The present study examined which types of alcohol protective behavioral strategies ([PBSs], Manner of Drinking [MOD] strategies, Serious Harm Reduction [SHR] or Limiting/Stopping Drinking strategies [LSD]) mediate the effects of a comprehensive number of distal antecedents on alcohol drinking quantity and alcohol-related negative consequences.

A sample of 762 last-month college freshman drinkers from Argentina (Mean age=19.60±3.80) completed a survey that assessed alcohol outcomes (drinking quantity during a typical week and alcohol-related negative consequences), frequency of use of PBS and a set of distal antecedents (i.e. age at drinking onset, drinking motives, college alcohol beliefs and impulsivity-like traits).

MOD significantly mediated the effects of college alcohol beliefs (higher beliefs were associated with lower MOD use) and coping motives (higher coping motives were associated with lower MOD use) on alcohol outcomes. SHR significantly mediated the effects of sex (women reported more frequent SHR use) and enhancement motives (higher motivation was associated with lower SHR use) on alcohol-related negative consequences.

Our findings partially support a mediational role of PBS in the association between risk factors and alcohol outcomes, and offer valuable information for the design of interventions to reduce alcohol use in South-American college students. More studies examining PBS as mediators of distal antecedents are needed to fully understand the peculiarities of these associations in different cultures.

The prevention of pulmonary insufficiency (PI) is a crucial part of the tetralogy of Fallot repair. Many techniques have been introduced to construct valves from different materials for the right ventricular outflow tract, including the most commonly constructed monocusp valves. We are introducing a new bicuspid valve made intraoperatively using the autologous right atrial appendage (RAA) to prevent PI in these patients.

The RAA valve was constructed and used in 21 patients with tetralogy of Fallot. The effective preservation of the native valve was impossible in all patients because of either a severe valve deformity or a small annulus. The RAA valve was created after ventricular septal defect closure and right ventricular outflow tract myectomy and was covered with a bovine transannular pericardial patch. The perioperative data were evaluated, and the echocardiography results were assessed immediately after operations and in follow-up with a median of 10.5 months. Number of recently published studies on nutritional support in the intensive care unit (ICU) have resulted in a paradigm shift of clinical practices. This review summarises the latest evidence in four main topics in the ICU, namely (1) function of validated nutrition screening/assessment tools, (2) types and validity of body composition measurements, (3) optimal energy and protein goals, and (4) delivery methods.

Recent studies that investigated the above aims were outlined and discussed. In addition, recent guidelines were also compared to highlight the similarities and differences in their approach to the nutrition support of critically ill patients.

Regardless of nutritional status and body composition, all patients with >48 hours of ICU stay are at nutrition risk and should receive individualised nutrition support. Although a recent trial did not demonstrate an advantage of indirect calorimetry over predictive equations, it was recommended that indirect calorimetry be used to set energy targets with better accuracy. Initiation of enteral nutrition (EN) within 24-48 hours was shown to be associated with improved clinical outcomes. The energy and protein goals should be achieved gradually over the first week of ICU stay. This practice should be protocolised and regularly audited as critically ill patients receive only part of their energy and protein goals.

Metabolic demands of critically ill patients can be variable and nutrition support should be tailored to each patient. Given that many nutrition studies are on-going, we anticipate improvements in the individualisation of nutrition support in the near future.
