Limited clinical evidence is available to help to predict poor outcomes after reverse shoulder arthroplasty (RSA) among patients with massive rotator cuff tears without glenohumeral arthritis. A retrospective case-control study was performed for patients who underwent RSA for massive rotator cuff tear without glenohumeral arthritis (Hamada score ≤3) and had a minimum of 2 years of follow-up. Preoperative risk factors for poor outcomes were subsequently analyzed. Sixty patients (mean age, 71.4±7.4 years) met the inclusion criteria. Of these, 18 (30%) patients had poor outcomes (case group). The case group had significantly worse postoperative Single Assessment Numeric Evaluation (SANE) (61.6±29.5 vs 84.9±14.1, respectively; P=.002), American Shoulder and Elbow Surgeons (58.9±22.5 vs 82.2±14.2, respectively; P less then .001), and Simple Shoulder Test (5.4±3.6 vs 8.5±2.4, respectively; P=.002) scores compared with the control group. Patients with poor outcomes had significantly higher preoperative SANE scores compared with control subjects (40.4±28.4 vs 18.8±15.7, respectively; P=.021). The results of this study suggest that patients with better overall preoperative function, as represented by higher SANE scores, have a greater likelihood of poor functional outcomes after RSA for massive rotator cuff tears without glenohumeral arthritis. For these patients, alternative treatment options should be considered. [Orthopedics. 2022;45(4)215-220.].We treated humeroradial joint disorder in rheumatoid elbows with arthroscopic partial excision of the radial head, in which the radial head is minimally resected under arthroscopy to ensure adequate joint space and articular congruity. To examine the effect of this method, we investigated outcomes using a retrospective case series. The hypothesis of this study was that this method decreases symptoms related to the humeroradial joint and ensures articular congruity. Since 2008, we have performed arthroscopic partial excision of the radial head for 14 patients (15 rheumatoid elbows) with more than 2 years of follow-up. Surgical indications for this method were motion pain with crepitus around the humeroradial joint and joint narrowing and sclerosis on plain radiography. After synovectomy, the surface of the radial head was resected 4 to 5 mm under arthroscopy, ensuring adequate joint space and articular congruity. Osteophyte removal and anterior capsular release were performed if necessary. At the final follow-up of 54 months, pain around the humeroradial joint had resolved in all cases. Range of motion improved from 115° flexion, -39° extension, 55° pronation, and 54° supination preoperatively to 127° flexion, -27° extension, 60° pronation, and 65° supination postoperatively. The articular congruity of the humeroradial joint was well maintained at final follow-up, with the exception of 2 cases in which the space decreased after 4 years. Arthroscopic partial excision of the radial head is a promising procedure for improvement of humeroradial symptoms. This method is effective, even for advanced cases, and should be considered before total arthroplasty. [Orthopedics. 2022;45(4)209-214.].The Sirveaux classification characterizes the severity of scapular notching after reverse total shoulder arthroplasty (rTSA). However, its reliability has not been validated. The goal of the current study was to determine the interobserver and intraobserver reliability of the Sirveaux classification. An online survey was sent to the American Shoulder and Elbow Surgeons (ASES), containing 10 radiographs showing a range of scapular notching. Members were asked to grade the degree of scapular notching with the Sirveaux classification system. Then ASES members from our institution regraded the images a second time after a minimum of 6 weeks. Fleiss' and Cohen kappa coefficients were calculated to determine the degree of interobserver and intraobserver reliability, respectively. A total of 50 ASES members graded the radiographs and 3 regraded images after more than 6 weeks. Fleiss' kappa coefficient was 0.2437, indicating fair interobserver agreement. Surgeons who perform more than 20 rTSA procedures per year (n=34) had a Fleiss' kappa of 0.2864. The mean Cohen kappa coefficient was 0.4763, indicating moderate intraobserver reliability. Surgeons should use additional means to describe the severity of notching, particularly when communicating with other physicians or publishing research. [Orthopedics. 2022;45(4)239-243.].Routine follow-up approximately every 2 to 5 years after total hip arthroplasty (THA) is a common practice. However, although patients are informed of the importance of follow-up, our mean follow-up rate for patients after standard non-metal-on-metal (MOM) THA is only 19%. The US Food and Drug Administration has released several statements on the importance of follow-up every 2 years after MOM THA. With the potential risks of MOM THA apparently widely known, we report on our ability to obtain timely follow-up at 2 separate centers. Two separate centers performed 570 MOM THA procedures between 2002 and 2010. An attempt was made to reach every patient by either telephone or letter to obtain ion levels, radiographs, and examinations. Repeat telephone calls and/or letters to those not reached were made annually. Patients were told of the unique importance of follow-up at each contact. Of the patients, 43% had not been seen within the past 5 years, and only 26% had been seen within the past 2 years. Only 61% had their first measurement of ion levels, and only 30% of patients had a second set of measurement of ion levels. A total of 48 revisions occurred in this group, and 36 patients died. Despite the apparent widespread dissemination of information regarding the potential risks of MOM THA and concerted efforts to contact patients for follow-up, we have been able to achieve a follow-up rate of only 26%. This rate is only marginally better than the mean follow-up for non-MOM THA in our practices. The implications of this poor follow-up are unknown. [Orthopedics. 