To analyse the incidence, risk factors and avoidability of acute kidney injury in adult patients admitted to a Dutch hospital.

Retrospective cohort study.

We assessed all consecutive admissions in our clinic during one week for the development of acute kidney injury and its cause. We compared medical information between patients with and without acute kidney injury to identify risk factors. We reviewed whether the current advice on kidney injury was followed to estimate the avoidability of acute kidney injury.

347 patients were older than 18 years and admitted for more than 1 day. Acute kidney injury occurred in 16.4% of patients. Almost half of the patients already developed acute kidney injury at home, before admission. Acute kidney damage was encountered in all medical specialties. Chronic kidney disease, diabetes mellitus and heart failure were significantly more common in patients with acute kidney injury. Patients with acute kidney injury used significantly more RAS-inhibitors and loop diuretics. The renal function completely restored in half of cases. This article describes a patient which was admitted to our hospital with severe polyarthritis and panniculitis. A meticulous work-up revealed an underlying focal alcoholic pancreatitis. The clinical course in our patient illustrates the severity of the PPP syndrome and emphasizes the need of a multidisciplinary approach.

Although we know that apprenticeships are effective in medical workplaces, its precise mechanisms remain tacit and implicit. However, decreasing exposure to relevant learning experiences forces us to develop an understanding of teaching and learning in apprenticeships. Central to the learning in apprenticeships are entrustment of autonomy and supply of expertise by the supervisors. Supervisors perform risk-assessments to estimate how much autonomy can be entrusted to their residents continuously. Such assessments are particular challenging to supervisors when the margins for error are narrow, for instance during complex tasks of surgical procedures. In this perspective we present our insights regarding the teaching and learning during surgical procedures. These guidelines differ in the extent to which they consider cardiovascular risk factors when deciding to start allopurinol. Observational studies and limited trial data suggest that treatment with allopurinol may reduce the risk of cardiovascular events in patients with gout. To describe trends and developments in the total duration of training medical specialists in the Netherlands, including their ages upon completion of the components of medical education undergraduate medical program, waiting time and further post-graduate specialist training.

Population cohort study.

From the combined data from Statistics Netherlands and the Registration Committee for Medical Specialists for the period 1986 to 2018 relevant populations were selected and training trajectories were mapped. The population size was 40,604 individuals for undergraduate medical programs, 41,885 for the duration of post-graduate specialist training, 31,915 for the waiting periods and 21666 for the total duration of the trajectory from the start of medical school until registration as a specialist.

The median duration of undergraduate medical programs was 7.1 years, which is longer than the nominal duration and this remained unchanged over the observation period. The average waiting time between graduating meage age at start of postgraduate specialist training has decreased, medical specialists are on average younger upon registration.Intermittent fasting (IF) is a broad concept and covers several fasting regimes. Studies of 'early time restricted feeding' and 'alternate day fasting' with energy restriction show a greater effect on weight and cardiometabolic health in overweight people in the short term, compared to a continuous caloric restriction (CCR). 'Late time restricted feeding' seems to have no or unfavorable effects. Long-term studies (up to 2 years) suggest that IF regimens are not superior to continuous caloric restriction.
