Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It collects and distributes clean trial data, ratings, and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses that compare treatment effect estimates across trials with different levels of pragmatism.
Background
Pragmatic studies are increasingly acknowledged as providing evidence from the real world for clinical decision making. However, the use of the term "pragmatic" is not uniform and its definition and evaluation requires clarification. Pragmatic trials are designed to guide clinical practices and policy decisions rather than confirm a physiological hypothesis or clinical hypothesis. A pragmatic trial should also strive to be as close to the real-world clinical environment as is possible, including the recruitment of participants, setting up and design of the intervention, its delivery and implementation of the intervention, and the determination and analysis of outcomes as well as primary analyses. This is a significant distinction from explanatory trials (as described by Schwartz and Lellouch1) which are designed to provide more thorough proof of the hypothesis.
The most pragmatic trials should not be blind participants or the clinicians. This can lead to an overestimation of treatment effects. Practical trials also involve patients from various healthcare settings to ensure that the results can be applied to the real world.
Additionally studies that are pragmatic should focus on outcomes that are important for patients, such as quality of life or functional recovery. This is especially important for trials involving invasive procedures or those with potentially serious adverse events. In addition to these characteristics pragmatic trials should reduce trial procedures and data-collection requirements to cut down on costs and time commitments. The creation of a PRECIS-2 tool that can provide an objective and standardized evaluation of pragmatic aspects is a good start.
Methods
In a practical trial, the aim is to inform clinical or policy decisions by demonstrating how the intervention can be implemented into routine care. This is distinct from explanation trials, which test hypotheses about the causal-effect relationship in idealized conditions. In this way, pragmatic trials may have less internal validity than explanatory studies and are more susceptible to biases in their design, analysis, and conduct. Despite these limitations, pragmatic trials may contribute valuable information to decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, ranging from 1 to 5 (very pragmatic). In this study, the recruit-ment organisation, flexibility: delivery and follow-up domains were awarded high scores, however the primary outcome and the procedure for missing data were not at the limit of practicality. This suggests that it is possible to design a trial using excellent pragmatic features without damaging the quality of its results.
However, it's difficult to determine how practical a particular trial is since pragmatism is not a binary attribute; some aspects of a study can be more pragmatic than others. Moreover, protocol or logistic changes during the trial may alter its pragmatism score. In addition 36% of the 89 pragmatic trials identified by Koppenaal and co. were placebo-controlled or conducted prior to licensing, and the majority were single-center. Therefore, they aren't as common and are only pragmatic in the event that their sponsors are supportive of the lack of blinding in these trials.
Another common aspect of pragmatic trials is that the researchers attempt to make their findings more meaningful by analysing subgroups of the trial. This can lead to unbalanced analyses that have lower statistical power. This increases the chance of omitting or misinterpreting differences in the primary outcomes. This was the case in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for covariates that differed at the time of baseline.
Additionally, pragmatic trials can also be a challenge in the collection and interpretation of safety data. This is due to the fact that adverse events are typically reported by participants themselves and are prone to reporting errors, delays or coding errors. It is therefore crucial to enhance the quality of outcomes for these trials, in particular by using national registries instead of relying on participants to report adverse events in a trial's own database.
Results
While the definition of pragmatism may not require that all trials are 100 percent pragmatic, there are advantages to incorporating pragmatic components into clinical trials. These include:
Increasing sensitivity to real-world issues, reducing study size and cost and allowing the study results to be more quickly transferred into real-world clinical practice (by including routine patients). However, pragmatic trials may also have drawbacks. The right kind of heterogeneity for instance could help a study generalise its findings to many different patients or settings. However the wrong type of heterogeneity could reduce the sensitivity of an assay and thus decrease the ability of a study to detect small treatment effects.
A variety of studies have attempted to classify pragmatic trials using a variety of definitions and scoring methods. Schwartz and Lellouch1 created a framework to discern between explanation-based studies that support a physiological or clinical hypothesis, and pragmatic studies that guide the selection of appropriate treatments in clinical practice. The framework was composed of nine domains scored on a 1-5 scale, with 1 being more lucid while 5 being more pragmatic. The domains covered recruitment and setting up, the delivery of intervention, flex compliance and primary analysis.
The original PRECIS tool3 included similar domains and scales from 1 to 5. Koppenaal et. al10 devised an adaptation of this assessment, called the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic reviews scored higher on average across all domains, however they scored lower in the primary analysis domain.
The difference in the main analysis domain could be explained by the fact that most pragmatic trials analyse their data in an intention to treat method while some explanation trials do not. The overall score for systematic reviews that were pragmatic was lower when the domains of organisation, flexible delivery and follow-up were merged.
It is important to remember that a study that is pragmatic does not mean that a trial is of poor quality. In fact, there are an increasing number of clinical trials which use the term 'pragmatic' either in their abstract or title (as defined by MEDLINE but which is neither precise nor sensitive). The use of these terms in abstracts and titles could indicate a greater understanding of the importance of pragmatism, however, it is not clear if this is reflected in the content of the articles.
Conclusions
In recent years, pragmatic trials are becoming more popular in research as the importance of real-world evidence is increasingly recognized. They are randomized studies that compare real-world care alternatives to new treatments that are being developed. They are conducted with populations of patients more closely resembling those treated in regular care. This method can help overcome the limitations of observational research that are prone to limitations of relying on volunteers, and the limited availability and the variability of coding in national registries.
Pragmatic trials offer other advantages, including the ability to use existing data sources and a higher likelihood of detecting meaningful differences than traditional trials. However, these trials could be prone to limitations that compromise their credibility and generalizability. For instance the participation rates in certain trials may be lower than anticipated due to the healthy-volunteer effect and financial incentives or competition for participants from other research studies (e.g., industry trials). Practical trials are often limited by the need to recruit participants quickly. Practical trials aren't always equipped with controls to ensure that observed differences aren't caused by biases that occur during the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatic and that were published from 2022. The PRECIS-2 tool was used to determine pragmatism. It includes areas such as eligibility criteria and flexibility in recruitment as well as adherence to interventions and follow-up. They discovered that 14 of the trials scored highly or pragmatic sensible (i.e. scores of 5 or higher) in any one or more of these domains and that the majority of these were single-center.
Studies with high pragmatism scores are likely to have broader criteria for eligibility than conventional RCTs. They also include populations from various hospitals. The authors argue that these traits can make the pragmatic trials more relevant and applicable to everyday practice, but they do not necessarily guarantee that a pragmatic trial is completely free of bias. Furthermore, the pragmatism of trials is not a fixed attribute A pragmatic trial that doesn't have all the characteristics of a explanatory trial can yield valid and useful results.
