ethic societies.Necrotizing fasciitis is a severe deep soft tissue infection with poor disease prognosis. The Aeromonas species is characterized as gram-negative, facultative, anaerobic small bacilli that are ubiquitously distributed in aquatic environments. Necrotizing fasciitis caused by this species is rare but has an extremely high mortality rate, especially in immunocompromised individuals. This study presents the case of a 39-year-old man with alcoholic liver cirrhosis, Child-Pugh class B, with necrotizing fasciitis caused by Aeromonas hydrophila. Despite debridement and bilateral above-knee amputation performed immediately, rapid progression to bilateral upper limbs and trunk was noted in 24 hours. The patient expired from septic shock with multiple organ failure in less than 48 hours following initial presentation. Two similar cases with different medical intervention and results have been reported in the literature and are further discussed in the present study. This allows the authors to suggest potential solutions for an improved clinical outcome.
Family presence during invasive procedures and cardiopulmonary resuscitation (FPDR) is a highly controversial issue, with divergence of opinion among health professionals, with clinical evidence in favor, and with barriers to its implementation in comprehensive patient care. Many international organizations support health policies for its implementation, but it is not a widespread practice. Little research has been conducted on this subject from the perspective of patients and families.

To learn opinions, perceptions, and desires of family members and patients in terms of family presence during invasive procedures and cardiopulmonary resuscitation.

Qualitative descriptive study, using semi-structured interviews with relatives and patients regarding their experiences and perceptions about FPDR (n = 18) from 8 October 2017 to 8 March 2018. The interviews were recorded and transcribed verbatim for later analysis using the method of content analysis.

This study has the approval of the Clinical Research Etdegree of invasiveness of the procedures. The results of this study provide information for a change of vision in patient-centered care.
The clinical use of angiotensin-converting enzyme inhibitors (ACEI) and angiotensin-receptor blockers (ARB) in patients with COVID-19 infection remains controversial. Therefore, we performed a meta-analysis on the effects of ACEI/ARB on disease symptoms and laboratory tests in hypertensive patients infected with COVID-19 virus and those who did not use ACEI/ARB.

We systematically searched the relevant literatures from Pubmed, Embase, EuropePMC, CNKI, and other databases during the study period of 31 December 2019 (solstice, 15 March 2020), and analyzed the differences in symptoms and laboratory tests between patients with COVID-19 and hypertension who used ACEI/ARB drugs and those who did not. All statistical analyses were performed with REVMAN5.3.

We included a total of 1808 patients with hypertension diagnosed with COVID-19 in six studies. alpha-Naphthoflavone solubility dmso Analysis results show that ACEI/ARB drugs group D-dimer is lower (SMD = -0.22, 95%CI -0.36 to -0.06), and the chances of getting fever is lower (OR = 0.74, 95%CI 0.55 to 0.98). Meanwhile, laboratory data and symptoms were not statistical difference, but creatinine tends to rise (SMD = 0.22, 95% CI 0.04 to 0.41).

We found that the administration of ACEI/ARB drugs had positive effect on reducing D-dimer and the number of people with fever. Meanwhile it had no significant effect on other laboratory tests (creatinine excepted) or symptoms in patients with COVID-19, while special attention was still needed in patients with renal insufficiency.

The preferred vascular access for hemodialysis is represented by arteriovenous fistula (AVF) due to fewer complications and more prolonged survival. Considerable efforts have been made to identify biomarkers associated with AVF dysfunction, but results are conflicting. Vascular cell adhesion molecule (VCAM-1) and advanced glycation end products are involved in atherogenesis, vascular calcification, peripheral artery disease, and neointimal hyperplasia in renal and non-renal patients. The objective of this study was to evaluate whether there is an association between VCAM-1, soluble receptor for advanced glycation end products (sRAGE), NcarboxymethylLysine (CML), and arteriovenous fistula dysfunction (AVF).

VCAM-1, sRAGE, and CML were performed using the ELISA technique in 88 HD patients. Ultrasound assessment of AVF reports brachial artery blood flow (Qa), brachial resistivity index (RI), presence of calcification, and the diameter. AVF dysfunction was defined as a brachial artery Qa ⩽ 500 ml/min or RI ⩾ ents with AVF dysfunction had a significantly higher level of VCAM-l. A positive correlation between VCAM-1 and sRAGE was identified in this group.
The results we obtained showed that patients with AVF dysfunction had a significantly higher level of VCAM-l. A positive correlation between VCAM-1 and sRAGE was identified in this group.
The therapeutic itinerary is not limited to the identification and availability of health services offered, but relates to the different individual searches and sociocultural and economic possibilities of each patient. In this study, we discuss the therapeutic itinerary of transsexual people seeking healthcare, from the user's perspective.

