The aim of this study was assessment of the laparoscopic surgery after using ileus tube(trans-anal or trans-nasal)for obstructive colorectal cancer compared with non-obstructive cancer.
Between April 2010 and March 2019, 129 patients underwent laparoscopic colorectal surgery. 97 patients were non-obstructive colorectal cancer(group N)and 32 patients were obstructive colorectal cancer(group O). Differences between the groups were analyzed using the Mann- Whitney's U-test, as appropriate.

In group O, the length of hospital stay was significantly long. There were no significant differences between-group differences in the operation time, estimated blood loss, the rate of conversion to open surgery and postoperative complications.

Lower gastrointestinal endoscopy showed a type 2 tumor in the rectal Ra, and biopsy revealed a well-differentiated adenocarcinoma. The patient had locally advanced rectal cancer with widespread contact with the sacrum, and preoperative radiochemotherapy (S-1 100 mg/day plus radiotherapy 50 Gy/25 Fr)was performed. After the treatment was completed, the tumor was remarkably reduced, but the surgery was strongly rejected, and therefore chemotherapy became the policy. XELOX plus bevacizumab therapy was started, but in the second course was performed, obstructive symptoms appeared, so a semi-urgent lower anterior resection and ileostomy were performed. Postoperative pathological findings showed only a small amount of tumor cells in the mucosa, suggesting that preoperative treatment was effective.A 47-year-old woman visited a neighboring hospital because of lower abdominal pain and high fever. She was diagnosed with acute pyelonephritis and administered(oral)antibiotics. However, there was no sign of improvement. She was referred to our hospital for closer examination. Computed tomography revealed an 8 cm abdominal wall abscess spreading from the prevesical space to the rectus abdominis. Incisional drainage was performed under local anesthesia. She developed rectus abdominis muscle diastasis, and a laparotomy was performed 2 months later. Intraoperative findings showed that her Meckel's diverticulum was continuous with the abdominal wall abscess. Diagnosed with Meckel's diverticulitis with abdominal wall abscess, the patient underwent surgery(excision)for these. selleck Histopathological findings showed adenocarcinoma cells in the abscess tissue and were continuous with gastric pyloric gland-like tissue in Meckel's diverticulum. Based on these findings, the patient was diagnosed with adenocarcinoma arising from ectopic gastric mucosa in the Meckel's diverticulum. The patient received postoperative adjuvant chemotherapy for a year. The patient is currently alive and has not experienced recurrence for 2 years since surgery. This case is rare due to the incidental finding of an abdominal abscess and the absence of recurrence 2 years after surgery.A 72-year-old woman was transferred to our hospital after being diagnosed with retroperitoneal and ovarian tumors. Abdominal computed tomography revealed a well-defined mass lesion measuring 35 mm on the dorsal side of the descending part of the duodenum that was surrounded by the head of the pancreas and inferior vena cava. In addition, a cystic mass measuring 90 mm was found in the pelvis. Hence, the patient was diagnosed synchronous retroperitoneal tumor and teratoma. Laparoscopic retroperitoneal tumor resection and right adnexectomy were performed. The pathological findings indicated that the retroperitoneal tumor was a diffuse large B-cell lymphoma and the pelvic tumor was a mature cystic teratoma of the ovary. On the basis of the computed tomography findings, we judged that the tumor had a poor tendency to infiltrate and could be peeled off and resected using laparoscopic surgery. In this rare case, the retroperitoneal tumor and mature teratocarcinoma were simultaneously resected laparoscopically.We report a rare case of hepatic fibrolamellar hepatocellular carcinoma(FLC). A 25-year-old Vietnamese male experienced loss of appetite and abdominal pain. He was referred for further examination of a 20 cm tumor in the left lobe of the liver detected in mass screening ultrasonography. He tested negative for HBs-antigen and HCV-antibody. The serum PIVKA- Ⅱ level was elevated. Liver function test findings were normal. The arterial phase of contrast enhanced abdominal CT revealed a 20×30 cm tumor that was well-enhanced, except for a central scar in the left lobe of liver. Enhanced MRI showed a high intensity tumor. T2-weighted MRI showed an iso-intensity tumor with a low-intensity central fibrous scar. Upon diagnosing the patient with FLC, we performed left hepatic trisegmentectomy. Pathological findings of the surgical specimen showed eosinophilic large neoplastic cells surrounded by fibrous stroma arranged in a lamellar fashion. This confirmed the diagnosis. FLC, which occurs in noncirrhotic livers of young patients, is a distinct clinicopathological variant of hepatocellular carcinoma. Hepatectomy for FLC should be accompanied with regional lymphadenectomy because of its association with lymph node metastasis. We also reviewed cases reported in Japan.Essential thrombocythemia(ET)is a rare myeloproliferative disorder characterized by thrombocytosis and a risk of thrombotic and hemorrhagic events. ET rarely occurs simultaneously with colorectal cancer. Including our case, only 5 cases of c o l orectal cancer with ET have been reported in Japan. Herein, we report a case of colon cancer in an ET patient who underwent laparoscopic right hemicolectomy. Our perioperative management avoided complications such as thrombosis or bleeding. An 81-year-old woman developed bloody stools. She was previously diagnosed with ET 9 years ago. Aspirin, cilostazol, and hydroxyurea(HU)were prescribed. Colonoscopy revealed a tumor at the ascending colon. Histopathological examination showed a well-differentiated tubular adenocarcinoma. Since the patient had anemia, aspirin and cilostazol were discontinued after diagnosis. HU was discontinued from the day before surgery to 2 days after surgery. Enoxaparin was subcutaneously administered for 1 to 3 days after surgery. Aspirin and cilostazol were resumed on the fourth day post-surgery.
