001). However, the decline in MED/claim was 5.4 times greater in the period following legalization (22.3mg/claim post vs. 4.1mg/claim pre). Total monthly opioid spending was also reduced to a greater extent post legalization ($Can267,000 vs. $Can95,000 per month). The findings were similar for private drug plans; however, the absolute drop in opioid use was more pronounced (76.9 vs. 30.8mg/claim). Over the 42-month period, gabapentin and pregabalin usage also declined.
Our findings support the hypothesis that easier access to cannabis for pain may reduce opioid use for both public and private drug plans.

Although rare, orbital cavernous hemangioma (OCH) is the most common benign orbital neoplasm in adults and may cause vision disturbance or loss due to optic nerve compression. The conventional treatment is surgical excision, which carries a risk of intraoperative nerve damage, whereas gamma knife radiosurgery (GKRS) can be a safe and effective alternative. Herein, we report the results of four patients with OCH treated with GKRS, and describe the method of treatment including the optic nerve protection.
This retrospective study included four consecutive patients (three women, one man; mean age 50 ± 14.7years) with OCH treated with single-session GKRS between 2014 and 2020. link= selleck compound Three patients had decreased visual acuity. During GKRS, the prescription dose delivered to the tumor margin was 12Gy at the 55-58% isodose line. The dose to the optic nerve margin was < 12Gy. Follow-up included sequential magnetic resonance imaging (MRI) and ophthalmological examinations at 6-month intervals.
The median follow-up period was 29.5 ± 23months (range, 12-63months). After GKRS, three patients with visual dysfunction had substantial vision improvement; the fourth patient continued to have normal vision without deterioration. Radiological outcomes after GKRS indicated an average tumor shrinkage of 70% ± 10.6% at the 6-month follow-up and 83% ± 2.64% at the 1-year follow-up. No adverse radiation effects were observed.
GKRS for OCH achieved favorable clinical outcomes, with substantial tumor volume reduction. OCH can be diagnosed based on characteristic MRI findings. GKRS may be considered a treatment option for OCH in selected cases.

Lung ultrasound can adequately monitor disease severity in pneumonia and acute respiratory distress syndrome. We hypothesize lung ultrasound can adequately monitor COVID-19 pneumonia in critically ill patients.
Adult patients with COVID-19 pneumonia admitted to the intensive care unit of two academic hospitals who underwent a 12-zone lung ultrasound and a chest CT examination were included. Baseline characteristics, and outcomes including composite endpoint death or ICU stay > 30days were recorded. Lung ultrasound and CT images were quantified as a lung ultrasound score involvement index (LUSI) and CT severity involvement index (CTSI). Primary outcome was the correlation, agreement, and concordance between LUSI and CTSI. Secondary outcome was the association of LUSI and CTSI with the composite endpoints.
We included 55 ultrasound examinations in 34 patients, which were 88% were male, with a mean age of 63years and mean P/F ratio of 151. The correlation between LUSI and CTSI was strong (r = 0.795), with an overall 15% bias, and limits of agreement ranging - 40 to 9.7. Concordance between changes in sequentially measured LUSI and CTSI was 81%. In the univariate model, high involvement on LUSI and CTSI were associated with a composite endpoint. In the multivariate model, LUSI was the only remaining independent predictor.
Lung ultrasound can be used as an alternative for chest CT in monitoring COVID-19 pneumonia in critically ill patients as it can quantify pulmonary involvement, register changes over the course of the disease, and predict death or ICU stay > 30days.