The aim of this study was to discuss the therapeutic itinerary of transsexual people seeking healthcare, from the user's perspective.

Individual interviews were performed with 10 transsexuals at the Trans Space of a University Hospital of Pernambuco, using the Universal Declaration of Human Rights as the theoretical reference and the Bardin's thematic content analysis as the reference methodological framework.

This study was approved by the Human Research Ethics Committee at the Federal University of Pernambuco under protocol no. 91284218.5.0000.5208.

The comprehensive care for transsexual people was evidenced through four categories analyzed low demand of transsender people may support evaluation processes of health service networks to ensure the access to and reorganization of these services. Understanding this dynamic allows fostering discussions about the structure of health services at all care levels for the care of this population. The effects of inclusion of powdered seeds of black cumin (B) (Nigella sativa L.) and fenugreek (F) (Trigonella foenum-graecum L.) on productive traits, selected blood constituents, microbiota and immunity of broilers were studied. A total of 648 day-old chicks were randomly assigned to nine treatments, with four pen replicates, each with 18 birds, including three levels of B seed powder (BSP; 0, 5 or 10 g/kg) and three levels of F seed powder (FSP; 0, 5 or 10 g/kg) in a 3 × 3 factorial arrangement. Neither powder affected feed intake. The FSP increased (p = 0.048) feed conversion ratio (FCR), but decreased daily BW gain (p = 0.02) between days 0 and 21, while BSP increased daily gain between days 22 and 42 and overall (both p = 0.005). Abdominal fat was decreased (p = 0.003) by BSP. Blood constituents were unaffected by either powder, but ileal Escherichia coli were decreased (p = 0.039) at day 42. The BSP increased a range of immunological titers, where BSP affected positively the measured variables. The interactions between BSP and FSP, specifically on broiler carcass cuts, suggested that where BSP is included at 10 g/kg, the inclusion of FSP at the same level may provide no additional benefit. Thus, while either powder could be included separately, the co-inclusion of both at 10 g/kg is not recommended.
An increasing number of patients worldwide are living with an ostomy. Among them, self-care management skills and stoma self-care abilities are associated with better quality of life.

To identify the sociodemographic and clinical characteristics of Italian ostomy patients and investigate their knowledge of stoma management.

A questionnaire was used to gather the sociodemographic and clinical data of 433 adult patients. Participants responded to a nine-item survey assessing their knowledge of stoma management.

Respondents reported having received extensive or adequate information in 64.4% of cases, and the remaining 35.6% reported receiving little or no information at any time. The health professional who imparted at least adequate information was a stoma nurse specialist in 32.1% of cases, usually during admission (24.0%) or before and during admission (19.6%).

This study demonstrated that most patients received adequate information regarding the ostomy.

A bezoar is a solid mass of indigestible material that accumulates in the digestive tract. After a blockage is identified, the nasogastric tube is usually removed and another one inserted. However, this can be dangerous and can easily cause tearing of the oesophageal mucosa, bleeding, and other serious complications. In this article, the authors present a case of nasogastric tube obstruction caused by oesophageal bezoars. After the nasogastric tube was replaced, the patient experienced two tears of the oesophageal mucosa. This article highlights the importance of the introduction of a procedure for nurses to follow in cases of nasogastric tube obstruction, bearing in mind the possibility of the presence of oesophageal bezoars. If necessary, a gastroscope should be used to ensure safe insertion of the nasogastric tube and prevent oesophageal mucosal tears.Several diseases and operations may necessitate the need for the formation of a stoma. Patients who are affected may be concerned about the effect of the stoma on their ability to carry out activities of daily living, as well as how it will affect their quality of life. Nurses who may be involved in the care of patients with a stoma should have an understanding of the reasons for stoma formation, and the types of stoma and appliances available, to enable them educate and support patients, and to allay any concerns.
Transgender individuals can have complex health needs, in both trans and non-trans related health, experiencing higher levels of discrimination and disadvantages in accessing health care. The health needs of the transgender community are not consistent with the wider population, so individuals are often required to research services to meet their needs which can contribute to a reluctance in accessing health care.

To analyse existing literature on transgender patients experiences of health care.

A search was performed of online databases (CINAHL, Medline, AMED, PubMed, ASSIA, PsychINFO, Web of Science and Scopus) and six studies were reviewed and analysed.

Three main themes emerged health professionals' knowledge and attitudes, navigating the system, and vulnerability and avoidance of health care.

The literature suggests that few encounters with health professionals were positive and that a lack of healthcare knowledge on trans-related issues and discrimination were the leading causes for dissatisfaction when accessing health services.
