Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. Background

Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision making. However, the usage of the term "pragmatic" is inconsistent and its definition as well as assessment requires clarification. The purpose of pragmatic trials is to inform policy and clinical practice decisions, not to confirm the validity of a clinical or physiological hypothesis. A pragmatic trial should aim to be as similar to actual clinical practice as possible, such as its participation of participants, setting and design as well as the implementation of the intervention, determination and analysis of the outcomes, and primary analysis. This is a major distinction from explanatory trials (as described by Schwartz and Lellouch1), which are designed to provide more thorough confirmation of the hypothesis.

Trials that are truly pragmatic should avoid attempting to blind participants or the clinicians, as this may cause distortions in estimates of the effect of treatment. This could lead to false claims about pragmatism, and the usage of the term should be made more uniform. The creation of a PRECIS-2 tool that provides an objective and standardized evaluation of the pragmatic characteristics is the first step.

Methods

In a practical study it is the intention to inform policy or clinical decisions by showing how an intervention can be integrated into routine treatment in real-world settings. Explanatory trials test hypotheses about the causal-effect relationship in idealized settings. Consequently, pragmatic trials may be less reliable than explanatory trials, and could be more susceptible to bias in their design, conduct and analysis. Despite these limitations, pragmatic trials may be a valuable source of information for decisions in the context of healthcare.

The PRECIS-2 tool measures the degree of pragmatism in an RCT by scoring it across 9 domains, ranging from 1 (very explicative) to 5 (very pragmatic). In this study, the recruit-ment, organization, flexibility in delivery and follow-up domains were awarded high scores, however, the primary outcome and the method for missing data fell below the practical limit. This suggests that a trial could be designed with effective pragmatic features, without damaging the quality.

It is hard to determine the level of pragmatism within a specific trial since pragmatism doesn't have a binary attribute. Some aspects of a study may be more pragmatic than other. Additionally, logistical or protocol modifications made during the trial may alter its pragmatism score. Koppenaal and colleagues found that 36% of 89 pragmatic studies were placebo-controlled, or conducted prior to licensing. The majority of them were single-center. This means that they are not as common and can only be described as pragmatic when their sponsors are accepting of the absence of blinding in these trials.

Additionally, a typical feature of pragmatic trials is that the researchers attempt to make their findings more valuable by studying subgroups of the sample. However, this often leads to unbalanced comparisons with a lower statistical power, which increases the chance of not or misinterpreting the results of the primary outcome. This was a problem during the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for covariates that differed at the time of baseline.


In addition, pragmatic studies can present challenges in the collection and interpretation of safety data. This is due to the fact that adverse events are typically reported by participants themselves and are prone to reporting delays, inaccuracies or coding errors. It is crucial to increase the accuracy and quality of the outcomes in these trials.

While the definition of pragmatism may not require that clinical trials be 100% pragmatist there are benefits of including pragmatic elements in trials. These include:

By incorporating routine patients, the results of trials can be translated more quickly into clinical practice. However, pragmatic studies can also have drawbacks. The right type of heterogeneity for instance could allow a study to generalise its findings to many different patients or settings. The original PRECIS tool3 was based on a similar scale and domains. Koppenaal and colleagues10 created an adaptation of this assessment, called the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic reviews scored higher in all domains, but scored lower in the primary analysis domain.

This difference in the primary analysis domain could be explained by the fact that most pragmatic trials process their data in an intention to treat method while some explanation trials do not. The overall score for pragmatic systematic reviews was lower when the domains of organisation, flexible delivery and follow-up were merged.

It is important to remember that a pragmatic study does not necessarily mean a low-quality study. In fact, there is increasing numbers of clinical trials that employ the term 'pragmatic' either in their title or abstract (as defined by MEDLINE however it is not precise nor sensitive). The use of these terms in abstracts and titles could indicate a greater understanding of the importance of pragmatism, however, it is not clear if this is evident in the content of the articles.

Conclusions

As appreciation for the value of real-world evidence becomes increasingly popular, pragmatic trials have gained popularity in research. They are randomized studies that compare real-world alternatives to experimental treatments in development. They include patient populations closer to those treated in regular care. This method can help overcome the limitations of observational research, like the biases that come with the use of volunteers and the lack of the coding differences in national registry.

Pragmatic trials have other advantages, including the ability to leverage existing data sources and a higher chance of detecting significant distinctions from traditional trials. However, they may be prone to limitations that compromise their credibility and generalizability. For instance the participation rates in certain trials might be lower than anticipated due to the healthy-volunteer effect as well as financial incentives or competition for participants from other research studies (e.g. industry trials).