Oxidation of model fuel (MF) containing dibenzothiophene (DBT) was carried out using an equimolar mixture of H2O2-CH3COOH as an oxidant at 40-70 °C in the presence of IL. The sulfur compound is converted into polar sulfone, and the maximum desulfurization efficiency was examined. The effect of process parameters such as reaction temperature, reaction time, molar ratio of oxidant to sulfur (n(O/S)), and the mass ratio of ionic liquid to model fuel (m(IL/MF)) was studied, and the conditions for maximizing the DBT conversion rate were found. Maximum conversion (> 99%) was obtained at a temperature of 70 °C with m(IL/MF) of 0.8. The oxidation reactivity of various sulfur compounds was studied at different time intervals. To verify the effect of ionic liquid and ultrasound irradiation, extractive desulfurization (EDS), oxidative desulfurization (ODS), and UAODS in the presence of IL were carried out. The experimental results show that the UAODS process gives the highest desulfurization efficiency. A kinetic study was performed to estimate the rate constant and the order of oxidation reaction. In the present study, we evaluated Furcraea foetida for the phytoremediation of cadmium (Cd)-contaminated soils. We selected F. foetida because it is a drought-resistant plant, produces high biomass, and needs minimum maintenance. It belongs to the leaf fiber group of plants and therefore has economic importance. Since it is a non-edible crop, there is no danger of food chain contamination. Despite possessing these ideal characteristics, surprisingly, to date, the plant is underutilized for phytoremediation purposes. link2 Therefore, to evaluate the phytoremediation potential of the plant, we exposed it to five levels of cadmium (0, 25, 50, 100, and 200 mg Cd kg-1 soil) and studied its influence on growth, dry matter production, uptake, and translocation efficiency. The plant showed good tolerance to Cd 200 mg kg-1 soil without exhibiting any visible toxicity symptoms. The metal mainly accumulated in the roots (233 μg g-1dw), followed by leaf (51 μg g-1 dw). The bioconcentration factor was > 1, but the translocation factor was less then 1. selleck compound The plant was not classified as a hyperaccumulator of Cd; however, because of its high uptake (897 μ g-1 plant) and translocation efficiency (78%), we concluded that the plant could be utilized for phytoextraction of Cd from soils with low to moderately contaminated soils. The aim of the conducted study was to characterize the attitudes and practices of Polish farmers in the area of performing chemical plant protection treatments. A particular attention was paid to identifying the relationship between the direction of changes in the volume of chemical plant protection product consumption and selected attributes of farms. The main time range of the analyses covered the period of 2013-2017. Statistical data and results of representative surveys carried out on a sample of 1101 farms in Poland were used in the research process. Due to the large number of variants of the analysed variables, a multiple correspondence analysis was used, which made it possible to determine the correlation between the examined features (direction of changes in pesticide use relative to the farm area, economic size of the farm and location of the farm). Statistical analysis showed the existence of strong relationships between the physical (1) and economic (2) size of farms and the direction of changes in pesticide consumption ((1) φ2 = 0.0907; (2) φ2 = 0.1141)). According to empirical studies, the reduction of pesticide consumption took place mainly on the smallest farms. The implementation of the integrated plant protection directive has not resulted in significant changes in the form of reduced pesticide use in large-scale field crops. link3 This raises the need to modify the strategy and model of crop protection in large-scale field crops in Poland.Waterpipe (aka hookah) tobacco smokers are exposed to toxicants that can lead to oxidative DNA and RNA damage, a precursor to chronic disease formation. This study assessed toxicant exposure and biomarkers of DNA [8-oxo-7, 8-dihydro-2'-deoxyguanosine (8-oxodG)] and RNA [8-oxo-7,8-dihydroguanosine (8-oxoGuo)] oxidative damage during smoking of flavored and non-flavored waterpipe tobacco. link2 Thirty waterpipe smokers completed two counterbalanced 2-h lab waterpipe smoking sessions (flavored vs. non-flavored waterpipe tobacco). link3 Urinary concentrations of 8-oxodG and 8-oxoGuo and expired carbon monoxide (eCO) were measured before and after the smoking sessions. A significant increase in the urinary concentrations of 8-oxodG (from 2.12 ± 0.83 to 2.35 ± 0.91 ng/mg creatinine, p = 0.024) and 8-oxoGuo (from 2.96 ± 0.84 to 3.45 ± 0.76 ng/mg creatinine, p = 0.003) were observed after smoking the non-flavored and flavored waterpipe tobacco, respectively. Our results also showed that the mean ± SD of eCO increased significantly after smoking the flavored (from 1.3 ± 1.1 to 20.3 ± 23.6 ppm, p less then 0.001) and non-flavored waterpipe tobacco (from 1.8 ± 1.2 to 24.5 ± 26.1 ppm, p less then 0.001). There were no significant differences in the means of 8-oxodG (p = 0.576), 8-oxoGuo (p = 0.108), and eCO (p = 0.170) between the flavored and non-flavored tobacco sessions. Smoking non-flavored and flavored waterpipe tobacco leads to oxidative stress and toxicant exposure. Our findings add to the existing evidence about the adverse effects of waterpipe tobacco smoking (WTS) and the need for strong policies to inform and protect young people from the risks of WTS.
The aim of this study was to analyze changes in characteristics of HCC and the modes of LR over 20years in order to show the impact of those changes in the outcome of LR. In addition, BCLC staging was used to assess the limitations of this classification system and changes over the decade.
In our department, 500 liver resections (LR) were performed for hepatocellular carcinoma (HCC) over the 20years between January 2000 and February 2020. The 208 cases performed through 2009 were designated as Era 1, and the 292 cases between 2010 and February 2020 were termed Era 2. We analyzed changes in the characteristics of HCC and mode of LR (Study 1), and final outcomes of LR are shown according to the BCLC staging classifications and eras using data from the 5years after LR (Study 2).
In Era 1, the mean age of the patients was 68, while in Era 2 the mean age was 71, which was significantly older than the patients in Era 1. HCC that developed from non-B, non-C liver cirrhosis was significantly increased in Era 2 (45%) as compared to that in Era 1 (34%).
