Although the incidence of immune checkpoint inhibitor (ICI)-related cardiovascular (CV) toxicity is low, the overall burden of CV events after ICI is unknown. Risk factors for CV events after ICI have yet to be identified.

We sought to evaluate the association between vascular calcification on routine baseline computed tomography (CT) imaging and CV events following ICI.

This was a single-center, retrospective cohort study of 76 patients referred to Cardio-Oncology with prior ICI treatment. Coronary and aortic calcification on non-gated chest and abdominal CT imaging were qualitatively assessed. The association of baseline clinical parameters and vascular calcification with symptomatic heart failure (HF), acute coronary syndrome, myocarditis, symptomatic arrhythmia, or pericardial effusion after ICI was evaluated.

Over 11months of follow-up, there were 80 CV events that occurred in 49 patients. Worse coronary and aortic calcification on pre-treatment CT imaging was seen in patients with a CV event (p=.018 and p=.014, respectively). There were no differences in traditional CV risk factors between those with and without a CV event. Eighteen patients (37%) were restarted on ICI therapy after a non- myocarditis or symptomatic systolic HF CV event without recurrent events or mortality over 13months of follow-up.

These novel therapeutic agents exert their antitumor effects by activating the patients' immune system against cancer cells. Immunotherapy can be divided into active agents, such as anti-tumour vaccines or adoptive T-cell transfer, and passive immunotherapies like monoclonal antibodies, checkpoint inhibitors, cytokine therapy, bispecific T-cell engagers. After initial experimental use, broad clinical application revealed a number of important cardiovascular side effects of immunotherapeutics, which limit treatment options and decrease patients' prognosis and quality of life. With the rising rate of new immunotherapeutics at a hand, the number of patients receiving cancer immunotherapy will constantly increase, resulting in improved long-term survival rates. This review aims to summarize available cancer immunotherapies, their mechanism of action, currently known cardiovascular toxicities and their treatment. Further optimization of patient care will depend on the combined efforts by oncologists, cardiologists and cardiac surgeons to identify patients at risk and the implementation of interdisciplinary screening and treatment strategies. To evaluate subclinical liver fibrosis after Fontan procedure using a comprehensive method that reflects the overall liver status rather than the focal biopsy lesion.

This cross-sectional study included 66 patients without symptoms of liver disease who had at least 5years of elapsed time since the Fontan procedure (time since Fontan procedure 17.5±7.3years). Serum enhanced liver fibrosis (ELF) scores and 2D shear wave elastography (SWE), which are expected to detect liver fibrosis earlier than liver enzyme tests and ultrasonography, were evaluated along with general examinations.

Liver cirrhosis diagnosed by ultrasonography showed a marked increase 16years after surgery (elapsed time 6-15years since Fontan, 43.8%; 16-25years, 70.8%; 26-35years, 90.0%). The age-adjusted ELF score and liver stiffness according to SWE were elevated during the early postoperative period (6-15years since Fontan). In adulthood (elapsed time>16years), the adjusted ELF score was correlated with liver stiffness (r=0.514, p=.009) and liver enzyme levels. There was no correlation between liver fibrosis and the Fontan palliation type. AST to platelet count ratio and FIB-4, which are non-invasive fibrosis markers, presumed absence of fibrosis in approximately 90% of patients.

Acute kidney injury (AKI) is a common complication following acute myocardial infarction (AMI) and associated with worse outcomes. Serum Potassium levels (K, mEq/L), which are regulated by the kidneys, are related with poor prognosis in patients with AMI.

To evaluate whether K levels predict imminent AKI in patients with AMI.

This retrospective nested case-control study was based on medical records of hospitalized AMI patients, 2002-2012. The cases (AKI group) were defined as an increase of ≥1.5-fold in serum creatinine level or a decrease of ≥25% in the estimated glomerular filtration rate (eGFR) during the hospitalization. The control group comprised of matched randomly selected patients that did not develop AKI. For both groups, all creatinine and K levels were obtained for up-to 72h prior to the AKI diagnosis (index time).

A total of 12,498/17,678 admissions met the inclusion criteria. The AKI and the control groups consisted of 430 and 1345 matched admission respectively. K levels, prior AKI diagnosis seemed to be higher in the AKI group. Multivariate analysis showed that K≥4.5 within 36-56h prior to the index time was an independent predictor of the subsequent AKI, OR=2.3, p<.001. The c-statistic of the model was 0.859, p<.001. Predictivity of K for AKI was stronger among ST-elevation (STEMI) vs. Non-ST-elevation AMI (NSTEMI) patients (OR=4, p<.001 vs. 1.7, p=.025 respectively; p-for-interaction=0.038).

The method to perform a precise mapping of non-pulmonary vein (PV) triggers has not been fully investigated. The purpose of this study was to assess the efficacy of self-reference mapping for eliminating non-PV triggers in a large series of patients including the long-term outcomes.

Among 446 atrial fibrillation (AF) ablation procedures in 431 patients at 2 institutions, we prospectively enrolled patients who had reproducible non-PV triggers. Non-PV triggers from the left atrial posterior wall (LAPW) and superior vena cava (SVC) were excluded. Ablation procedure and long-term clinical outcomes were evaluated. The origin of non-PV triggers were detected using a self-reference mapping technique, which does not require any other reference catheters. Instead of using signals obtained from a fixed intracardiac catheter as the reference, an operator repeatedly moved a multi-electrode catheter to the earliest site creating a new reference each time to map the non-PV trigger.

A total of 32 non-PV triggers excluding origins from the LAPW and SVC were induced in 23 patients.
