This work provides a new and facile approach for the synthesis of high-quality multicomponent TMD nanosheets with precise process control, the potential for mass production, and outstanding performance, providing great promise in cancer theranostics. The formation of a G-quadruplex motif in the promoter region of the c-MYC protooncogene prevents its expression. Accordingly, G-quadruplex stabilization by a suitable ligand may be a viable approach for anticancer therapy. In our study, we used the 4-(4-methylpiperazin-1-yl)aniline molecule, previously identified as a fragment library screen hit, as a template for the SAR-guided design of a new small library of clickable fragments and subjected them to click reactions, including kinetic target-guided synthesis in the presence of a G-quadruplex forming oligonucleotide Pu24. We tested the clickable fragments and products of click reactions for their G-quadruplex stabilizing activity and determined their mode of binding to the c-MYC G-quadruplex by NMR spectroscopy. The enhanced stabilizing potency of click products in biology assays (FRET, Polymerase extension assay) matched the increased yields of in situ click reactions. In conclusion, we identified the newly synthesized click products of bis-amino derivatives of 4-(4-methylpiperazin-1-yl)aniline as potent stabilizers of c-MYC G-quadruplex, and their further evolution may lead to the development of an efficient tool for cancer treatment. The paper presents a study of the effect of a constant magnetic field (CMF) on the basic processes of quercetin electrochemical reactions. According to the observation made in previous studies, the presence of a double bond in the C-ring of quercetin enhances the antioxidant properties of that compound, whereas the presence of -OH groups also affects the antioxidant properties. Using cyclic voltammetry it was found that the constant magnetic field improves the efficiency of quercetin electrooxidation, especially of the third stage of the process, i. e. the stage in which the oxidation of the OH groups in the A-ring is the most difficult. The use of HPLC confirmed the electrochemical measurements and the results of cyclic voltammetry studies. The beneficial effect of the magnetic field on the efficiency of quercetin oxidation was confirmed by the results of impedance spectroscopy measurements. The chemical recycling of poly(lactide) was investigated based on depolymerization and polymerization processes. Using methanol as depolymerization reagent and zinc salts as catalyst, poly(lactide) was depolymerized to methyl lactate applying microwave heating. An excellent performance was observed for zinc(II) acetate with turnover frequencies of up to 45000 h-1. In a second step the monomer methyl lactate was converted to (pre)poly(lactide) in the presence of catalytic amounts of zinc salts. Here zinc(II) triflate revealed excellent performance for the polymerization process (yield 91 %, Mn ∼8970 g/mol). Moreover, the (pre)poly(lactide) was depolymerized to lactide, the industrial relevant molecule for accessing high molecular weight poly(lactide), using zinc(II) acetate as catalyst.Invited for this month's cover is the group of Stephan Enthaler at the University of Hamburg (Germany). The cover picture shows the general structure of chemical recycling processes. In more detail, the chemical recycling of end-of-life polymers (plastics) is subdivided into two linked processes. The depolymerization allows the conversion of the end-of-life polymer to monomers, which can be reused in the polymerization step for the synthesis of the polymer. By doing so advantages with respect to resource-efficiency and sustainability can be created. In our manuscript we applied the chemical recycling concept via depolymerization-polymerization for poly(lactide). Imidazolyl, benzimidazolyl, pyrazolyl, pyrrolyl, 1,3-dioxoisoindolyl and pyridinium groups were taken into account as heterocyclic units, attached to the aromatic skeleton via -CH2-, -CH2NHCH2- or -CH2N=CH- linkers. In addition to the seventeen 2,4,7-trisubstituted 9,9-diethylfluorenes, two macrocyclic compounds were prepared on the basis of 2,7-bis(aminomethyl)-9,9-diethylfluorene. The excellent yield of the macrocyclization reaction is worth a special mention. Both the acyclic and the macrocyclic fluorene-based compounds have, among other things, the potential to act as artificial receptors for different substrates in analogy to the known receptors consisting of a benzene or biphenyl core.[This corrects the article DOI 10.1002/jgf2.310.].Hospital integration among rural districts to concentrate medical resources is one of the main projects of the Japanese government. In advance of this, we experienced hospital integration and screening for chronic kidney disease as definitive risk for end-stage kidney disease and cardiovascular mortality. After that, high-risk patients have been appropriately referred from generalists to nephrologists and/or cardiologists without acute deterioration of renal function or cardiac sudden death.
In Japan, even if physicians have not experienced surgical training, they face many instances in which they must provide initial surgical treatment, especially during off-hours. This study aimed to identify the frequency and fields of commonly encountered problems in a Japanese emergency department.