2022;45(4)e196-e200.].Articular fractures of the distal humerus in adults are challenging fractures requiring adequate surgical exposure for optimum reconstruction. Most commonly, an articular osteotomy of the olecranon is performed, but complications have been reported related to both creating and repairing the articular osteotomy. We describe the use of an extra-articular olecranon osteotomy for approaching articular fractures of the distal humerus. We highlight the surgical steps required to obtain adequate exposure facilitating anatomical reduction, stable fixation, and early range of motion of the elbow joint. This technique can be added to the surgeon's armamentarium for the management of these complex injuries. [Orthopedics. 2022;45(4)e220-e225.].Primary total hip arthroplasty (pTHA) and revision total hip arthroplasty (rTHA) are not often compared in terms of patient-reported outcome measures (PROMs). However, surgeons and patients need to better understand the differences in PROMs between primary and revision surgery to set realistic patient expectations and recovery milestones. A matched cohort study of pTHA to rTHA was performed with our arthroplasty database of a single surgeon's experience from 2012 to 2018. There was a significant difference in both pre-operative assessment and change from preoperative to postoperative assessment of the PROMs. Patients undergoing pTHA had higher visual analog scale (VAS) pain scores (67.9 vs 57.9, P=.004). Those undergoing rTHA had higher Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) (57.1 vs 50.1, P=.008) and Veterans RAND 12 Item Health Survey (VR-12) (33.5 vs 33.1, P=.01) scores. However, a significant change was noted in the difference from preoperative to postoperative scores between the pTHA and rTHA groups Harris Hip Score (HHS) total score (pTHA 30.7 vs rTHA 4.4, P less then .001), WOMAC score (pTHA 29.3 vs rTHA 12.2, P less then .001), and VAS pain score (pTHA -48.3 vs rTHA -26.5, P less then .001) as well as groin pain (pTHA 1.4% vs rTHA 7.1%, P=.02). Further, PROMs after rTHA were inferior to those after pTHA with several outcome instruments, including HHS, WOMAC score, and VAS pain score. In addition, groin pain was significantly greater in the rTHA cohort compared with the pTHA cohort at the latest follow-up. This study allows surgeons and patients to better understand the differences in PROMs to set realistic patient expectations and recovery milestones. [Orthopedics. 2022;45(4)251-255.].Ideal component positioning is critical to outcomes in total hip arthroplasty. We describe our early results using digital radiographs with a combined ante-version technique. We report the incidence of component adjustments made based on digital radiographs and show how this technology can be used during patient positioning to achieve appropriate starting pelvic alignment. We reviewed 176 cases of primary total hip arthroplasty performed by a single arthroplasty surgeon using a posterior approach. Mean follow-up was 1 year (range, 3 months to 2.7 years). Digital radiographs (Surgeon's Checklist Hip; Radlink) and a combined anteversion technique were used for component positioning. For a subset of 100 patients, we recorded the incidence of pelvic mal-positioning found on digital radiographs obtained during initial positioning of the patient. For this same subset we also detail the component changes made as a result of intraoperative radiographs during trialing. Among 176 cases we have had 2 (1.1%) postoperative dislocations. Both underwent closed reduction with no further dislocation. We have had no revisions and 1 intraoperative calcar fracture. For our 100-case subset, digital radiographs during patient positioning identified pelvic malpositioning greater than 5° (coronal or axial plane) among 17% of cases. During trialing, we made component adjustments 71% of the time because of findings on intraoperative imaging. Digital radiography can be a valuable tool for component positioning during total hip arthroplasty. We experienced favorable early outcomes and show the utility of digital radiographs for proper pelvic positioning and making intraoperative adjustments to achieve optimal component placement. Ninety percent of orthopedic residents pursue fellowship training after residency, representing the highest rate of subspecialty training among surgical specialties. The goal of this study was to determine the factors considered most important by pediatric orthopedic fellowship program directors (PDs) in evaluating applicants and determining a rank list. A web-based survey was sent to all 42 US pediatric orthopedic fellowship programs. The PDs were contacted through publicly accessible email addresses found on program websites or the Pediatric Orthopaedic Society of North America website. Respondents were asked to indicate the fellowship program size and the number of applicants interviewed and ranked each year. The PDs were then asked to rank a list of 12 factors to reflect the relative importance of these criteria in evaluating fellowship applicants. Three emails were sent 1 at the initial survey release and 2 reminder emails at 2 and 4 weeks.
