Brazil has one of the greatest hydroelectric potential in the world with high number of reservoirs for the electricity generation. However, little is known about the influence of these environments on the water quality. The water quality monitoring data from 14 stations distributed throughout the Irapé HPP reservoir (lentic environment), and its main tributaries (lotic environment), between the years 2008 and 2018, were evaluated and compared to assess the spatial variability of water quality. The analyzed parameters included total alkalinity, thermotolerant coliforms, electric conductivity, biochemical oxygen demand, dissolved iron, total phosphorus, nitrate, total ammoniacal nitrogen, dissolved oxygen, pH, total dissolved solids, sulfate, water temperature, and turbidity. Cluster analysis (CA), Kruskal-Wallis (KW) tests, Spearman rank-order correlation, and principal component analysis (PCA) were applied to identify and compare the relationship between the main parameters in the lotic and lentic environments. The CA resulted in four clusters according to proximity and the environment type (lotic or lentic). In general, the water quality showed better conditions in the reservoir and in the lotic stations on the immediate surround. The results may be associated with the greater sedimentation in the lentic environment. The analyses indicated that agricultural activities and the geochemical characteristics of the region are the main responsible for changes in the water quality.
Idiopathic free-floating thrombus (FFT) of the aorta is a rare occurrence, but it can lead to catastrophic consequences. The initial symptoms are typically cerebral or peripheral embolisms. Surgical thrombectomy and thrombolysis are two primary treatments for FFT. Here, we report three cases of patients with idiopathic FFT in the absence of coagulopathy who were treated successfully by surgery with no recurrent thrombi or relapse of symptoms.
Case 1 involved a 72-year-old male patient with a pedunculated thrombus in the distal aortic arch. Case 2 involved a 62-year-old female patient with a cylinder thrombus in the aortic arch and left common carotid artery. Case 3 involved a 65-year-old male patient with three pedunculated thrombi in the ascending aorta, aortic arch, and left subclavian artery. None of the patients had clinical signs of coagulopathy. Pedunculated or cylinder thrombi have a greater risk of breaking off, which can produce severe peripheral embolism in contrast with intramural thrombi (73% ld be selected carefully according to the location of the thrombus. After surgery, anticoagulant is important to prevent recurrent idiopathic thrombi.
For a large pedunculated or cylinder thrombus located in the thoracic aorta, surgical thrombectomy should be performed. And, in surgical thrombectomy, the location of the cannulas and cross-clamp should be selected carefully according to the location of the thrombus. This study uses a series of transgenic murine models to explore for the first time the role of the hypoxia-inducible factors, HIF-1α and HIF-2α in the pulmonary and systemic circulations as potential regulators of systemic vascular function in normoxic or hypoxic conditions and in response to inflammatory stress. We developed a series of transgenic mouse models, the HIF-1α Tie2Cre, deficient in HIF1-α in the systemic and pulmonary vascular endothelium and the L1Cre, a pulmonary endothelium specific knockout of HIF-1α or HIF-2α. In vivo, arterial blood pressure and metabolic activity were monitored continuously in normal atmospheric conditions and following an acute stimulus with hypoxia (10%) or lipopolysaccharide (LPS). Forty-eight gilts with an initial weight of 21.75 ± 0.138 kg and final weight of 98.65 ± 2.106 kg were subjected to one of the following six dietary soybean oil inclusions (0.00, 1.086, 2.173, 3.259, 4.345, and 5.432%). Experimental design was completely randomized block with six treatments and four replicates of two animals each. Lipid profile of backfat and longissimus lumborum muscle was analyzed by gas chromatography. Increasing dietary soybean oil levels did not influence final weight, daily weight gain, and feed intake (P > 0.05) but improved feed to gain ratio (P less then 0.05). The inclusion of soybean oil modified the lipid profile of backfat and muscle, reduced saturated and monounsaturated fatty acids, and increased polyunsaturated fatty acids concentration, mainly linoleic and α-linolenic acids (P less then 0.05). Increasing dietary soybean oil inclusion decreased atherogenic and thrombogenic indexes, and the omega-6omega-3 ratio of the backfat and longissimus lumborum muscle (P less then 0.05). Given the increased incidence of rheumatic conditions in older adults, it is of great importance for healthcare providers to be aware of the potential benefits and risks of antirheumatic drugs. The present article aims to provide a comprehensive review regarding antirheumatic drug use in older patients, particularly by focusing on safety issues and polypharmacy. Antirheumatic medications include nonsteroidal anti-inflammatory drugs (NSAIDs), glucocorticoids and disease-modifying antirheumatic drugs (DMARDs), which comprise conventional synthetic DMARDs, targeted synthetic DMARDs and biological DMARDs. Due to the alteration in drug pharmacokinetics and pharmacodynamics in old people, antirheumatic drug efficiency and safety may be different than in the younger population. Polypharmacy and multimorbidity are other potential challenges to be faced during the treatment of older patients with rheumatic diseases. To analyze the amount of free abdominal gas and ascites on computed tomography (CT) images relative to the location of a perforation.
We retrospectively included 172 consecutive patients (9379 = mf) with GIT perforation, who underwent abdominal surgery (ground truth for perforation location). The volume of free air and ascites were quantified on CT images by 4 radiologists and a semiautomated software. The relation of the perforation location (upper/lower GIT) and amount of free air and ascites was analyzed by the Mann-Whitney test. Furthermore, best volume cutoff for upper and lower GIT perforation, areas under the curve (AUC), and interreader volume agreement were assessed.
There was significantly more abdominal ascites with upper GIT perforation (333ml, range 5 to 2000ml) than with lower GIT perforation (100ml, range 5 to 2000ml, p = 0.022). The highest volume of free air was found with perforations of the stomach, descending colon and sigmoid colon. Significantly less free air was found with perforations of the small bowel and ascending colon compared to the aforementioned. An ascites volume > 333ml was associated with an upper GIT perforation demonstrating an AUC of 0.63 ± 0.04.
Using a two-step process based on the volumes of free air and free fluid can help localizing the site of perforation to the upper, middle or lower GI tract.

To evaluate the associations of DECT parameters with extracorporeal shock wave lithotripsy (ESWL) outcomes in pediatric patients.
A retrospective study of consecutive patients with calculi who underwent ESWL and DECT in our hospital was performed in 2011-2019. The primary outcome was DECT imaging's correlation with ESWL outcomes. The secondary outcome was to determine DECT parameters independently predicting ESWL outcomes, including stone-free (SF) and residual stone (RS) statuses.
The study included 207 patients. The mean CT attenuations at 140 kVp, 80 kVp, and 120 kVp and effective atomic number (Zeff) were significantly correlated with stone free (SF) and residual stone (RS) (P < 0.05). Areas under the curves (AUCs) of CT attenuations at 120 kVp, 80 kVp, 140 kVp, and dual-energy index (DEI) were 0.784 (95% CI 0.672-0.897), 0.780 (95% CI 0.677-0.884), 0.766 (95% CI 0.658-0.885), and 0.709 (95% CI 0.578-0.840) (all P < 0.05). With cutoffs of 882.5, 1330.5, 1042.5, and 0.103 for CT attenuations at 140 kVp, 80 kVp, 120 kVp, and DEI, respectively, sensitivities and specificities were 75.0% and 31.1%, 83.3% and 31.8%, 80.3% and 31.1%, and 58.3% and 44.7%, respectively.
Our results demonstrated that the parameters of DECT could be used to predict ESWL outcomes (especially the SF status) in children with urolithiasis. ESWL success can be accurately predicted by DECT, and it is hard to predict ESWL failure.