A retrospective review was performed to identify walk-in outpatients with exogenous problems visiting during off-hours in the Japanese educational hospital providing primary to tertiary emergency care between January 1 and December 31, 2014. Diseases were aggregated according to International Classification of Primary Care (Second Edition; ICPC-2).

During the study period, 33424 patients visited and 7476 were classified into the "exogenous" group. We analyzed the data of 7421 patients after excluding 55 who were deemed undiagnosable based on reviews of the charts. The median age of patients who visited the ED during off-hours was 29years (range 0-101years, IQR 8-60years). Altogether, 226 types of problems included in ICPC-2 were identified during the study period. The majority fields of exogenous problems were 'skin,' 'Musculoskeletal,' and 'eye.' The 15 problems with the highest frequencies accounted for 50.2% of the total problems.

We identified surgical problems with high treatment frequencies among patients visiting the ED during off-hours. Providing education focusing on these frequent surgical problems can help to improve the initial treatment quality and reduce the anxiety for those doctors who provide initial surgical treatment.

In this article, we report chronological changes in strawberry tongue in TSST-1-mediated exanthematous disease. We present a case of diffuse aspiration bronchiolitis (DAB) with a false-positive Gaffky score. "Tree-in-bud" opacities detected after aspiration should be considered DAB rather than mycobacterial infection.A 51-year-old afebrile man visited the hospital with acute severe neck pain with stiff neck, but cervical CT showed no calcification around the odontoid process. Cervical MRI revealed prevertebral hyperintense area of edema at the C1-C5 vertebral bodies, providing the diagnosis of retropharyngeal calcific tendinitis. The patient was spared for lumbar puncture, and his symptoms swiftly disappeared only with oral NSAIDs.A 27-year-old woman complaining of persistent fever and sore throat was consulted. Examination of the laryngopharynx revealed multiple aphthous ulcers in the uvula, arytenoids, epiglottis, and laryngeal pyriform fossa. Crohn's disease was diagnosed by colonoscopy.A 61-year-old Japanese man presented with a 2-month history of colicky abdominal pain and watery diarrhea. He had begun organic farming using cattle manure 1 year previously.Adult-onset Still's disease can develop valve lesions. Since AOSD may complicate valvular lesion, differentiation of endocarditis in patients with AOSD is required.A 66-year-old man with a 2-year history of suspected scalp eczema with excessive dandruff developed painful swollen joints in the extremities. Four months after developing polyarthritis and polydactylitis, eczema gradually spread to the face. He was referred to our hospital for intractable scalp and facial eczema and polyarthritis. Based on the appearance of the head and facial skin lesions, psoriasis was suspected. Treatment with apremilast (a phosphodiesterase-4-inhibitor) was initiated, which swiftly alleviated the skin lesions. The joint deformities persisted, but the pain in the joints disappeared. This case implies that psoriatic arthritis should be suspected even if psoriatic skin lesions are localized to the scalp. In this report, we present a case in which long-term swelling and pain because of an ankle sprain were successfully treated with the traditional Japanese herbal (Kampo) medicine Jidabokuippo. Jidabokuippo was created in Japan and has been used to treat swelling and pain associated with trauma. A 44-year-old woman sprained her right ankle and received the standard treatments including icing and immobilization for three weeks. However, the swelling, redness, and pain of her ankle continued for two months after the treatments. After initiating Jidabokuippo, her pain and swelling were promptly improved. This suggests that Jidabokuippo is a potentially promising pharmacotherapy for patients with ankle sprain which has not recovered smoothly. It is said that Jidabokuippo can be prescribed simply referring to pain and swelling of the affected areas; therefore, it should be considered as a treatment for trauma patients with long-term swelling and pain.A 62-year-old woman presented with a dry cough lasting 18 months. She had previously been examined by multiple doctors, but no abnormalities were observed. Several medications such as rabeprazole and inhaled corticosteroids were administered as test treatments without any improvement. Therefore, the possibility of biological disease, including acid reflux, had been mistakenly ruled out. Selleck GSK-3 inhibitor We examined the sputum gram stain. The result showed phagocyted normal bacterial flora, suggesting aspiration. Laryngoscopy revealed edema of the arytenoid cartilage. The patient was finally diagnosed with laryngopharyngeal reflux and silent aspiration. This case suggested that the ineffectiveness of proton-pump inhibitors cannot always exclude the presence of reflux disease and the usefulness of gram stain examination to detect silent aspiration.
