally spaced tangential flankers. After training, this radial-tangential anisotropy in the BOLD signal was significantly reduced. Specifically, we found significant changes in BOLD responses for the radial flanker configuration.

Our results demonstrate that training-induced changes in the anisotropic shape of the crowding zone are reflected in the BOLD signal in the early visual cortex.

To study the choroid in uncomplicated pregnant women using advanced optical coherence tomography (OCT) imaging and analysis.

Women with uncomplicated pregnancy in the third trimester (>28 weeks gestational age) and age-matched nonpregnant women were enrolled in this prospective study. All subjects underwent spectral-domain OCT with enhanced depth imaging mode and spectral-domain optical coherent tomography angiography (OCTA). Main outcomes were subfoveal choroidal thickness (SFCT), choroidal vascularity index (CVI), and choriocapillaris flow deficits (CC FD).

Twenty-two eyes of 12 uncomplicated pregnant women and 23 eyes of 15 nonpregnant, female controls were included. The mean age was 32.27 ± 6.96 years for the pregnant group and 30.08 ± 5.49 years for control group (
> 0.05). Mean SFCT was 238.70 ± 82.96 µm in the pregnant group, and 277.40 ± 61.79 µm in the control group. CVI was 67.58% ± 2.45% in the pregnant group and 67.31% ± 2.62% in the control group. The CC FD% was 54.06 ± 3.95 in the pregnant group, and 55.02 ± 3.78 in the control group. There was no significant difference between the pregnant and control groups (
> 0.05) in SFCT, CVI or CC FD.

Although extensive hemodynamic changes occurred with pregnancy, choroidal measurements by OCT and OCTA demonstrated no differences in uncomplicated third-trimester pregnant women compared with nonpregnant controls.
