Qualitative research has gained increasing prominence in health-related research and is experiencing greater use in dermatology. A major strength of and reason to perform qualitative research is that it allows one to gain an understanding of the insider (e.g., patient, medical provider, other players in the healthcare system) perspectives on health and insights about their behaviors, motivations, and expectations. This is particularly important in the field of dermatology where most diseases are, fortunately, not directly fatal but have major effects on affected individuals' lives in ways that are often not readily quantifiable. As such, it is important for dermatologists, both researchers and clinicians, to understand the basic tenets of qualitative methodology to properly utilize qualitative methods, evaluate the qualitative literature, and identify relevant findings to apply to clinical practice. In this article, we provide an overview of qualitative research and some of the more commonly used qualitative methods with an aim to increase awareness and appreciation of the power of qualitative studies.
Frailty predisposes patients to poor postoperative outcomes. We evaluated whether using local rather than general anesthesia for hernia repair could mitigate effects of frailty.

We used the Risk Analysis Index (RAI) to identify 8,038 frail patients in the 1998-2018 Veterans Affairs Surgical Quality Improvement Program database who underwent elective, open unilateral inguinal hernia repair under local or general anesthesia. Our outcome of interest was the incidence of postoperative complications.

In total, 5,188 (65%) patients received general anesthesia and 2,850 (35%) received local. Local anesthesia was associated with a 48% reduction in complications (OR 0.52, 95%CI 0.38-0.72). Among the frailest patients (RAI≥70), predicted probability of a postoperative complication ranged from 22 to 33% with general anesthesia, compared to 13-21% with local.

Local anesthesia was associated with a ∼50% reduction in postoperative complications in frail Veterans. Given the paucity of interventions for frail patients, there is an urgent need for a randomized trial comparing effects of anesthesia modality on postoperative complications in this vulnerable population.

How the translucency and color of ceramic restorations are affected by surface changes from the corrosive environment in the oral cavity and aging of materials is unclear.

The purpose of this invitro study was to compare the impact of acidic exposure and aging on the color and translucency of monolithic zirconia and lithium disilicate ceramics.

Twenty computer-aided design and computer-aided manufacturing (CAD-CAM) monolithic zirconia specimens and 20 pressed lithium disilicate specimens were fabricated. Half of the specimens of each group were aged in an autoclave (7.5 hours, 134 °C, 0.2 MPa), and the rest were immersed in hydrochloric acid to simulate the acidic conditions in the oral cavity from gastric reflux. The color coordinates L∗, a∗, and b∗ were measured with an ultraviolet spectrophotometer before and after aging or acidic storage. In this study, cervical epithelial cells from mice during the follicular phase were cultured invitro and treated with different concentrations of P4 (5, 10, and 20nM). The results of the present study suggest that treatment of murine cervical epithelial cells with 10nM P4 for 24h contributed to (1) significantly increased expression of protein kinase A (PKA), cytosolic phospholipase A2 (cPLA2) and PGE synthase (PGES)-1; (2) higher phosphorylated (p-) to total extracellular signal-regulated kinase (ERK) 1/2 and hormone-sensitive lipase (HSL) ratios; (3) a significant decrease in the number of lipid droplets (LDs) and fatty acid content within LDs in epithelial cells; and (4) enhanced arachidonic acid and PGE2 levels in cells compared with the control (0nM P4) group (P less then 0.01 for all findings). In contrast, the PKA inhibitor H89 contributed to significantly decreased cPLA2, PGES-1 and HSL expression, ERK1/2 phosphorylation and arachidonic acid and PGE2 levels, even in the presence of P4. These data show that P4 can act via the PKA/ERK1/2 pathway to stimulate lipolysis of triacylglycerol in the LD core and degradation of phospholipid in the LD membrane to promote PGE2 synthesis in murine cervical epithelial cells.Exosomes are nanoscale vesicles released from different-type cells. The effect of antibodies on cell migration was investigated using TranswellTM assay. Gelatin zymography and Western blotting were used to detect the changes of matrix metalloproteinase 2 (MMP2), MMP9, focal adhesion kinase (FAK) and phosphorylated FAK (p-FAK) after treatment with humanized anti-tissue factor antibody. Results The purity of the humanized anti-tissue factor antibody was estimated 96.9% by the two-step method. The purified antibody showed an obvious anticoagulant activity. The antibody treatment had a significant killing effect on colorectal cancer cells through ADCC and a significant inhibiting effect (99%) on cell migration. The antibody significantly inhibited expression of MMP2 and p-FAK. Conclusion The humanized anti-tissue factor antibody can effectively kill tumor cells through ADCC and inhibit cancer cell migration, which is possibly mediated by MMP2 expression through suppression of FAK signaling.Objective To investigate the regulatory effect of interleukin-6 (IL-6) on the expression of indoleamine 2, 3-dioxygenase (IDO) in the early pregnant chorionic villi and decidua tissues. Methods The chorionic villi and decidua tissues of women who received induced abortion at early pregnancy were collected. The expression of IL-6 and IDO in the chorionic villi and decidua tissues was detected by Western blotting. Subsequently, 10, 50 and 100 ng/mL IL-6 was added into the chorionic villi and decidua tissues to culture for 48 hours. In addition, changes in the IDO mRNA and protein expression levels in chorionic villi and decidua tissues were detected by real-time quantitative reverse PCR (qRT-PCR) and Western blotting. Results Both IDO and IL-6 were expressed in human early pregnant chorionic villi and decidua tissues. Besides, the expression of these two proteins were positively correlated (r=0.72, 0.91). After being cultured with 10, 50 and 100 ng/mL IL-6 for 48 hours, IDO protein expression significantly increased in the cultured early pregnant chorionic villi and decidua tissues in an IL-6 concentration-dependent manner. Conclusion The expression of IL-6 and IDO proteins at the maternal-fetal interface show a positive correlation in normal physiological pregnancy, and IL-6 may up-regulate the expression of IDO.Objective To investigate the potential mechanism of follicular regulatory T (Tfr) cells in the pathogenesis of rheumatoid arthritis (RA). Methods The percentage of CD4+CXCR5+FOXP3+ICOS+ Tfr cells in the peripheral blood of 20 patients with RA and 20 healthy subjects were detected by flow cytometry, and the correlation between the proportion of Tfr cells and the clinical laboratory index of RA was analyzed. The serum levels of interleukin-10 (IL-10), IL-12, IL-2, transforming growth factor-β (TGF-β) and C-X-C motif chemokine ligand 13 (CXCL13) were measured by ELISA. The mRNA expression of Bcl6 and B lymphocyte induced mature protein 1 (Blimp-1) in peripheral blood mononuclear cells (PBMCs) was detected by real-time quantitative PCR. Immunohistochemistry was used to detect the expression of Bcl6 and CXCL13 in synovium of RA patients. Results The percentage of Tfr cells in the RA patients significantly decreased compared with the healthy controls, which was negatively related to the C reactive protein (CRP), rheumatoid factor (RF), erythrocyte sedimentation rate (ESR), disease activity score in 28 joints (DAS28). The serum levels of IL-12 and CXCL13 in the patients with RA went up obviously, while the levels of IL-2, IL-10 and TGF-β went down remarkably. The Bcl6 mRNA level in PBMCs of the RA patients was significantly raised, while the expression of Blimp-1 mRNA was significantly reduced. Bcl6 and CXCL13 were highly expressed in the synovium of the RA patients. Conclusion The decrease of the proportion of Tfr cells in the peripheral blood may be related to the occurrence and development of RA.
