The optimum conditions for the AgNPs synthesis were found as 0.25 mM AgNO3 concentrations with pH 12 values at 45°C after 64 hr incubation in dark. Biosynthesized AgNPs were characterized via microscopic and spectroscopic techniques such as transmission electron microscopy, scanning electron microscopy, energy dispersive X-ray analysis, Fourier transform infrared spectrophotometer, and ultraviolet-visible spectroscopy. Zetasizer measurements presented that the high negative potential value (-18.1 mV) and PDI (0.495) supported the excellent colloidal nature of AgNPs with long-range stability and high dispersity. AgNPs exhibited cyto-genotoxicity in Allium cepa root meristem cells by decreasing mitotic index and increasing chromosome aberrations in a dose-dependent manner. Then, 100 and 50% concentration of biosynthesized AgNPs showed antibacterial activity on Staphylococcus aureus and Bacillus subtilis. A decreasing biofilm formation of Pseudomonas aeruginosa 80.69, 48.32, and 28.41% was also observed at 100, 50, and 25% of mycosynthesized AgNP, respectively.The potential role of active CMV infection in promoting acute Graft-versus-Host Disease (aGvHD) in allogeneic hematopoietic stem cell transplantation (allo-HSCT) remains a matter of debate. We further addressed this issue conducting a retrospective, observational, multicenter study of 632 patients subjected to allogeneic peripheral blood HSCT at 20 Spanish centers. Monitoring of CMV DNA load in plasma or whole blood was performed by real-time PCR assays. Cumulative incidence of CMV DNAemia was 48.9% (95% CI, 45%-52.9%), of any grade aGvHD, 45.6; 95% (CI, 41.3%-50.1%), and of grade II-IV aGvHD, 30.7 (95% CI, 24.9%-36.4%). Overall, development of CMV DNAemia at any level resulted in an increased risk of subsequent all grade (HR, 1.38; 95% CI, 1.08 - 1.76; P = .009) or grade II-IV (HR, 1.58; 95% CI, 1.22 - 2.06; P = .001) aGvHD. The increased risk of aGvHD linked to prior occurrence of CMV DNAemia was similar to the above when only clinically significant episodes were considered for the analyses (HR for all grade aGvHD, 1.48; 95% CI, 1.13 - 1.91; P = .041, and HR for grade II-IV aGvHD, 1.53; 95% CI. 1.13-1.81; P = .04). The CMV DNA doubling time in blood was comparable overall in episodes of CMV DNAemia whether followed by aGvHD or not. Whether CMV replication is a surrogate risk marker of aGvHD or it is causally involved is an important question to be addressed in future experimental research.
The risk of progression of indeterminate observations to hepatocellular carcinoma (HCC) after direct-acting antivirals (DAA) is still undetermined. To assess whether DAA therapy changes the risk of progression of observations with low (LR-2), intermediate (LR-3) and high (LR-4) probability for HCC in cirrhotic patients and to identify predictors of progression.

This retrospective study included cirrhotic patients treated with DAA who achieved sustained virological response between 2015 and 2019. A total of 68 patients had pre-DAA indeterminate observations and at least six months CT/MRI follow-up before and after DAA. Two radiologists reviewed CT/MRI studies to categorize observations according to the LI-RADSv2018 and assess the evolution on subsequent follow-ups. Predictors of evolutions were evaluated by using the Cox proportional hazard model, Kaplan-Meier method and log-rank test.

A total of 109 untreated observations were evaluated, including 31 (28.4%) LR-2, 67 (61.5%) LR-3 and 11 (10.1%) LR-4. During a median follow-up of 41months, 17.4% and 13.3% of observations evolved to LR-5 or LR-M and LR-5, before and after DAA respectively (P=.428). There was no difference in rate of progression of neither LR-2 (P=1.000), LR-3 (P=.833) or LR-4 (P=.505). At multivariate analysis, only initial LI-RADS category was an independent predictor of progression to LR-5 or LR-M for all observations (hazard ratio 6.75, P<.001), and of progression to LR-5 after DAA (hazard ratio 4.34, P=.047).

Malnutrition reflects the general condition of a patient including physical condition, protein turnover, and immune competence. Contrast-induced acute kidney injury (CI-AKI) is a disorder that adversely affects the prognosis of older adults. In our study, we aimed to show the relationship between CI-AKI and malnutrition status in elderly patients over 65years of age with chronic coronary artery disease (CAD).

Study enrolled 360 consecutive patients with coronary angiography performed because of chronic coronary artery disease. Patients pre-procedural and post-procedural blood samples were taken and prognostic nutritional index (PNI), controlling nutritional status (CONUT) score, and geriatric nutritional risk index (GNRI) malnutrition scores were calculated.

The median age of the patients included in the study was 69 (67-72, IQR) and CI-AKI was seen in 91 (25.2%) patients. Univariate regression analysis showed that age, diabetes mellitus, baseline creatinine, body weight-adapted contrast agent, haemoglobin, left ventricular ejection fraction, CONUT score, PNI score, and GNRI score were independent predictors of CI-AKI. In model 1, increase in CONUT score (2 to 5) (OR 3.21 (2.11-4.88), in model 2, increase in PNI score (37.4 to 45) (OR 0.34, (0.24-0.49)), and in model 3, increase in GNRI score (89.5 to 103.8) (OR 0.55, (0.38-0.81)) were independently associated with the presence of CI-AKI. PNI showed better results than other models in discriminating the predictable capability for CI-AKI.

The concentrations of heavy metals were higher in the root as compared to shoot and were more pronounced in SH-13 than FA-08 cultivar. The decrease in the seedling length and biomass was observed when the metals were applied in combined form (Cd-Cr 80-120, Cd-Cr 100-120, Cr-Cd 140-80, and Cr-Cd 140-100). There were more declines in root length in the cultivar SH-13 as compared to the shoot length, as the concentration of HMs increased. The root at level Cr-140 and shoot at level Cd-100 showed more reduction in SH-13 than FA-08. The high concentration of Cd and Cr affected the root epidermis, the cortical cells, and the xylem vessel. The size and number of stomata, length of long cells and short cells, and trichome were reduced at the concentration Cd-100 and Cr-140. There are no publications addressing the level of experience Australian surgical trainees achieve in inguinal hernia surgery. Internationally, some training boards have set minimum competency requirements, but this is not true in Australia. The longer learning curve for laparoscopic inguinal hernia repairs (LIHRs) compared to open inguinal hernia repairs (OIHRs) has placed greater demands on trainees.

Logbook data on OIHR and LIHR for Australian surgical trainees who graduated as fellows between 2013 and 2018 were obtained. A literature review was performed to analyse international published logbook numbers for surgical trainees from the past decade. International training board requirements, estimations of the learning curve and hernia society guidelines for each procedure were reviewed.

In total, 7946 operations were recorded from 58 trainees. On average 49.2 OIHRs (range 13-101), 21.5 LIHRs (range 1-94) and 71.1 inguinal hernia repairs overall (range 25-129) were performed during training. The European Hernia Society recommends that at least 30-50 of each procedure be performed during training. The learning curves for LIHRs (50-100 procedures) have been shown to be longer than for OIHRs (40-64 procedures).

Australian general surgical trainees are achieving adequate primary operator logbook numbers for OIHRs but are not completing the required number of LIHRs. The tailored approach to inguinal hernia repair requires skill in both open and laparoscopic repairs. This may not be possible with the current training structure in Australia.
